Book excerpt: Oral diseases continue to be a global public health problem, with more than 3.5 billion people affected. With the COVID-19 pandemic declaration, dental services have been severely disrupted. This unprecedented situation has forced organizations such as the World Health Organization (WHO), American Dental Association (ADA), and Centers for Disease Control and prevention (CDC) to develop protocols and guidelines to improve and ensure the reduction of risks for dental health care personnel (DHCP) and to protect the patients in need of emergency treatment or urgent dental care. Ministries of Health and local dental organizations have also created policies for reopening dental services. In Latin America, dental health professionals face new challenges to provide treatment and care. This study aims to describe and compare the policies from Latin America providing guidelines to dental services during COVID-19. The aims of the analysis were to 1) describe and compare the policies to manage the reopening of dental services; 2) identify specific differences in the policies to respond during the COVID-19 pandemic in countries with a high incidence of COVID-19 in Latin America, and 3) identify which international guidelines or pre-existing policies influenced the development of policies in the countries described. Methods: We conducted the comparative and descriptive policy analysis of dental policies for the reopening of dental settings during the COVID-19 pandemic, including online searches of dental policies in countries with a high incidence of COVID-19 cases in Latin America, published from March 1st to July 31st of 2020, with free online access. The countries included in this research were: Brazil, Peru, Chile, Colombia, and Argentina. We used the WHO's interim guidance Considerations for the provision of essential oral health services in the context of COVID-19 to develop a policy taxonomy for COVID-19 prevention policies for dental services comprising five domains. Findings from the research showed some key similarities and differences between the five countries' policies from South America. Overall, these policies provide appropriate guidance to reduce infection risk to DHCP and to protect the patients in need of emergency services. More research to assess the implementation and effectiveness of these dental polices is necessary.

Book excerpt: Background: COVID-19 reached Latin-American countries slightly later than European countries, around February/March, allowing some emergency preparedness response in countries characterized by low health system capacities and socioeconomic disparities. Objective: This paper focuses on the first months of the pandemic in five Latin American countries: Brazil, Chile, Colombia, Ecuador and Peru. It analyses how the pre-pandemic context, and the government's responses in terms of containment and mitigation and economic measures have affected the COVID-19 health outcomes. Methods: Extensive qualitative document analysis was conducted focused on publicly-available epidemiological data and federal and state/regional policy documents since the beginning of the pandemic. Results: The countries were quick to implement stringent COVID-19 measures and incrementally scaled up their health systems capacity, although tracing and tracking have been poor. All five countries have experienced a large number of cases and deaths due to COVID-19. The analysis on the excess deaths also shows that the impact in deaths is far higher than the official numbers reported to date for some countries. Conclusion: Despite the introduction of stringent measures of containment and mitigation, and the scale up of health system capacities, pre-pandemic conditions that characterize these countries (high informal employment, and social inequalities) have undermined the effectiveness of the countries' responses to the pandemic. The economic support measures put in place were found to be too timid for some countries and introduced too late in most of them. Additionally, the lack of a comprehensive strategy for testing and tracking has also contributed to the failure to contain the spread of the virus.#

Book excerpt: Over the past three decades, many countries of Latin America and the Caribbean have recognized health as a human right. Since the early 2000s, 46 million more people in the countries studied are covered by health programs with explicit guarantees of affordable care. Reforms have been accompanied by a rise in public spending for health, financed largely from general revenues that prioritized or explicitly target the population without capacity to pay. Political commitment has generally translated into larger budgets as well as passage of legislation that ring-fenced funding for health. Most countries have prioritized cost-effective primary care and adopted purchasing methods that incentivize efficiency and accountability for results, and that give stewards of the health sector greater leverage to steer providers to deliver on public health priorities. Evidence from the analysis of 54 household surveys corroborates that investments in extending coverage are yielding results. Though the poor still have worse health outcomes than the rich, disparities have narrowed considerably - particularly in the early stage of the life course. Countries have reached high levels of coverage and equity in utilization of maternal and child health services; coverage of noncommunicable disease interventions is not as high and service utilization is still skewed toward the better off. Catastrophic health expenditures have declined in most countries; the picture regarding equity, however, is mixed. While the rate of impoverishment owing to health-care expenditures is low and generally declining, 2-4 million people in the countries studied still fall below the poverty line after health spending. Efforts to systematically monitor quality of care in the region are still in their infancy. Nonetheless, a review of the literature reveals important shortcomings in quality of care, as well as substantial differences across subsystems. Improving quality of care and ensuring sustainability of investments in health remain an unfinished agenda.

We study the effect of attributing health recommendations to experts on agreement using a series of large survey experiments in twelve Latin American countries. We document a robust backlash against experts: agreement with recommendations is lower when these are phrased as originating from an expert. This backlash is only present for the recommendations that are specific to the pandemic and does not depend on the type of expert (academic, public or private sector). For individuals initially reporting low levels of trust in experts, agreement with all recommendations is lower when they are attributed to experts as compared to individuals who trust experts, indicating that anti-intellectualism plays a role in expert backlash but cannot fully explain the differential pattern of expert backlash that we observe across recommendations. We find suggestive evidence that individual traits and perceptions of social pressure could contribute to these backlash effects.

Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
