Book excerpt: The main aim of this practical Handbookis to strengthen counselling and communication skills of skilled attendants (SAs) and other health providers, helping them to effectively discuss with women, families and communities the key issues surrounding pregnancy, childbirth, postpartum, postnatal and post-abortion care. Counselling for Maternal and Newborn Health Careis divided into three main sections. Part 1 is an introduction which describes the aims and objectives and the general layout of the Handbook. Part 2 describes the counselling process and outlines the six key steps to effective counselling. It explores the counselling context and factors that influence this context including the socio-economic, gender, and cultural environment. A series of guiding principles is introduced and specific counselling skills are outlined. Part 3 focuses on different maternal and newborn health topics, including general care in the home during pregnancy; birth and emergency planning; danger signs in pregnancy; post-abortion care; support during labor; postnatal care of the mother and newborn; family planning counselling; breastfeeding; women with HIV/AIDS; death and bereavement; women and violence; linking with the community. Each Session contains specific aims and objectives, clearly outlining the skills that will be developed and corresponding learning outcomes. Practical activities have been designed to encourage reflection, provoke discussions, build skills and ensure the local relevance of information. There is a review at the end of each session to ensure the SAs have understood the key points before they progress to subsequent sessions.

Book excerpt: Abstract: A breastfeeding education program for mothers participating in the Supplementary Feeding Program for Women, Infants, and Children (WIC) sought to increase both the number of mothers choosing to breastfeed and the number of mothers who would breastfeed exclusively for 3 or more months. The program focused on a method for supporting low-income women in maintaining lactation. The education program results indicated that current support protocols for breastfeeding women in the maternity service are inadequate and may be counterproductive to enhancing the decision to breastfeed. In this study, however, at 30-day follow-up, a significantly higher proportion of women who were provided breastfeeding education materials that were developed in the program were still breastfeeding compared to women who were not given such materials. Book excerpt: The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) began 40 years ago as a pilot program and has since grown to serve over 8 million pregnant women, and mothers of and their infants and young children. Today the program serves more than a quarter of the pregnant women and half of the infants in the United States, at an annual cost of about $6.2 billion. Through its contribution to the nutritional needs of pregnant, breastfeeding, and post-partum women; infants; and children under 5 years of age; this federally supported nutrition assistance program is integral to meeting national nutrition policy goals for a significant portion of the U.S. population. To assure the continued success of the WIC, Congress mandated that the Food and Nutrition Service of the U.S. Department of Agriculture (USDA) reevaluate the program's food packages every 10 years. In 2014, the USDA asked the Institute of Medicine to undertake this reevaluation to ensure continued alignment with the goals of the Dietary Guidelines for Americans. This, the second report of this series, provides a summary of the work of phase I of the study, and serves as the analytical underpinning for phase II in which the committee will report its final conclusions and recommendations.

Available in PDF, EPUB and Kindle. Book excerpt: The current recommendation by health care professionals for breastfeeding is as follows: infants should be exclusively breastfed for at least six months, followed by breastfeeding in combination with complementary solid foods for at least twelve month of age. However, according to available data, 70 percent of infants were breastfed at postpartum, 32 percent at 6 months, and 16 percent at 12 months of age in the United States. Healthy People 2010 proposed goals for breastfeeding are 75 percent at birth, 50 percent at 6 months and 25 percent at 12 months. With these statistics in mind, it is important for healthcare professionals to encourage breastfeeding and promote breastfeeding education in order to increase the number of women who will initiate breastfeeding and continue to do so for the recommended time frame. Currently, there is little research available specifically on the role of nutrition education and its incorporation into breastfeeding education. There is little involvement from clinical dietitians in the promotion of breastfeeding as well. Most breastfeeding or lactation classes are taught by either a lactation consultant or other trained professional. It is not uncommon that registered dietitians become certified lactation consultants, but dietitians still represent a small percentage of trained professionals who teach lactation classes. Therefore, there is no current focus on nutrition and lactation within existing breastfeeding classes. The focus of existing breastfeeding education classes is on such topics as the benefits to both mother and baby, anatomy and physiology of breastfeeding, nutrition equipment and technical issues such as latching, positioning, and barrier and problem solving. The purpose of this study was to develop, implement, and evaluate an intervention to increase the likelihood of pregnant mothers to breastfeed their infants up to at least 6 months to one year. A nutrition education component was added to an existing breastfeeding class taught by a lactation consultant in this study. After participating in the breastfeeding education session including a nutrition component, the participants are expected to demonstrate an increase in knowledge about the nutritional benefits of breastfeeding as measured by their comparative scores on a pre-test and post-test. Nine pregnant women participated in this study. A 10 question pre-test was distributed before the class to assess the participants' base knowledge in lactation nutrition and the benefits of breastfeeding. At the completion of the existing breastfeeding class, the researcher provided written materials as well as a 20 minute presentation on lactation nutrition and the benefits of breastfeeding. Following the presentation, the participants completed a 10 question post-test to determine the effectiveness of the education in increasing their knowledge. Data analysis of the test results were developed and the mean scores were calculated. All nine participants either showed improvement or no change when comparing their pre-test and post-test scores. The pre-test score was 90% and the mean post-test score was 90% as well. The average pre-test score was 88.8% and the average post-test score was 92.2%. The results supported the hypothesis that adding a nutrition education portion to the breastfeeding class will increase the participants' lactation nutrition knowledge and was shown by the improvement in their scores. Increasing this population's knowledge increases their likelihood of initiating breastfeeding and its duration. Therefore, adding nutrition education to breastfeeding classes can assist in increasing the number of breastfeeding women towards meeting the Healthy People 2010 goals. This study shows the impact of increasing breastfeeding and therefore increasing infant health outcomes which encourages future research in this area.
