As usual, some historical precursors of this approach can be d- cerned in the scientific literature of the last decades. However, the present state of affairs appears to date from observations made discreetly in 1988 by Wolff, Malone, Felgner, and colleagues, which were described in a 1989 patent and published in 1990. Quite surprisingly, they showed that genes carried by pure plasmid DNA and injected in a saline solution, hence the epithet “naked DNA,” could be taken up and expressed by skeletal muscle cells with a low but reproducible frequency. Such a simple methodology was sure to spawn many applications. In a separate and important line of experimentation, Tang, De Vit, and Johnston announced in 1992 that it was indeed possible to obtain humoral immune responses against proteins encoded by DNA delivered to the skin by a biolistic device, which has colloquially become known as the “gene gun. ” The year 1993 saw the publication of further improvements in the me- ods of naked DNA delivery and, above all, the first demonstrations by several groups of the induction of humoral and cytotoxic immune responses to viral antigens expressed from injected plasmid DNA. In some cases, protection against challenge with the pathogen was obtained. The latter result was - questionably the touchstone of a method of vaccination worthy of the name.

The research plays a critical role in molecular targeted therapies directed at altering the kynurenine pathway of tryptophan metabolism. The initial and rate-limiting step of tryptophan metabolism is mediated by one of two enzymes, tryptophan-2,3-dioxygenase (TDO; predominantly in the liver, but also in the brain) and indoleamine-2,3-dioxygenase (IDO; in a host of tissues in response to immune activation). Targeting the enzymes IDO and TDO, as well as other downstream effectors would therefore be likely to generate novel treatment options that would be helpful in a wide variety of clinical settings. Book excerpt: The burden of cutaneous drug reactions is significant, in both outpatient and inpatient settings, and can result in morbidity and even mortality. This book is unique in its approach to this problem. This text is divided into basic principles, common drug reactions, skin conditions mimicked by drug reactions, drug reactions to the skin appendages, life-threatening drug reactions, less common drug reactions, and special groupings of drug reactions. For the clinician, the skin can only morphologically react in to many limited ways. This is also true for the pathologist. Combining these two forever linked specialties is a synergistic paradigm that greatly enhances diagnosis, and ultimately therapy, for these pernicious conditions. Drug reactions in the skin remain a common complication of therapy. True incidences of drug reactions are not available. For general discussion, the rule of 3% can be applied with reasonable assuredness. Approximately 3% of all hospitalized patients develop an adverse cutaneous drug reaction. Approximately 3% of these reactions are considered severe. Outpatient data is even more obscure, but at least 3% of dermatology clinic outpatient visits are due to a drug reaction. Cutaneous drug reactions compromise approximately 3% of all drug reactions. Even more challenging is the fact that the most vulnerable populations to drug reactions are increasing and include the elderly patients on prolonged drug therapy, and patients that use multiple drugs at the same time.
