Book excerpt: This colposcopy manual was developed in the context of the cervical cancer screening research studies of the International Agency for Research on Cancer (IARC) and the related technical support provided to national programs. It is thus a highly comprehensive manual, both for the training of new colposcopists and for the continuing education and reorientation of those who are more experienced. This manual offers a valuable learning resource, incorporating recent developments in the understanding of the etiology and pathogenesis of cervical intraepithelial neoplasia (CIN), as well as in colposcopy and cervical pathology. Expertise in performing satisfactory, safe, and accurate colposcopic examinations requires high competence in the technical, interpretive, and cognitive aspects, and the capability to develop pragmatic and effective management plans and treatment. This comprehensive and concise manual covers all these aspects and serves as a useful handbook for acquiring the necessary skills for the visual recognition and interpretation of colposcopic findings and for developing the personal and professional attributes required for competence in colposcopy.

Book excerpt: This book offers clear, up-to-date guidance on how to report cytologic findings in cervical, vaginal and anal samples in accordance with the 2014 Bethesda System Update. The new edition has been expanded and revised to take into account the advances and experience of the past decade. A new chapter has been added, the terminology and text have been updated, and various terminological and morphologic questions have been clarified. In addition, new images are included that reflect the experience gained with liquid-based cytology since the publication of the last edition in 2004. Among more than 300 images, some represent classic examples of an entity while others illustrate interpretative dilemmas, borderline cytomorphologic features or mimics of epithelial abnormalities. The Bethesda System for Reporting Cervical Cytology, with its user-friendly format, is a “must have” for pathologists, cytopathologists, pathology residents, cytotechnologists, and clinicians.

Available in PDF, EPUB and Kindle. Book excerpt: Most women who die from cervical cancer, particularly in developing countries, are in the prime of their life. They may be raising children, caring for their family, and contributing to the social and economic life of their town or village. Their death is both a personal tragedy, and a sad and unnecessary loss to their family and their community. Unnecessary, because there is compelling evidence, as this Guide makes clear, that cervical cancer is one of the most preventable and treatable forms of cancer, as long as it is detected early and managed effectively. Unfortunately, the majority of women in developing countries still do not have access to cervical cancer prevention programmes. The consequence is that, often, cervical cancer is not detected until it is too late to be cured. An urgent effort is required if this situation is to be corrected. This Guide is intended to help those responsible for providing services aimed at reducing the burden posed by cervical cancer for women, communities and health systems. It focuses on the knowledge and skills needed by health care providers, at different levels of care.

Available in PDF, EPUB and Kindle. Book excerpt: Various scientific evaluations taken from these chapters were presented during the international symposium, 'Carcinoma of the Cervix,' which was held from September 4-7, 1980 in Kiawah Island, Charleston, South Carolina, U.S.A. An international selection of research investigators have contributed reviews designed to be informative to medical, graduate and post-graduate students, as well as clinicians, oncologists and investigators working in the area of female carcinoma. An attempt has been made to provide a total coverage of current progress in carcinoma of the cervix. In particular, the following major areas are included: Epidemiology, Histology & Histochemistry, Ultra structure, Physiology & Biochemistry, Genetics & Cytogenetics, Immunology, Diagnosis, and Patho physiology & Complications. Major changes have occurred in incidence and mortality rates for carcinoma of the cervix in the past decade. Mass screening programs and their ability to detect earlier stage diseases have attributed to a reduction in invasive diseases. Most cervical cancers could be prevented by an extension of cytologic screening programs in high risk areas. Greater resources must be provided to reach high risk areas in order to prevent the occurrence of cervical cancer.

Available in PDF, EPUB and Kindle. Book excerpt: Cytological screening for the identification of intraepithelial neoplasia of the cervix as a precursor lesion for cervical cancer has been well established as an effective means for decreasing the incidence of invasive carcinoma. Despite these screening efforts, carcinoma of the cervix remains one of the more common malignancies in women and it is the leading cause of cancer death in many countries in the western hemisphere. It is estimated that in 1986 there will still be 14,000 new cases of invasive cancer, with 6,800 deaths in the United States alone. Unfortunately, many of these patients present with advanced disease, posing difficult management problems for the clinician responsible for their care. The treatment of early stage invasive carcinoma of the cervix (lesions confined to the cervix and vagina) remains either radical surgery, radical radiation therapy or a combination thereof This approach is extraordinarily effective in the vast majority of patients. However, there remains a subset of patients with early stage disease that are at high risk for recurrence. Dr Kjorstad (Chapter 2) has identified adenocarcinomas and adenosqua mous carcinomas as having a particularly poor prognosis. In addition, patients with more than three positive lymph nodes or with involvement of lymph nodes outside of the pelvis have a very poor prognosis. He has iden tified the CEA as a potentially predictive marker for these patients with poor prognosis, especially in patients with adenocarcinomas.

Book excerpt: Recent introduction of HPV vaccines has raised hopes for immunization against cervical cancer and for the first time in the history of humanity for eradication of one malignant disease. This new “opportunity” has changed many current views on cervical cancer prevention, control diagnosis and treatment. Many canons and guidelines became subject of review and many revisions are coming. This book is intended to summarize most of these events and to present them to all women in a language understandable by the eneral public. We expect the book will bring all readers the rationale for optimism and will provide guidance as how to gain knowledge and skills for critical thinking and making an educated decision when it will be necessary in their lives.
