Book excerpt: Context: Hydration has been a heavily researched area within the athletic population, but little research has been done on the effect of hydration education and its effect on hydration status with an emphasis on long term retention rate. Research has been conducted on short-term retention rate, but no studies have looked longer periods (>2 weeks) or the use of a text message intervention. This study was designed to fill those gaps of a longer retention period with multiple testing periods and reminders. Objective: The purpose of this study was to research the effect of a repeated social media hydration education intervention on a healthy, active general population. A secondary purpose was to determine if the education would have an impact on hydration practices, knowledge, and attitudes and retention of hydration information. We hypothesized that the participants would not be euhydrated when performing a urinalysis at baseline, but that the intervention will improve hydration status. Design: This study was a cohort, repeated measure study with the use of a hydration questionnaire and a collection of 3 urine samples over the course of 2 weeks. Participants: Thirty-six healthy, active (1-8 hours of exercise/week) participants between ages 18-31. Intervention: An intervention and control group was used for this study. The intervention group received text messages twice a day about proper hydration strategies and protocols while the control participants received text messages twice a day that were unrelated to hydration such as athletic events happening on campus. The data were analyzed pre-vs-post and correlated with the behaviors and practices of hydration through the use of ANOVA’s and correlations. Setting: The participants collected their urine sample upon waking in the morning and brought the sample into the lab on day 1, day 3, and day 14 for urine specific gravity analysis with a refractometer. Main Outcome Measures: Urine specific gravity was obtained via urinalysis with a refractometer. The knowledge of hydration was assed using a validated questionnaire. Results: No statistical significance was found between groups in this study for the effect of a repeated educational intervention on hydration status (P=0.516). There was also no statistical significance found when comparing hydration knowledge over the course of the 2 weeks (P=0.501). Conclusions: Education alone does not work in changing hydration status. Healthcare professionals should be aware that it may take a long time to change patients’ practices regarding hydration and to be constant and consistent with their education if they do want to help patients become better hydrated.

Book excerpt: There is increasing evidence of athletes entering their training sessions in an already dehydrated state. It is crucial for athletes to properly replenish fluid and electrolytes lost via sweat in order to optimize their training and recovery. The purpose of this study is to assess the effect of a text-message based hydration education intervention on the knowledge and hydration status of division I female collegiate soccer players.A total of 14 participants completed baseline testing, which included hydration knowledge and behavior questionnaires, and pre- and post-practice weights to calculate % weight change (%WC). The participants also provided a baseline urine sample pre- and post-practice to assess urine specific gravity (USG). A total of 18 text messages were sent during the 6-week intervention phase, with 3 text messages sent per week. The text messages contained educational information on hydration topics. Post-questionnaires were administered after the intervention online, via Qualtrics. Post-intervention USG and weight assessments were not able to be collected due to the COVID-19 pandemic. The averages of pre-training USG (M=1.02, SD= .01) and post-training USG (M= 1.02, SD= .01), did not change from before to after a practice session. There were no significant correlations between pre- training USG and pre- total hydration knowledge scores (r= -.274, p>.05), post- training USG and pre- total knowledge scores (r= .011, p >.05), and %WC and pre- total scores (r= -.184,p>.05). Of the pre-knowledge questionnaire questions there was a significant, moderate, positive relationship between USG and the true/false question asking, 'An athlete should consume 7-10 fl oz of water or sports drink a couple hours before exercise' (r= 0.55, p=.04). There was a numerical increase in the total scores when comparing post questionnaire total scores (M=84.62, SD=6.05) to the pre-questionnaire total scores (M= 79.91, SD=9.55) but it did not reach statistical significance (p=0.14). This study showed that text message-based forms of communication are a feasible intervention that could show a positive improvement in both hydration status and hydration knowledge of athletes. The significance of this study is unclear, and the full impact of this intervention was unable to be determined. More research needs to be gathered on this topic.

Book excerpt: It is widely documented and accepted that athletes have difficulty maintaining adequate hydration status and that dehydration is a key risk factor for the heat-related illnesses commonly observed among athletes. Research has also suggested that hydration status can influence cognitive performance. Educational interventions focused on rehydration strategies have had minimal success reducing dehydration rates; hence, alternative interventions promoting adequate hydration status in athletes should be explored. This trial examined the efficacy of a commercial hydration mobile application (app) for reducing dehydration rates in campus athletes. Fifty-eight college students aged 18-40 y, who participated in club-level collegiate athletics were recruited from a large Southwestern university and randomized by team to one of two study arms, the Standard of Care -- Education (EDU) or the hydration mobile app (APP), to determine if app technology improved hydration status as compared to traditional education messaging. Twenty-three (79%) in the EDU group and twenty (69%) in the APP group were mildly-dehydrated at baseline based on the three-day averages of hydration assessment (USG 1.010). Moreover, 31% (n=9) and 28% (n=8) of the EDU and APP groups, respectively, were dehydrated (USG 1.020). No significant differences were found between the EDU and APP groups following the intervention. Three-day average post-intervention USG testing showed 76% (n=22) and 72% (n=21) of the EDU and APP groups respectively were at best mildly-dehydrated. Additionally, 28% (n=8) and 17% (n=5) were considered dehydrated. Neither intervention improved hydration status after four weeks of treatment. Book excerpt: Skipping breakfast and poor breakfast choices are common among male adolescent athletes. This can lead to nutrient deficiencies and have negative impacts on growth and development. Nutrient deficiencies can also lead to a decrease in athletic performance. This study analyzed the effects of skipping breakfast/breakfast choice on daily energy level and the nutrition knowledge of male adolescent high school varsity basketball players using a pretest/posttest model. Eleven subjects from a private high school in Haverford, Pennsylvania recorded their daily breakfast intake, if any, and their self-assessed daily energy level. The subjects also answered two nutrition knowledge questions specific to them as athletes. All eleven subjects initially ate breakfast and reported a mean energy score of 6.09. Two ate a nutritious breakfast which (foods that were not high in sugar and/or saturated fat). Two answered the first nutrition question correctly, and nine answered the second question correctly. After the intervention, all ten subjects (one subject failed to attend) ate breakfast and reported a mean energy level of 7.2. Six ate a nutritious breakfast. Five out of the six subjects who improved the nutritional quality of their breakfast choice from the pretest also reported a higher energy level. All subjects answered the first nutrition question correctly, while eight answered the second nutrition question correctly. Three weeks after the posttest, the follow-up revealed that nine out of ten subjects (one subject failed to attend) ate breakfast and reported a mean energy level 7.7. Seven ate a nutritious breakfast. Five out of the seven subjects who improved the nutritional quality of their breakfast choice reported a high energy level measured pretest to follow-up. All subjects answered the first nutrition question correctly while nine answered the second question correctly during the follow-up. These results indicate that there is a strong correlation between breakfast choice and daily energy level. This study also showed that subjects demonstrated an increase in nutrition knowledge as it pertains to breakfast consumption. The results of this study may be utilized by local schools as an outline to develop and implement a sports specific nutrition education program for all sports teams. The results may also add to the literature as a guide for future nutrition professionals seeking to improve athletic performance in high schools sports via healthy food choices. This study could be improved if team members were involved in the nutrition education of the subjects, since peer-led nutrition education programs have been known to be effective. It would also be useful to increase the number of subjects and involve multiple in season sports teams to get a clearer picture of the overall food choice behavior of male adolescent athletes. A more comprehensive program involving multiple schools from different areas would address the limited generalizability of this study. Eating healthy goes far beyond high school athletics. Establishing a foothold on the importance of everyday nutrition during adolescence is vital to having a firm grip on the understanding of overall health later in life.

Despite growing awareness of sports-related concussions and campaigns to educate athletes, coaches, physicians, and parents of young athletes about concussion recognition and management, confusion and controversy persist in many areas. Currently, diagnosis is based primarily on the symptoms reported by the individual rather than on objective diagnostic markers, and there is little empirical evidence for the optimal degree and duration of physical rest needed to promote recovery or the best timing and approach for returning to full physical activity. Sports-Related Concussions in Youth: Improving the Science, Changing the Culture reviews the science of sports-related concussions in youth from elementary school through young adulthood, as well as in military personnel and their dependents. This report recommends actions that can be taken by a range of audiences - including research funding agencies, legislatures, state and school superintendents and athletic directors, military organizations, and equipment manufacturers, as well as youth who participate in sports and their parents - to improve what is known about concussions and to reduce their occurrence. Sports-Related Concussions in Youth finds that while some studies provide useful information, much remains unknown about the extent of concussions in youth; how to diagnose, manage, and prevent concussions; and the short- and long-term consequences of concussions as well as repetitive head impacts that do not result in concussion symptoms. The culture of sports negatively influences athletes' self-reporting of concussion symptoms and their adherence to return-to-play guidance. Athletes, their teammates, and, in some cases, coaches and parents may not fully appreciate the health threats posed by concussions. Similarly, military recruits are immersed in a culture that includes devotion to duty and service before self, and the critical nature of concussions may often go unheeded. According to Sports-Related Concussions in Youth, if the youth sports community can adopt the belief that concussions are serious injuries and emphasize care for players with concussions until they are fully recovered, then the culture in which these athletes perform and compete will become much safer. Improving understanding of the extent, causes, effects, and prevention of sports-related concussions is vitally important for the health and well-being of youth athletes. The findings and recommendations in this report set a direction for research to reach this goal.

The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed.
