Since consumer-driven health plans (CDHPs) were first offered in 2001, estimates of how many workers would be enrolled in these plans have varied widely. The U.S. Treasury Department has predicted that 25-30 million people would be covered by a health savings account (HSA)-eligible plan and would have an account by 2010. The percentage of employers offering CDHPs has gone from virtually none in 2000 to 12 percent in 2009. When examining the entire employment-based market, what had been steady growth in offer rates appears to have slowed. Between 2008 and 2009, the percentage of small firms (3-199 workers) that offered a CDHP declined from 13 percent to 11 percent (although the change was not statistically significant). However, larger firms continued to add a CDHP as an option. The percentage of employers with 500-999 workers reporting that they were likely to offer a CDHP in 2009 was 20 percent, or 6 percentage points higher than the actual offer rate in 2008. The percentage of firms with 1,000 or more workers that offered a CDHP increased from 22 percent in 2008 to 28 percent in 2009. The overall trend rate did not change because employers with 1,000 or more employees account for about 15 percent of all employers. Based on the various sources of data on enrollment in health reimbursement arrangements (HRAs) and HSA-eligible plans, it appears that 15-19 million people were enrolled in these plans in 2009, representing 9-11 percent of the privately insured market. Book excerpt: Objective: To investigate the effects of High Deductible Health Plans paired with a Health Savings Account on the healthcare expenditure and health services utilization. Methods: Data from the 2014 and 2015 Medical Expenditure Panel Survey was analyzed. Subjects enrolled in a Consumer Directed Health Plan (CDHP) were compared with subjects enrolled in non-CDHP after matching. A Chi-square analysis was conducted to assess frequency distribution of subjects in the treatment group as compared to the control. The control group was weighted using propensity score weights to match treatment group. The difference in the healthcare expenditures between the two groups was estimated using a Wilcoxon sign rank test and independent sample t-test. The effect of CDHP enrollment on health services utilization was estimated using a regression model. Results: The total sample population was 2132. The CDHP group was found to have a higher proportion of White, Non-Hispanics with high level of education and high family income as compared to the control group. A significant difference was found between the out-of-pocket costs (OOP) (p=0.0004) and the overall healthcare expenditure (p=0.0119) with the CDHP group having higher costs. There was no significant association between low or high use of health services. Conclusion: The findings suggest that CDHPs might not be fulfilling their required goal of reducing healthcare expenditure as well use of unnecessary care.

This Issue Brief focuses on whether and how attitudes and behaviors change with the amount of time an individual has been enrolled in their health plan. Overall satisfaction does not increase when participants are in health plans for long periods of time (e.g., 10 years), regardless of whether the plan is a traditional health plan, consumer-driven health plan (CDHP), or high deductible health plan (HDHP). Likewise, consumer engagement does not increase with time enrolled in a health plan. However, there is some evidence that health savings account (HSA)-eligible health plan enrollees are more engaged in some aspects of their HSA the longer they have been enrolled in their HSA-eligible health plan.

Book excerpt: Enrollment in "consumer-directed" health insurance plans - plans with high deductibles that are frequently coupled with personal savings accounts - has soared over the past four years and now exceeds 5 million. Approximately a third of firms offering health insurance benefits now offer a high-deductible plan and another third say they are interested in doing so. At the same time, prior studies have found that consumers prefer low-deductible plans, say they are willing to pay a premium for them, and are less satisfied when enrolled in high-deductible plans. This paper explores the determinants of enrollment in consumer-directed plans using data from a survey of 40 employers offering high-deductible plans. The sample of employers was stratified by the level of the deductible, the type of account offered (if any), and level of employer contributions to the account. The survey asks the employers for detailed information about their plan offerings, plan benefit designs, and enrollment levels; this includes information about both their consumer-directed option and any traditional HMOs or PPOs they may offer. It also asks about their consumer-directed plan implementation strategy and about resources available to help employees choose the best plan for them. We explored the factors associated with higher levels of enrollment when consumer-directed plans are offered alongside other options. These included three types of variables motivated by economic theory: 1) financial variables including plan benefit design elements such as deductible levels and employer contributions towards accounts; 2) behavioral variables such as requiring employees to make a proactive choice of plan each year; 3) information provision through employee communications. We find that factors within all three types of variables are important predictors of enrollment levels. We interpret our results about enrollment elasticities cautiously given our sample size, but do discuss the relative magnitudes of these effects and their implications.
