Ces phénomènes peuvent avoir de sérieuses conséquences sur les soignants (Roldan et al., 2013). La violence génèrerait un coût économique et humain considérable (Gadon, 2006). Objectif : Identifier les facteurs de risque et les moyens de prévenir la violence verbale et physique de patients hospitalisés en urgences psychiatriques envers le personnel soignant. Méthode : Critères d'inclusion : patients adultes (18 à 65 ans) admis en urgences psychiatriques. Type de résultats : facteurs de risque et outils/interventions. Deux bases de données ont été consultées : CINHAL et PsycINFO. Dix articles ont été inclus et analysés à l'aide de la Grille de Fortin. Les données relevées ont été classées en facteurs de risque non-modifiables, modifiables et outils. Résultats : Des facteurs de risque de violence comme le genre, l'âge, le diagnostic, le nombre d'hospitalisations, l'unité, la relation patient-soignant, les conflits ou la réactivité au stress ont été identifiés. Des interventions comme la prévision du risque, la formation du personnel, la médication involontaire, l'isolement, la contention ou la massothérapie ont cherché à réduire ces facteurs de risque. Discussion : Les résultats ont été éclairés par la théorie des relations interpersonnelles de Peplau. Rompre la relation patient-soignant pourrait engendrer de la violence. Pour préserver cette relation, des moyens existent.

Cela fait partie des risques psychosociaux. L'étude de la prévalence de cette violence au sein d'une UHSA (Unité hospitalière spécialement aménagée, unité hospitalière de psychiatrie en milieu pénitentiaire) n'a jamais été réalisée. L'objectif principal de ce travail est d'estimer la prévalence de la violence exogène subie par les soignantes de l'UHSA de Seclin. Les objectifs secondaires sont d'évaluer les conséquences psychopathologiques des violences sur ces soignants, d'évaluer les mesures mises en place à la suite d'un fait de violence, et de comparer la prévalence de la violence subie par les soignants de l'UHSA avec celle subie par ceux du Centre hospitalier de Tourcoing (services de médecine et des urgences) et ceux des Unités Tourquennoises de Psychiatrie (UTP). Matériel et Méthodes : Nous avons réalisé une étude épidémiologique, observationnelle, transversale. L'ensemble du personnel de l'UHSA a été inclus. Un auto-questionnaire a permis de recueillir la prévalence des faits de violence, des données socio-professionnelles et de santé mentale (trouble de stress post traumatique, burnout, troubles anxiodépressifs, troubles de l'usage des substances). Les données de prévalence de la violence subie par les soignants de Tourcoing ont été issues d'une étude précédente empruntant la même méthodologie. Résultats : La prévalence de la violence exogène a l'UHSA était de 80%. Les victimes de violences ont réalisé une Feuille d'évènement indésirables dans 54,2% des cas. Aucune victime n'a fait de déclaration à l'Observatoire national des violences en santé. Seules 6,2% ont déposé une plainte. L'échelle MBI (burnout) retrouvait un score pathologique chez 54,2% des participants, avec une atteinte importante du score d'accomplissement personnel. La violence était significativement plus fréquente à l'UHSA que dans les services de médecine tourquennois. Aucune différence significative n'a été retrouvée entre la prévalence de la violence à l'UHSA et celle des UTP et des services des urgences de Tourcoing. Conclusion : La violence exogène semble être un problème fréquent à l'UHSA de Seclin, comme dans les services des urgences de Tourcoing et les UTP. Les troubles psychiques chez le personnel hospitalier sont fréquents. Les mesures d'accompagnement et de signalement sont trop peu connues et trop peu utilisées par les victimes. Cela devrait être amélioré.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
