Le développement rapide des anticancéreux administrables par voie orale et l'augmentation de la prise en charge ambulatoire des patients nous ont conduits à étudier l'intérêt d'un entretien pharmaceutique officinal et à conceptualiser un lien ville-hôpital pour améliorer la prise en charge des patients atteints de cancer. Dans un premier temps, nous avons passé en revue le cadre législatif (plan cancer, loi hôpital patient santé et territoires), les structures existantes dans la prise en charge des patients atteints de cancer ainsi que la place de l'éducation thérapeutique du patient et de l'entretien pharmaceutique. Dans un deuxième temps, nous avons détaillé les thèmes pouvant être abordés et avons proposé une trame d'entretien pharmaceutique pour les patients atteints de cancer. Enfin, nous avons conceptualisé un lien ville-hôpital. Pour évaluer l'intérêt des professionnels de santé hospitaliers, un sondage a été réalisé auprès de praticiens hospitaliers prenant en charge des patients atteints de cancer. La coordination des soins entre les professionnels de santé hospitaliers et libéraux est primordiale pour assurer une prise en charge optimale de qualité. Le moyen le plus efficace d'échanger des informations de santé sont à ce jour les messageries de santé. Le pharmacien, comme professionnel de santé de proximité, spécialiste du médicament est un maillon essentiel du lien ville-hôpital. Par la réalisation d'entretiens pharmaceutiques de suivis il participe au suivi des patients atteints de cancer et sécurise la prise des traitements.

De nombreux projets ont été initiés en Pays de la Loire pour répondre à ces enjeux et pour apporter des réponses innovantes. Des travaux, conduits par l'observatoire dédié au cancer de l'OMEDIT Pays de la Loire ont fait l'objet d'une expérimentation pour les patients atteints de cancer dans le contexte d'une situation palliative. Ces travaux, initiés par une étude en 2009, ont permis d'élaborer le score pronostic PRONOPALL, significativement corrélé à la survie des patients. Basé sur quatre critères, il représente un outil d'aide à la décision pour le prescripteur afin d'optimiser la meilleure réponse thérapeutique pour les patients atteints de cancer en situation palliative. L'expérimentation présentée ici a pour objectif d'optimiser l'accès à l'équipe de coordination des soins palliatifs, de favoriser l'appropriation de l'outil PRONOPALL au sein des équipes médicales ainsi que de réduire l'administration d'anticancéreux proches du décès.

Book excerpt: La survie des patients atteints de cancer a été améliorée ces dernières années par un diagnostic plus précoce pour les tumeurs accessibles au dépistage et par les progrès thérapeutiques. Une révolution est en cours avec une utilisation croissante des chimiothérapies orales, ce qui amène la prise en charge oncologique à s'exercer de plus en plus au domicile du patient. Ces différents constats ont entraîné, de la part du personnel soignant, une réflexion accrue sur la qualité de vie des survivants. Le dernier Plan Cancer 2014-2019 prévoit, qu'en 2020, la proportion de chimiothérapie orale pourrait atteindre 50% des traitements à base d'anticancéreux. Bien que les chimiothérapies par voie orale apportent un confort pour les malades, elles sont parfois considérées à tort comme moins toxiques que les chimiothérapies intraveineuses. Le patient n'étant pas à l'hôpital pendant le traitement, la gestion des effets secondaires peut être difficile et le risque de non-observance est bien réel. La prise en charge ambulatoire soulève donc les problèmes de la tolérance et de l'observance de ces traitements. Une meilleure évaluation des symptômes que subissent les patients peut permettre d'améliorer ces axes par une intervention précoce du corps médical. Or la mise en place de ces prises en charge ne se fera pas sans difficultés car une réorganisation du système de soin est nécessaire et va générer des contraintes sur le plan économique. Le développement récent des logiciels de Santé (eSanté/mSanté) de suivi des symptômes auto-déclarés par les patients pourrait à la fois répondre aux contraintes budgétaires et organisationnelles mais aussi rendre ces patients acteurs de leur prise en charge. Ce travail propose une revue de la littérature de ces nouvelles perspectives de suivi.

Ovarian cancer is cancer that begins in the cells that constitute the ovaries, including surface epithelial cells, germ cells, and the sex cord-stromal cells. Cancer cells that metastasize from other organ sites to the ovary (most commonly breast or colon cancers) are not then considered ovarian cancer. According to the American Cancer Society, ovarian cancer accounts for 4 percent of all cancers among women and ranks fifth as a cause of their deaths from cancer. The American Cancer Society statistics for ovarian cancer estimate that there will be 25,400 new cases and 14,300 deaths in 2003. The death rate for this disease has not changed much in the last 50 years. Unfortunately, almost 70 percent of women with the common epithelial ovarian cancer are not diagnosed until the disease is advanced in stage -- i.e., has spread to the upper abdomen (stage III) or beyond (stage IV). The 5-year survival rate for these women is only 15 to 20 percent, whereas the 5-year survival rate for stage I disease patients approaches 90 percent and for stage II disease patients approaches 70 percent. Ovarian tumors are named according to the type of cells the tumor started from and whether the tumor is benign or cancerous. The three main types of ovarian tumors are: Epithelial Tumors, Germ Cell Tumors and Stromal Tumors.
