Book excerpt: Dr. Kummer's Cleft Palate and Craniofacial Anomalies: Effects on Speech and Resonance provides comprehensive coverage of a complex subject in a logical progression of learning in which each chapter builds on information from the previous chapter. This is the only text of its kind that covers both oral and facial anomalies in addition to cleft lip and palate. Designed for students, clinicians new to this population, and any member of a cleft palate team, this book emphasizes what practicing clinicians need to know in the workplace. Current research and best practices are summarized for readers. From basic information on anatomy, physiology, and embryololgy of the face and oral cavity to oral, dental and ENT anomalies and their effects on speech, resonance, and feeding this book includes information on common craniofacial anomalies, associated anomalies, and genetic syndromes. The evaluation section includes the perceptual evaluation of speech and resonance, the intraoral examination, and instrumental assessment procedures. The treatment section includes surgical procedures, prosthetic devices, and therapies designed to address speech, resonance, and velopharyngeal dysfunction. The need for multidisciplinary team assessment and treatment is emphasized throughout with contributions from members of a working cleft palate team.

Book excerpt: This surgical atlas describes the multidisciplinary approach to treating pediatric velopharyngeal insufficiency (VPI) and reviews the diagnostic and therapeutic modalities involved. The publication begins with a review of the key anatomic features of children with VPI and focuses on a clinically relevant approach to surgical anatomy. How to perform nasal endoscopy and how to decide between different surgical treatments are key diagnostic questions when treating VPI, which are discussed in detail in this volume. Included with the published text are 5- to 7-minute-long narrated audio/video recordings of each surgery. The videos show the key surgical steps as well as the pearls and pitfalls of each surgical intervention. This publication gives an excellent overview of the various types of procedures, and it should prove to be essential reading for VPI surgeons, both the novice and experienced, who need to understand the relevant anatomy and the pearls and pitfalls of each of the various surgical procedures.

Skolnick and E.R. Cohn present multiview videofluoroscopy as a technique for the radiological evaluation of speech defects in patients with cleft palate. Dr. Skolnick's invaluable contributions as both the originator and leading authority on the subject are discussed in a concise, clinical fashion. Topics examined include the anatomy and imaging of the velopharyngeal portal; the equipment and techniques of multiview videofluoroscopy; the interpretation of results and various patterns of velopharyngeal closure; Passavant's Ridge and patterns of velopharyngeal closure; normal and abnormal speech production; and the evaluation of test results. This book is the only source which concisely and completely describes the technique and its interpretations for those who need a description of the clinical procedure.

Book excerpt: Speech and Language: Volume 3, Advances in Basic Research and Practice is a compendium of papers that discusses theories, clinical issues, and pathology of language and speech. Some papers discuss theories of phonological development, the encoding/decoding system of language, and the application of phonological universals in speech pathology. Other papers deal with the role of the speech-language clinician, a psychological framework for speech perception, and the formulation of a model for biomechanical analysis of velopharyngeal structure and function. Several papers analyze speech control mechanisms in skilled and non-skilled speakers, the rationale for the delayed auditory feedback (DAF) treatment program, and biofeedback in relation to speech pathology. One paper cites a study of Williams (1974) that shows strategies used in learning a new phonetic system depend upon whether the speaker is still within the critical period for language learning or already well beyond it. The paper notes that if adults can ignore their previously learned sound system and be childlike again in their freedom to experiment and be sensitivity to their own results, then they can achieve supra-segmental and segmental nuances of a new language. The compendium can prove helpful for linguists, ethnologists, psychologists, speech therapists, researchers in linguistics or communications, and general readers interested in speech or learning issues.

Available in PDF, EPUB and Kindle. Book excerpt: Cleft lip and palate represents the most common congenital facial anomaly. Congenital anomalies are one of the top 10 causes of disability and suffering in many low and middle income countries (LMICs.) Further, an estimated 30% of the global burden of disease is treated primarily through surgery, and among surgical conditions, treating clefts conveys disproportionately large benefit relative to cost because suffering can be halted at a young age and deliver a lifetime of health. Over the last 50 years, cleft care has advanced in low-resource settings through international surgical missions and the efforts of sparse local providers. While exciting anecdotes of impact abound from these efforts, recent studies have identified a complication rate that often exceeds 50% and there is concern about the sustainability of foreign volunteer missions. This book serves as a thorough resource for providing medical and surgical care of cleft lip and palate deformities in low-resource settings, geared to the context of those settings. The content is consensus-based and evidence-based, with emphasis on treatment that is appropriate to a low-resource context and how this differs from that in higher-resource environments. It is authored by recognized cleft experts in close partnership with providers who practice in low-resource settings. It is heavily pictorially based through illustrative figures, photos and diagrams, to be facile for those who speak English as a second language. Following the introduction, Complete Cleft Care in Low-Resource Settings has six main sections: patient workup, surgical treatment, comprehensive care, educational innovation, care delivery models, and finally a series of illustrative case studies. Uniquely, the book’s breadth encompasses cleft care comprehensively, but does so through a practical “how to” format with many “from the field” examples of what has worked.
