Designed as a portable resource, the book provides typical dictations to guide the resident. Comprised of 226 procedures, this new edition comprises the majority of procedures commonly performed by general and vascular surgeons. For each procedure, a list of common indications is given. This list covers 95% of the situations in which a particular procedure will be used. A list of essential steps follows. These can be used to mentally rehearse the procedure before it is performed. The next heading, Note these technical variations, introduces a list that is intended to prompt the surgeon for particular things to note and dictate within the template. A list of possible complications that are typically associated with that particular surgical procedure follows. Finally, a template operative dictation is provided. Common opening and terminating sequences (for example, how to enter and close the abdomen) in all laparotomy dictations are included. The volume has two primary objectives. First, it is designed to serve as a source of model operative dictations that may be individualized and used as templates. Second, it is intended as an aide-memoir, giving the surgeon a short list of pertinent information about each procedure. Ample space is provided to allow each surgeon to add notes. Book excerpt: When North Korean forces invaded South Korea on June 25, 1950, Otto Apel was a surgical resident living in Cleveland, Ohio, with his wife and three young children. A year later he was chief surgeon of the 8076th Mobile Army Surgical Hospital constantly near the front lines in Korea. Immediately upon arriving in camp, Apel performed 80 hours of surgery. His feet swelled so badly that he had to cut his boots off, and he saw more surgical cases in those three and a half days than he would have in a year back in Cleveland. There were also the lighter moments. When a Korean came to stay at the 8076th, word of her beauty spread so rapidly that they needed MPs just to direct traffic. Apel also recalls a North Korean aviator, nicknamed "Bedcheck Charlie," who would drop a phony grenade from an open-cockpit biplane, a story later filmed for the television series. He also tells of the day the tent surrounding the women's shower was "accidentally" blown off by a passing helicopter. In addition to his own story, Apel details the operating conditions, workload, and patient care at the MASH units while revealing the remarkable advances made in emergency medical care. MASH units were the first hospitals designed for operations close to the front lines, and from this particularly difficult vantage, their medical staffs were responsible for innovations in the use of antibiotics and blood plasma and in arterial repair. On film and television, MASH doctors and nurses have been portrayed as irreverent and having little patience with standard military procedures. In this powerful memoir, Apel reveals just how realistic these portrayals were.

September 23, 1996 KLM Flight 287 rolled to a seamless stop on the tarmac, and settled on its wheels under the translucent Moscow sky. A contingent of American heart surgeons and support staff blinked and stretched. Their leader was famous for dozing off as soon as the wheels were up on any flight of length, and more than one of his retinue had mimicked that feat on this last leg of their journey from Houston via Amsterdam. This was not the first sojourn to Russia for their Chief - that had been back in 1958, when the country had another name and quite another polity. Many other visits had followed, accompanied by accolades and fetes, mostly with a Cold War political undertone he neither shared nor acknowledged. By nature he was fond of reflecting on change and history, and he did not miss the significance of returning to this place, the both of them now so different, in a position to alter the life path of a man who, himself, was responsible for much of this country's metamorphosis. The President of Russia, Boris Yeltsin, was suffering from congestive heart failure and severe coronary artery disease. The situation was grave; he was not expected to live without open-heart surgery - coronary bypass - but it was not clear that he could survive the procedure itself. The leading Russian heart surgeons and cardiologists had asked this Houston team to come to Moscow to assess the risks and provide recommendations as to how to proceed.
