Available in PDF, EPUB and Kindle. Book excerpt: This unique textbook – the first of its kind – presents a thoughtful and comprehensive discussion of the significance of varicocele and its impact on male fertility. This hot topic is covered from a multitude of angles in seven thematic sections: origin and pathophysiology, clinical evaluation, medical and surgical therapy, controversies, pro and con debates, and clinical case scenarios. The scope of varicocele includes basic and transitional research, genetics, diagnostic testing with conventional and advanced molecular biology approaches, hormonal control, interventional therapy and assisted reproductive technology (ART). Repair of varicocele, which can be carried out by various methods, not only alleviates oxidative stress‐associated infertility but also prevents and protects against the progressive character of varicocele and its consequent upregulations of systemic oxidative stress. Even with the advances in the understanding of this intriguing disease and consensus on some areas such as diagnosis and pathophysiology, substantial controversy still exists, in particular concerning the benefits of treatment and to whom treatment should be offered. Additional chapters discuss, in depth, such controversies surrounding the role of varicocele in male infertility and present clinical case scenarios dealing with management of subclinical and clinical varicocele. A series of editorial comments is provided by the editors at the end of selected chapters, containing an objective and concise summary of the information from each chapter. In addition, chapters open up with key points for quick references and conclude with multiple choice questions and answers for immediate review and retention of the rich content. Generously illustrated, Varicocele and Male Infertility: A Complete Guide represents an invaluable tool for medical students in reproductive medicine as well as researchers and clinicians working in the field of infertility (e.g., urologists, gynecologists, reproductive endocrinologists, and embryologists) and is comprised of chapters written by leading and internationally recognized clinicians and scientists with expertise in varicocele, skillfully edited by leaders in the field.

Available in PDF, EPUB and Kindle. Book excerpt: The new edition of this canonical text on male reproductive medicine will cement the book's market-leading position. Practitioners across many specialties - including urologists, gynecologists, reproductive endocrinologists, medical endocrinologists and many in internal medicine and family practice – will see men with suboptimal fertility and reproductive problems. The book provides an excellent source of timely, well-considered information for those training in this young and rapidly evolving field. While several recent books provide targeted 'cookbooks' for those in a male reproductive laboratory, or quick reference for practising generalists, the modern, comprehensive reference providing both a background for male reproductive medicine as well as clinical practice information based on that foundation has been lacking until now. The book has been extensively revised with a particular focus on modern molecular medicine. Appropriate therapeutic interventions are highlighted throughout.

Available in PDF, EPUB and Kindle. Book excerpt: F. H. Comhaire Many diseases were, at the beginning, merely defined by the description of their clinical appearance. Next, the pathogenic mechanisms underlying the diseases were recognized. Since then, the proof of presence of the pathogenic agent or agents has been required to confirm the diagnosis. However, it sometimes happens that the pathogenic agent can be demonstrated without the disease being clinically evident. Confusion arising from this observation may cause endless, often purely emotional discussions between "believers" and "non-believers". Moreover, if the disease involves potential disturbance of male fertility, the problem is further obscured by the difficulty of defining man's fertility. Indeed, during the short history of andrology, the criteria for judging a man and his ejaculate as potentially fertile or infertile have repeatedly changed. Andrological "landmarks" in general do not hold up for long, and some scientists continue to set themselves the task of proving the "old" definitions invalid. Certainly, such developments are necessary to make science more exact and to improve medical care. However, while this research is being done, the male partners of barren marriages continue to seek advice and treatment. Common sense and an empirical approach in the handling of these cases may result in obviously encouraging results, which non-believers will ascribe to "witchcraft" and believers will see as confirmation of their opinions.

Book excerpt: The book aims to bring into focus the current understanding of male reproduction and the pathological basis of failure to reproduce in men. Infertility in men is a common disorder. However, attempts at scientific study of male infertility are of very recent origin. Many questions about the physiology and pathophysiology of male reproduction are still not clear. The chapters are written by authorities in the field with great clinical experience. The primary focus would be on clinical perspective; however emphasis would also be placed on basic research and molecular biology.

Available in PDF, EPUB and Kindle. Book excerpt: It is not surprising that so much investigation has been undertaken to establish the cause of childlessness, especially when the potentia coeundi of the male is not impaired. As long ago as 1957 the German Society for the Study of Fertility and Sterility was founded, embracing gynecology, andrology, and veterinary medicine. After its inception, meetings conducted in the spirit of its foundation were held every 2 years. This interdisciplinary, coordinated scientific work in the field of human reproduction achieved its value as a result of the stimuli provided by the permanent involvement of veterinary scientists. After about 20 years of activity, the Society adopted a highly differentiated pattern of work-directions in the field of human and veterinary medicine. Since 1976 annual meetings have been held on the topic of animal and human reproduction. These meetings have proved of great value, especially in the area of comparative medicine, and have led to excellent interdisciplinary associations. One of the most evident successes was the first extracorporal fertilization in humans with subsequent embryo transfer and full-term pregnancy. Gynecologists have found that in 40% of cases the cause of undesired childlessness rests with the male, and it is therefore no surprise that also in the field of andrology certain factors concerned with infertility have received great attention.

Introductory chapters consider imaging instrumentation, clinical evaluation, and clinical and imaging anatomy. The full range of disorders is then discussed in individual chapters organized according to clinical presentation. All clinical and imaging aspects are covered in depth, with full description of symptoms and explanation of the value of different clinical tests and imaging modalities. In addition, underlying histopathological features are presented and correlated with imaging features in order to clarify their pathological basis. For each disorder, therapeutic strategies are discussed and appraised. Adults and children are considered separately whenever necessary, bearing in mind that they often present essentially different scrotal pathology. The many images are all of high quality and were obtained using high-end equipment.

Although the exact causes of sperm DNA damage are unknown, it is clear that infertile men possess substantially higher levels of sperm DNA damage than do fertile men. Written by leading, internationally renowned clinicians and basic scientists with expertise in sperm DNA, Sperm Chromatin: Biological and Clinical Applications in Male Infertility and Assisted Reproduction provides readers with a thoughtful and comprehensive review of the biological and clinical significance of sperm DNA damage. The work covers the fundamental principles of sperm chromatin architecture and function, the proposed modes of DNA damage and repair, the tests of sperm DNA damage, the clinical aspects of DNA damage and the impact of DNA damage on reproductive outcome. Unlike any other title on the topic, Sperm Chromatin: Biological and Clinical Applications in Male Infertility and Assisted Reproduction is an invaluable addition to the literature and will serve as an indispensable resource for basic scientists with an interest in sperm biology and for urologists, gynecologists, reproductive endocrinologists, and embryologists working in the field of infertility.

Laparoscopy is at the forefront of urology today, with dramatic advances in engineering and surgical technique. Because they promise to be less invasive, the innovations presented in this volume will become tomorrows standard. In utero and pediatric laparoscopic surgery are discussed, as are varying approaches to laparoscopic adrenalectomy. Other topics include radical surgery for renal cell carcinoma and transitional cell carcinoma, nephron-sparing laparoscopic surgery for renal cancer, and retroperitoneal lymphadenectomy for testicular cancer. With sections on pyeloplasty, nephropexy, urolithiasis, stress incontinence, kidney transplantation, and dialysis, this book is an especially valuable source in the fields of endourology, laparoscopic surgery, pediatric surgery, and endocrine surgery.
