Available in PDF, EPUB and Kindle. Book excerpt: Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: The Social Security Administration (SSA) administers two programs that provide benefits based on disability: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. This report analyzes health care utilizations as they relate to impairment severity and SSA's definition of disability. Health Care Utilization as a Proxy in Disability Determination identifies types of utilizations that might be good proxies for "listing-level" severity; that is, what represents an impairment, or combination of impairments, that are severe enough to prevent a person from doing any gainful activity, regardless of age, education, or work experience.

Book excerpt: Evidence from the public health sector demonstrates that health care is only one of the determinants of health, which also include genes, behavior, social factors, and the built environment. These contextual elements are key to understanding why health care organizations are motivated to focus beyond their walls and to consider and respond in unprecedented ways to the social needs of patients, including transportation needs. In June 2016 the National Academies of Sciences, Engineering, and Medicine held a joint workshop to explore partnerships, data, and measurement at the intersection of the health care and transportation sectors. This publication summarizes the presentations and discussions from the workshop.

Available in PDF, EPUB and Kindle. Book excerpt: This valuable handbook makes the U.S. health care system understandable, reviews the Affordable Care Act (ACA) or "Obamacare," describes past health care reform efforts, and covers the important organizations and people involved in U.S. health care reform. Why does the United States produce poorer outcomes for Americans in terms of health care than most other developed countries that spend a lower percentage of their Gross Domestic Product (GDP) on health care? Should health insurance be private or should it be managed by the government? The second edition of Healthcare Reform in America: A Reference Handbook addresses these complex and difficult questions and many more in its thorough treatment of one of the most controversial issues in contemporary American discourse. The work provides a broad introduction to the history and key issues in the development and reform of the U.S. health care system. It then addresses the recent passage of the Affordable Care Act and the myriad of significant expected changes due to the Act, thereby providing readers with information essential to understanding the current issues regarding health care reform. This work serves as a valuable resource to high school and college students as well as to general readers wanting to learn about the history and current focus of health care reform in the United States.

This transition aligns the incentives of all stakeholders, including payers, purchasers, clinicians, and most importantly, patients, by prioritizing health over care, and facilitating competition based on health outcomes and cost. Developed in partnership with the American Academy of Orthopaedic Surgeons (AAOS) and edited by Eric C. Makhni, MD, MBA, FAAOS, Benedict Nwachukwu, MD, MBA, and Kevin J. Bozic, MD, MBA, FAAOS, this unique, authoritative text covers essential information not often covered in medical school or orthopaedic residency training—offering a comprehensive discussion of the principles of value-based health care as applied to orthopaedics.

Book excerpt: Between 2010 and 2014, the Office of the National Coordinator for Health Information Technology (ONC) contracted with RTI International to administer the State Health Policy Consortium (SHPC). The project objective was to provide states with resources needed to develop solutions to challenges preventing or impeding health information exchange (HIE) across state lines. The methods used to establish, fund, support, and manage consortiums, which we refer to as an “agile policy analysis methodology,” resulted in a number of successful HIE pilot and demonstration projects. The structured yet flexible framework developed by RTI helped to identify and implement high-value projects through a grassroots approach. Challenges and opportunities within the HIE environment evolve rapidly. As solutions are implemented and HIE expands, new opportunities and tipping points emerge. The flexible SHPC model allowed ONC to take advantage of these opportunities as they emerged, to make rapid progress. This report describes the agile policy analysis methodology the SHPC developed and used to support collaboration between states regarding HIE, enumerates the benefits of this methodology and the conditions under which it succeeds, and discusses how it can be employed in the future to further support both HIE and other challenges requiring multistate collaboration.
