Book excerpt: Being a complex disease that affects millions of people world over, cancer research has assumed great significance. Translational cancer research transforms scientific discoveries in the laboratory into clinical application to reduce incidence of cancer, morbidity and mortality. On the other hand, personalized medicine in cancer is the concept that selection of a treatment should be tailored according to the individual patient’s specific genomic characteristics, including mutations, chromosomal aberrations, protein interactions, and SNPs, and even more, taking into account the inmume system, the metabolism and maybe in the next future also the microbiome.

Book excerpt: "Cancer research is at a crossroads. Traditionally, cancer has been thought of as a disease of gene mutation, where the stepwise accumulation of cancer gene mutations is the key, and the identification of common gene mutations has been considered to be essential for diagnosis and treatment. Despite extensive research efforts and accumulated knowledge on cancer genes and pathways, the clinical benefits of this traditional approach have been limited. Recently, cancer genome sequencing has revealed an extensive amount of genetic heterogeneity where the long-expected common mutation drivers have been difficult, if not impossible, to identify. These realities ultimately challenge the conceptual framework of current cancer biology. This book introduces a new concept of genome theory of cancer evolution, in an attempt to unify the field. Many important and representative, but often confusing, questions and paradoxes are critically analyzed. By comparing gene- and genome-based theories, the hidden flaws of many popular viewpoints are addressed. This discussion is intended to initiate a much-needed critical re-evaluation of current cancer research."--

Book excerpt: This collection of 25 research papers comprised of 22 original articles and 3 reviews is brought together from international leaders in bioinformatics and biostatistics. The collection highlights recent computational advances that improve the ability to analyze highly complex data sets to identify factors critical to cancer biology. Novel deep learning algorithms represent an emerging and highly valuable approach for collecting, characterizing and predicting clinical outcomes data. The collection highlights several of these approaches that are likely to become the foundation of research and clinical practice in the future. In fact, many of these technologies reveal new insights about basic cancer mechanisms by integrating data sets and structures that were previously immiscible. Accordingly, the series presented here bring forward a wide range of artificial intelligence approaches and statistical methods that can be applied to imaging and genomics data sets to identify previously unrecognized features that are critical for cancer. Our hope is that these articles will serve as a foundation for future research as the field of cancer biology transitions to integrating electronic health record, imaging, genomics and other complex datasets in order to develop new strategies that improve the overall health of individual patients.

Available in PDF, EPUB and Kindle. Book excerpt: Technologies collectively called omics enable simultaneous measurement of an enormous number of biomolecules; for example, genomics investigates thousands of DNA sequences, and proteomics examines large numbers of proteins. Scientists are using these technologies to develop innovative tests to detect disease and to predict a patient's likelihood of responding to specific drugs. Following a recent case involving premature use of omics-based tests in cancer clinical trials at Duke University, the NCI requested that the IOM establish a committee to recommend ways to strengthen omics-based test development and evaluation. This report identifies best practices to enhance development, evaluation, and translation of omics-based tests while simultaneously reinforcing steps to ensure that these tests are appropriately assessed for scientific validity before they are used to guide patient treatment in clinical trials.

Book excerpt: Tumor progression is driven by mutations that confer growth advantages to different subpopulations of cancer cells. As a tumor grows, these subpopulations expand, accumulate new mutations, and are subjected to selective pressures from the environment, including anticancer interventions. This process, termed clonal evolution, can lead to the emergence of therapy-resistant tumors and poses a major challenge for cancer eradication efforts. Written and edited by experts in the field, this collection from Cold Spring Harbor Perspectives in Medicine examines cancer progression as an evolutionary process and explores how this way of looking at cancer may lead to more effective strategies for managing and treating it. The contributors review efforts to characterize the subclonal architecture and dynamics of tumors, understand the roles of chromosomal instability, driver mutations, and mutation order, and determine how cancer cells respond to selective pressures imposed by anticancer agents, immune cells, and other components of the tumor microenvironment. They compare cancer evolution to organismal evolution and describe how ecological theories and mathematical models are being used to understand the complex dynamics between a tumor and its microenvironment during cancer progression. The authors also discuss improved methods to monitor tumor evolution (e.g., liquid biopsies) and the development of more effective strategies for managing and treating cancers (e.g., immunotherapies). This volume will therefore serve as a vital reference for all cancer biologists as well as anyone seeking to improve clinical outcomes for patients with cancer.

Cancer biology has undergone several intellectual revolutions in the past 50 years. A mutation-centric view of cancer has given way to the tumor microenvironment view. Reductionistic studies of one gene at a time have given way to systems biology approaches that analyze the whole genome (omics) at the same time. However, this text combines the complex levels studying cancer at the molecular biology level, endocrinology level, and transcriptomics level. What researchers are now realizing is that there is a need to combine omics with physiology concepts in order to better understand cancer and this book will give insight to the merging of these two fields in order to define how cancer is studied in the future.

Book excerpt: Precision cancer medicine promises better treatments to a disease as complex and heterogenous as cancer. Many anti-cancer therapies are beneficial to only a subset of patients due to the variability in patient genetic and tumor heterogeneity. Thus, we need better frameworks for understanding underlying genomic and transcriptomic patterns influencing differential patient outcomes, yet our understanding of how genetic alterations connect to treatment in in vivo and in vitro models remains understudied. To address this gap, I utilized human patient data from The Cancer Genome Atlas (TCGA), hepatocellular carcinoma (HCC) models, prostate cancer (PCa) models, and chronic myelogenous leukemia (CML) cell lines. Through the integration of multi-omic data, I identified parallel features of human and model organism data that could reveal disease specific characteristics. Additionally, I characterized the landscape of acquired resistance for a panel of chemotherapeutic treatments and revealed potential alleles and genes that mediate the process. The analyses I conducted expose the role of genetic information and suggest future applications for development of precision medicine.

Available in PDF, EPUB and Kindle. Book excerpt: Genetic alterations in cancer, in addition to being the fundamental drivers of tumorigenesis, can give rise to a variety of metabolic adaptations that allow cancer cells to survive and proliferate in diverse tumor microenvironments. This metabolic flexibility is different from normal cellular metabolic processes and leads to heterogeneity in cancer metabolism within the same cancer type or even within the same tumor. In this book, we delve into the complexity and diversity of cancer metabolism, and highlight how understanding the heterogeneity of cancer metabolism is fundamental to the development of effective metabolism-based therapeutic strategies. Deciphering how cancer cells utilize various nutrient resources will enable clinicians and researchers to pair specific chemotherapeutic agents with patients who are most likely to respond with positive outcomes, allowing for more cost-effective and personalized cancer therapeutic strategies.
