The pig harbors relatively few diseases which could be transmitted inadvertently to human patients. The ability to genetically modify swine to ameliorate the consequences of the human immune response offers a further significant advantage. Another important consideration for an animal model is that basic biologic back ground information be available for investigators to design future prospective studies.

Book excerpt: Abstract: Even though the crude birth rate in the United States is decreasing, the premature birth rate is at its highest level with a 29% rate increase since 1981. Mortality and morbidity are concerns for this specialized population, which requires continuous re-evaluation of the medical care protocols. One standard of care that has been controversial, but proven effective, is aggressive nutrition support, specifically the provision of protein to these fragile neonates. Protein, an essential macronutrient, aids in body functions, nitrogen accretion, and thus growth. However, many practitioners remain cautious with implementation of such nutritional care protocols for aggressive protein to begin hours after birth in fear of protein intolerance. The purpose of this study was to describe extrauterine growth, nutritional intake, and clinical outcomes of premature infants receiving parenteral protein within the first 24 hours of life as compared to parenteral protein initiated post 24 hours after birth. Medical records for 108 infants were retrospectively reviewed and only seventy-five infants met study eligibility. The study included thirty-five (35) infants between October 1999 to April 2001 receiving no more than 1.0 gram parenteral protein initiated post 24-hours after birth (Less AA) and forty (40) infants between October 2003 to April 2005 receiving a 2.0-gram parenteral protein starter solution within hours after birth (2.0-gram AA). All subjects were cared for in the Special Care Nursery at the Mount Carmel Medical Center in Columbus, Ohio. Data collected for each neonate included birth statistics, nutritional intake for protein, calories, and fluid in the first 120 hours of life, anthropometric changes in the first 28 days of life, number of days parenteral nutrition ordered, length of stay, and peak blood urea nitrogen (BUN) serum levels reached between 24 to 120 hours of life. Nutritional intake was found to be significant between protein groups and birthweight categories overall and most days. The 2.0-gram AA group received significantly more protein and calories in the first 120 hours of life, while the Less AA group received significantly more fluids. Difference in fluid may have impacted the results for growth, which only reached significance for days to reach nadir in the 1250 to 1500 gram birthweight category and for head circumference (HC) changes in the 1000 to 1250 gram birthweight category. The peak BUN was also significantly higher for all birthweight categories in the 2.0-gram AA group but acceptable. The results of this study indicate that use of a 2.0-gram parenteral protein starter solution has a positive impact on the premature infant's nutritional intake without a detrimental effect on clinical outcomes. Even though growth was not significant for most parameters in this study, other prospective research should further investigate growth while controlling for fluid provision with the 2.0-gram protein parenteral starter solutions, specifically any HC changes and long-term developmental outcomes.
