Le poids économique du SAHOS est majeur de par ses comorbidités et les explorations diagnostiques ainsi que les traitements mis en place notamment la PPC. L'objectif de ce travail est l'étude préliminaire des patients présentant un SAHOS traités par PPC selon le protocole actuel de l'unité de sommeil du Centre Hospitalier de Dunkerque, en vue d'un essai thérapeutique futur. Méthode : Il s'agit d'une étude observationnelle non comparative, rétrospective et monocentrique. Elle est basée sur l'analyse des dossiers des patients, des données relevées sur le site de l'Observatoire Sommeil de la Fédération de Pneumologie et sur les sites des prestataires de service. La description de la population a été réalisée à l'aide de statistiques univariées. Résultats : Notre étude a observé 100 patients dont 70% d'hommes, d'âge moyen de 56 +/- 12 ans (de 22 à 82 ans), souffrant d'un SAHOS sévère (Index d'Apnées-Hypopnées (IAH) médian à 44/h), avec un Index de Masse Corporelle moyen de 36 +/- 9kg/m2. Ces patients apnéiques pris en charge au sein de notre service ont présenté une bonne compliance à la PPC. Nous avons retrouvé des pressions efficaces de l'ordre de 8 cm H2O, les pressions maximales et les P90 percentile de 10 cm H2O, un IAH résiduel de 3/h et des fuites résiduelles de 22 L/min. Cinquante pour cent des patients utilisaient la PPC 96% du temps ou plus avec une bonne observance (5,78 h par nuit). Conclusion : Nous avons montré que notre population était comparable à celle de la littérature. Nous pourrons donc envisager une étude randomisée de non infériorité comparant l'observance et l'efficacité d'une PPC instaurée au domicile via un prestataire de service versus en centre hospitalier par une équipe de sommeil. Une prise en charge du SAHOS en ambulatoire devrait permettre de réduire le délai et le coût des explorations diagnostiques ainsi que l'instauration du traitement tout en conservant une efficacité similaire.
