Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Further efforts to identify policies from countries of the Commonwealth of Independent States were unfruitful. Based on this evidence synthesis the following policy considerations are proposed: - consider the existing policies and related activities gathered in this review to develop or enhance health literacy policies and related activities to benefit citizens patients and communities; - broaden the range of areas of activity required for holistic health literacy policies to include the lived environment the workplace the media and digital/e-health at all societal levels ? individual community organization and system (legislative); - strengthen the evidence base for health literacy at all societal levels to ensure that policies address needs specific to the national or local context; -incorporate robust qualitative and quantitative evaluations into health literacy policies and interventions ? quantitative methods could include pre- and post-activity health literacy evaluations of evidence of health social and economic effects at all levels; and ?incorporate facilitators of successful implementation such as intersectoral working political leadership and strategies to overcome cultural barriers into health literacy policy. Member States would benefit from adopting such comprehensive frameworks and using metrics to design effective policies that support the development of a health-literate Europe.

Book excerpt: This public inquiry report into serious failings in healthcare that took place at the Mid Staffordshire NHS Foundation Trust builds on the first independent report published in February 2010 (ISBN 9780102964394). It further examines the suffering of patients caused by failures by the Trust: there was a failure to listen to its patients and staff or ensure correction of deficiencies. There was also a failure to tackle the insidious negative culture involving poor standards and a disengagement from managerial and leadership responsibilities. These failures are in part a consequence of allowing a focus on reaching national access targets, achieving financial balance and seeking foundation trust status at the cost of delivering acceptable care standards. Further, the checks and balances that operate within the NHS system should have prevented the serious systemic failure that developed at Mid Staffs. The system failed in its primary duty to protect patients and maintain confidence in the healthcare system. This report identifies numerous warning signs that could and should have alerted the system to problems developing at the Trust. It also sets out 290 recommendations grouped around: (i) putting the patient first; (ii) developing a set of fundamental standards, easily understood and accepted by patients; (iii) providing professionally endorsed and evidence-based means of compliance of standards that are understood and adopted by staff; (iv) ensuring openness, transparency and candour throughout system; (v) policing of these standards by the healthcare regulator; (vi) making all those who provide care for patients , properly accountable; (vii) enhancing recruitment, education, training and support of all key contributors to the provision of healthcare; (viii) developing and sharing ever improving means of measuring and understanding the performance of individual professionals, teams, units and provider organisations for the patients, the public, and other stakeholders.

Chapter 2 identifies the challenges facing the NHS in the 21st century: ever higher expectations; demand driven by demographics as people live longer; health in an age of information and connectivity; the changing nature of disease; advances in treatment; a changing health workplace. Chapter 3 outlines the proposals to deliver high quality care for patients and the public, with an emphasis on helping people to stay healthy, empowering patients, providing the most effective treatments, and keeping patients as safe as possible in healthcare environments. The importance of quality in all aspects of the NHS is reinforced in chapter 4, and must be understood from the perspective of the patient's safety, experience in care received and the effectiveness of that care. Best practice will be widely promoted, with a central role for the National Institute for Health and Clinical Excellence (NICE) in expanding national standards. This will bring clarity to the high standards expected and quality performance will be measured and published. The full range of society's diversity is reflected in patients and staff. Increasing religious and ethnic plurality, alongside decades of secularizing trends, is bringing new attention to how religion and nonreligion are expressed in public spaces. Through critical ethnographic research in Vancouver and London, Prayer as Transgression? reveals how prayer occurs in hospitals, long-term care facilities, and community-based clinics in a variety of forms and circumstances. Prayer occurs quietly on the edges of day-to-day healthcare provision and in designated sacred spaces. Some requests for prayer, however, interrupt and transgress the clinical machinery of a hospital, such as when a patient asks for prayer from the chaplain while the operating room waits. With contributions by researchers, healthcare practitioners, and chaplains, the authors consider how prayer transgresses the clinical priorities that mark healthcare, opening up ways to think differently about institutional norms and social structures. They show how prayer highlights trends of secularization and sacralization in healthcare settings. They also consider the ambivalences about prayer arising from staff and patients' varied views on religion and spirituality, and their associated ethical concerns amidst clinical and workload demands. Book excerpt: Focusing on a matter of continuing contemporary significance, this book is the first work to offer an in-depth exploration of exploitation in the doctor-patient relationship. It provides a theoretical analysis of the concept of exploitation, setting out exploitation’s essential elements within the authors’ account of wrongful exploitation. It then presents a contextual analysis of exploitation in the doctor-patient relationship, considering the dynamics of this fiduciary relationship, the significance of vulnerability, and the reasons why exploitation in this relationship is particularly wrongful. Two case studies – sexual exploitation and assisted dying – are employed to assess what the appropriate legal, ethical and regulatory responses to exploitation should be, to identify common themes regarding the doctor’s behaviour (such as the use of undue influence as a conduit through which to take advantage of and misuse patients), and to illustrate the effects of exploitation on patients. A recurring question addressed is how exploitation in the doctor-patient relationship is and should be dealt with by ethics, regulators and the law, and whether exploitation in this relationship is a special case. The book provides a critical, interdisciplinary evaluation of exploitation in the doctor-patient relationship that will be of interest to health care lawyers, bioethicists, legal academics and practitioners, health care professionals and policymakers.
