Available in PDF, EPUB and Kindle. Book excerpt: Midwifery is used to describe a number of different types of health practitioners, other than doctors, who provide prenatal care to expecting mothers, attend the birth of the infant and provide postnatal care to the mother and infant. Nurse-midwives also provide gynaecological care to women of all ages. Practitioners of midwifery are known as midwives, a term used in reference to both women and men (the term means "with the woman"). Most are independent practitioners who work with obstetricians when the need arises. They usually deal with normal births only but are trained to recognise and deal with deviation from the norm. If something abnormal is discovered during prenatal care, the client is sent to an obstetrician. Other midwives will deal with abnormal births, including breech birth. There are two main divisions of modern midwifery in the United States, nurse-midwives and direct-entry midwives. In the United Kingdom midwives are practitioners in their own right, and take responsibility for the antenatal, intrapartum and immediate postnatal care of women. In many parts of the world, midwives delivery far more children than doctors. This new book brings together the latest research on this ever-changing field.

Book excerpt: Despite today's historically low maternal and infant mortality rates in the United States, labor continues to evoke fear among American women. Rather than embrace the natural childbirth methods promoted in the 1970s, most women welcome epidural anesthesia and even Cesarean deliveries. In Deliver Me from Pain, Jacqueline H. Wolf asks how a treatment such as obstetric anesthesia, even when it historically posed serious risk to mothers and newborns, paradoxically came to assuage women's anxiety about birth. Each chapter begins with the story of a birth, dramatically illustrating the unique practices of the era being examined. Deliver Me from Pain covers the development and use of anesthesia from ether and chloroform in the mid-nineteenth century; to amnesiacs, barbiturates, narcotics, opioids, tranquilizers, saddle blocks, spinals, and gas during the mid-twentieth century; to epidural anesthesia today. Labor pain is not merely a physiological response, but a phenomenon that mothers and physicians perceive through a historical, social, and cultural lens. Wolf examines these influences and argues that medical and lay views of labor pain and the concomitant acceptance of obstetric anesthesia have had a ripple effect, creating the conditions for acceptance of other, often unnecessary, and sometimes risky obstetric treatments: forceps, the chemical induction and augmentation of labor, episiotomy, electronic fetal monitoring, and Cesarean section. As American women make decisions about anesthesia today, Deliver Me from Pain offers them insight into how women made this choice in the past and why each generation of mothers has made dramatically different decisions.

Available in PDF, EPUB and Kindle. Book excerpt: The Future of Nursing explores how nurses' roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America's increasingly complex health system. At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: 'The 4th edition of this extensive text is an outstanding resource prepared by nurses (and a librarian) for nurses. In a structured and helpful style it presents thousands of items from the literature - published papers, reports, books and electronic resources - as a clear, accessible, and most of all useful collection. The efforts to signpost and lead the reader to the sought-for information are effective and well-conceived, and the "How to use this book" section is remarkably simple...the book should be found in every nursing and health library, every research institute and centre, and close to many career researchers' desks' - RCN Research This latest edition of Resources for Nursing Research provides a comprehensive bibliography of sources on nursing research, and includes references for books, journal papers and Internet resources. Designed to act as a 'signpost' to available literature in the area, this Fourth Edition covers the disciplines of nursing, health care and the social sciences. Entries are concise, informative and accessible, and are arranged under three main sections: · 'Sources of Literature' covers the process of literature searching, including using libraries and other tools for accessing literature · 'Methods of Inquiry' includes an introduction to research, how to conceptualize and design nursing and health research, measurement and data collection, and the interpretation and presentation of data · 'The Background to Research in Nursing' encompasses the development of nursing research; the profession's responsibilities; the role of government; funding; research roles and careers; and education for research. Fully revised and updated, the Fourth Edition includes just under 3000 entries, of which 90% are new. It has extensive coverage of US, UK literature and other international resources. This new edition will be an essential guide for all those with an interest in nursing research, including students, teachers, librarians, practitioners and researchers.

Book excerpt: The delivery of high quality and equitable care for both mothers and newborns is complex and requires efforts across many sectors. The United States spends more on childbirth than any other country in the world, yet outcomes are worse than other high-resource countries, and even worse for Black and Native American women. There are a variety of factors that influence childbirth, including social determinants such as income, educational levels, access to care, financing, transportation, structural racism and geographic variability in birth settings. It is important to reevaluate the United States' approach to maternal and newborn care through the lens of these factors across multiple disciplines. Birth Settings in America: Outcomes, Quality, Access, and Choice reviews and evaluates maternal and newborn care in the United States, the epidemiology of social and clinical risks in pregnancy and childbirth, birth settings research, and access to and choice of birth settings.

Book excerpt: More than 30 years ago, the Institute of Medicine (IOM) and the National Research Council (NRC) convened a committee to determine methodologies and research needed to evaluate childbirth settings in the United States. The committee members reported their findings and recommendations in a consensus report, Research Issues in the Assessment of Birth Settings (IOM and NRC, 1982). An Update on Research Issues in the Assessment of Birth Settings is the summary of a workshop convened in March, 2013, to review updates to the 1982 report. Health care providers, researchers, government officials, and other experts from midwifery, nursing, obstetric medicine, neonatal medicine, public health, social science, and related fields presented and discussed research findings that advance our understanding of the effects of maternal care services in different birth settings on labor, clinical and other birth procedures, and birth outcomes. These settings include conventional hospital labor and delivery wards, birth centers, and home births. This report identifies datasets and relevant research literature that may inform a future ad hoc consensus study to address these concerns.

Available in PDF, EPUB and Kindle. Book excerpt: Qualitative research, particularly phenomenology, is increasingly popular as a method for midwifery and health-related research. These approaches enable rich and detailed explanations to be uncovered and bring experience to life. Important recommendations and practice- based implications may then be raised and debated for future use. This book brings together a range of phenomenological methods and insights into one accessible text. Illustrated with plenty of examples of successful phenomenological research, Qualitative Research in Midwifery and Childbirth keeps the focus applied to midwifery and childbirth and makes clear the links to practice throughout. The book introduces three key phenomenological approaches – descriptive, interpretive and the life world – and includes a comparative chapter which discusses the differences between these varied perspectives and methods. Each chapter focuses on how these approaches are used within midwifery research. The remaining chapters present a number of different research projects. These demonstrate how different phenomenological approaches have been used to explore and uncover experiences of childbirth and maternity as well as offering important insights into how women experience different facets of the birth experience during the antenatal, intra-partum and postnatal period. Designed for researchers and students undertaking research projects on midwifery and childbirth, this text includes contributions from a range of international and highly regarded phenomenological authors and researchers.
