Book excerpt: This Treatment Improvement Protocol (TIP)is a companion to TIP 51, Substance Abuse Treatment: Addressing the Specific Needs of Women. These two volumes look at how gender-specific treatment strategies can improve outcomes for men and women, respectively. The physical, psychological, social, and spiritual effects of substance use and abuse on men can be quite different from the effects on women, and those differences have implications for treatment in behavioral health settings. Men are also affected by social and cultural forces in different ways than women, and physical differences between the genders influence substance use and recovery as well. This TIP, Addressing the Specific Behavioral Health Needs of Men, addresses these distinctions. It provides practical information based on available evidence and clinical experience that can help counselors more effectively treatment with substance use disorders

Managing Managed Care offers an urgently needed assessment of managed care for behavioral health and a framework for purchasing, delivering, and ensuring the quality of behavioral health care. It presents the first objective analysis of the powerful multimillion-dollar accreditation industry and the key accrediting organizations. Managing Managed Care draws evidence-based conclusions about the effectiveness of behavioral health treatments and makes recommendations that address consumer protections, quality improvements, structure and financing, roles of public and private participants, inclusion of special populations, and ethical issues. The volume discusses trends in managed behavioral health care, highlighting the emerging role of the purchaser. The committee explores problems of overlap and fragmentation in the delivery of behavioral health care and discusses the issue of access, a special concern when private systems are restricted and public systems overburdened. Highly applicable to the larger health care system, this volume will be of particular interest to all stakeholders in behavioral healthâ€"federal and state policymakers, public and private purchasers, health care providers and administrators, consumers and consumer advocates, accrediting organizations, and health services researchers.

Available in PDF, EPUB and Kindle. Book excerpt: EXECUTIVE SUMMARY AND RECOMMENDATIONS: The goal of this TIP is to recommend guidelines for primary care clinicians to follow in caring for patients with alcohol and other drug use disorders. These guidelines were developed by a Consensus Panel of clinicians, researchers, and educators who work on the prevention and treatment of substance use disorders. Protocols are based partly on research evidence, partly on Panel members' clinical experience. The algorithm to the left follows a patient with substance use problems who presents in a primary care setting. The chart will serve as a guide or road map through screening, brief assessment, brief intervention, assessment, referral, specialized treatment, and followup care as they are detailed in the TIP. Since substance use disorders are often chronic conditions that progress slowly over time, primary care clinicians, through their regular, long-term contact with patients, are in an ideal position to screen for alcohol and drug problems and monitor each patient's status. Futhermore, studies have found that primary care clinicians can actually help many patients decrease alcohol consumption and its harmful consequences through office-based interventions that take only 10 to 15 minutes (Kahan et al., 1995; Wallace et al., 1988). This potential, however, is largely untapped: Saitz and colleagues found that of a sample of patients seeking substance abuse treatment, 45 percent reported that their primary care physician was unaware of their substance abuse (Saitz et al., in press). Yet even though screening and limited treatment of substance use disorders do not require a large time investment, the Consensus Panel that developed this TIP recognized that many primary care clinicians are already overwhelmed by the demands imposed by expanded gatekeeper functions. The Panel realized that a practical approach to addressing patients' substance abuse problems was needed: one that recognized the time and resource limitations inherent in primary care practice and offered a series of graduates approaches that could be incorporated into a normal clinic or office routine. Biological, medical, and genetic factors as well as psychological, social, familial, cultural, and other environmental features all bear on substance abuse. Addressing the condition effectively requires a team effort, especially when it has progressed beyond the early stage. For this reason, in addition to screening and intervention treatment options, these guidelines include information about viable referral for assessment and treatment, as well as followup. Readers will notice that the TIP contains more information on alcohol use and abuse than on use of illicit drugs. This reflects both the scope of the problems and the research literature available about them. It is estimated that about 18 million people with alcohol use problems and 5 million users of illicit drugs need treatment. Although the Panel recognizes that tobacco is an addictive substance with a major public health impact, it is not included in this TIP because the topic falls outside CSAT's purview. Readers are referred to "Smoking Cessation: a Guide for Primary Care Clinicians," published by the Agency for Health Care Policy and Research (Agency for Health Care Policy and Research, 1996). The Consensus Panel's recommendations are based on a combination of clinical experience and research-based evidence. In the list below, the summary guidelines supported by the research literature are followed by (1); clinically based recommendations are marked (2). Citations supporting the former are referenced in the body of the document. Screening and assessment instruments mentioned below are reproduced and discussed in Chapters 2 and 4 and Appendix C. The guidelines are presented in more detail in Chapter 6.

Book excerpt: For nearly three decades, methadone hydrochloride has been the primary means of treating opiate addiction. Today, about 115,000 people receive such treatment, and thousands more have benefited from it in the past. Even though methadone's effectiveness has been well established, its use remains controversial, a fact reflected by the extensive regulation of its manufacturing, labeling, distribution, and use. The Food and Drug Administration regulates the safety and effectiveness of methadone, as it does for all drugs, and the Drug Enforcement Administration regulates it as a controlled substance. However, methadone is also subjected to a unique additional tier of regulation that prescribes how and under what circumstances it may be used to treat opiate addiction. Federal Regulation of Methadone Treatment examines current Department of Health and Human Services standards for narcotic addiction treatment and the regulation of methadone treatment programs pursuant to those standards. The book includes an evaluation of the effect of federal regulations on the provision of methadone treatment services and an exploration of options for modifying the regulations to allow optimal clinical practice. The volume also includes an assessment of alternatives to the existing regulations.

Book excerpt: At a time when the mental health difficulties/disorders of the elderly are coming to the fore of many practitioners' patient rosters, naming and treating those problems is still too often handled as an art as much as a science. Inconsistent practices based on clinical experience and intuition rather than hard scientific evidence of efficacy have for too long been the basis of much treatment. Evidence-based practices help to alleviate some of the confusion, allowing the practitioner to develop quality practice guidelines that can be applied to the client, identify appropriate literature that can be shared with the client, communicate with other professionals from a knowledge-guided frame of reference, and continue a process of self-learning that results in the best possible treatment for clients. The proposed volume will provide practitioners with a state-of-the-art compilation of evidence-based practices in the assessment and treatment of elderly clients. As such it will be more clinically useful than anything currently on the market and will better enable practitioners to meet the demands faced in private and institutional practice. Focusing on the most current research and best evidence regarding assessment, diagnosis, and treatment, the volume covers difficulties including, but not limited to: social isolation/loneliness, elder abuse/neglect, depression and suicidal inclinations, anxiety disorders, substance abuse, dementias, prolonged bereavement, patients with terminal illnesses. Because concrete research evidence is so often not used as the basis for practice, this book provides a timely guide for clinicians, social workers, and advanced students to a research-oriented approach to serving the mental health needs of elderly adults. - Fully covers assessment, diagnosis & treatment of the elderly, focusing on evidence-based practicesConsolidates broadly distributed literature into single source and specifically relates evidence-based tools to practical treatment, saving clinicians time in obtaining and translating information and improving the level of care they can provide - Detailed how-to explanation of practical evidence-based treatment techniques - Gives reader firm grasp of how to more effectively treat patients - Chapters directly address the range of conditions and disorders most common for this patient population - i.e. social isolation, elder abuse/neglect, depression, anxiety disorders, terminal illnesses/disabilites, bereavement, substance abuse, and dementias - Prepares readers for the conditions they will encounter in real world treamtent of an elderly patient population - Cites numerous case studies and provides integrative questions at the end of each chapter - Exposes reader to real-world application of each treatment discussed - Offers reader easy base for further study of subject, saving clinicians time

Hendrickson and published by Routledge. This book was released on 2014-02-25 with total page 252 pages. Available in PDF, EPUB and Kindle. Book excerpt: Your blueprint to develop and manage effective co-occurring treatment programs! Sequential or parallel treatments for co-occurring disorders are much less effective than a quality co-occurring treatment program. Most mental health and substance abuse professionals know how to provide an effective program for individuals with a substance use or mental health disorder, but few are aware of the issues involved in providing services for those with dual diagnosis. Designing, Implementing, and Managing Treatment Services for Individuals with Co-Occurring Mental Health and Substance Use Disorders: Blueprints for Action gives you with the tools you need to develop an effective program specific to co-occurring treatment as well as to implement and manage the program's services. Author Edward Hendrickson shares his knowledge from over two decades of developing and implementing co-occurring treatment services. Designing, Implementing, and Managing Treatment Services for Individuals with Co-Occurring Mental Health and Substance Use Disorders: Blueprints for Action is thorough, insightful, and informative, covering all facets of plan and execution, helping you form strategies to address a co-occurring treatment program’s unique issues. The book examines the historical basis of the current dual treatment system to provide the philosophical tenets the program must follow and the essential qualities for an effective treatment program. It explores the issues in creating a new program or expanding an existing one, as well as the mechanics of day-to-day management. Helpful appendices, tables, and case examples illustrate the discussion. Designing, Implementing, and Managing Treatment Services for Individuals with Co-Occurring Mental Health and Substance Use Disorders: Blueprints for Action discusses: developing the necessary infrastructure for integrated treatment programs the eight essential qualities for an effective treatment program choosing a target population identifying specific services to implement implementation of services in an existing treatment setting implementation of services in a non-mental health or substance abuse setting hiring and training staff clinical supervision and human resource development program management issues multilevel and multi-organization systems steps to ensure program survival Designing, Implementing, and Managing Treatment Services for Individuals with Co-Occurring Mental Health and Substance Use Disorders: Blueprints for Action is more than an instructional text for students; it is an essential resource for any substance abuse and mental health professional considering implementing a co-occurring treatment program.
