Available in PDF, EPUB and Kindle. Book excerpt: Before the 1940s, children in the United States with severe emotional difficulties would have had few options for care. The first option was usually a child guidance clinic within the community, but they might also have been placed in a state mental hospital or asylum, an institution for the so-called feebleminded, or a training school for delinquent children. Starting in the 1930s, however, more specialized institutions began to open all over the country. Staff members at these residential treatment centers shared a commitment to helping children who could not be managed at home. They adopted an integrated approach to treatment, employing talk therapy, schooling, and other activities in the context of a therapeutic environment. Emotionally Disturbed is the first work to examine not only the history of residential treatment but also the history of seriously mentally ill children in the United States. As residential treatment centers emerged as new spaces with a fresh therapeutic perspective, a new kind of person became visible—the emotionally disturbed child. Residential treatment centers and the people who worked there built physical and conceptual structures that identified a population of children who were alike in distinctive ways. Emotional disturbance became a diagnosis, a policy problem, and a statement about the troubled state of postwar society. But in the late twentieth century, Americans went from pouring private and public funds into the care of troubled children to abandoning them almost completely. Charting the decline of residential treatment centers in favor of domestic care–based models in the 1980s and 1990s, this history is a must-read for those wishing to understand how our current child mental health system came to be.

Book excerpt: The life span of day treatment for children in the United States is relatively short, covering a period of about 50 years. Although the first 20 years saw little growth in the number of centers operating around the country, the concept of day treatment was recognized by the Joint Commission on Mental Illness and Health in 1961 as the most significant treatment innovation of this century. Enthusiasm for this treatment modality gained impetus from growing dissatisfaction among many mental health care providers who had no choice but to place children in a highly restrictive hospital environment. Day treat ment did not carry the stigma associated with inpatient placement. The children could now remain with their own families and within their own communities. The parents could be actively included in their child's treatment. This new modality avoided the short- and long-term negative effects of institutionalization, and there was a fa vorable cost discrepancy between day and inpatient mental health services. In more recent years, there has been growing evidence of the efficacy of day treatment as an intensive therapeutic environment for children and their parents. Despite these advantages, day treatment has continued to be underutilized in favor of inpatient treatment by both the psychiatric community and third-party payers. Only recently is it being acknowledged by some insurers as a therapeutically sound and financially advantageous alternative to inpatient services. Conse quently, it is showing signs of intense growth nationally.

In an opening statement, Chairwoman Schroeder discussed issues of children's mental health and suggested that the committee study: (1) the effectiveness of community-based care in a model service system in California; (2) the importance of having service systems in place prior to a crisis; (3) creative funding strategies; and (4) the impact of inappropriate services on families. A fact sheet discusses the prevalence of childhood mental health problems; youth homelessness; lack of mental health care for children; inappropriate institutionalization; inadequate research in childhood mental health; inadequate insurance; and limited federal support. Statements from other committee members were included in the record. Seven other individuals presented statements. These statements included descriptions of the attempts of the State of Virginia to restructure its services and funding to better meet the needs of troubled youths and families; a demonstration community-based services project at Fort Bragg, North Carolina; and the California model system of care, a system expanded from the earlier Ventura model. Available in PDF, EPUB and Kindle. Book excerpt: The editors of this volume are committed to the philosophy of treating emotionally disturbed children in the life milieu. Both have been intensely involved in training "online" therapists--child care workers. They are convinced that there is no one "right way" in milieu treatment, and propose a electric model for treatment. Like many of the other writers included in this book, Whittaker and Trieschman conceive of treatment as a total life experience. Th ey do not see the individual versus the group, but the individual within the group situation. They also do not see permissiveness versus limitations, professional staff versus nonprofessional staff , or the institution versus an outside of the community. The book is divided into two sections: the fi rst is a dialogue between the editors on current issues in residential treatment and problems in treating children. The second is a collection of readings. This is one of the first sourcebooks covering the therapeutic milieu for children in residential treatment centers, specifi cally emotionally disturbed children. It is also an excellent text for courses on the emotionally disturbed child, milieu treatment, and child welfare. James K. Whittaker is The Charles O. Cressey Endowed Professor Emeritus at the University of Washington School of Social Work, Seattle where he has served as a member of senior faculty since 1970. His research and teaching interests encompass child and family policy and services including residential treatment, and the integration of evidence-based practices into contemporary child & family services. He is also series editor of Modern Applications of Social Work for Transaction Publishers. Albert E. Trieschman was a Staff Clinical Psychologist at Children's Hospital Medical Center in Boston. From 1960 until his death in 1984, he was founding Executive Director of the Walker Home & School in Needham, Massachusetts.
