It is disproportionately responsible for mortality among women in developing countries compared to those in developed countries. There are several challenges associated with the effective management of breast cancer in Nigeria; financial barriers limit womens access to screening and treatment services, late-stage presentation, high incidence of triple negative breast cancers and failure in stewardship by government in their inability to provide the best possible cancer care like their counterparts in the West. There is an urgent need to step up activities through governmental and non-governmental agencies to promote advocacy, national policy on training of personnel for diagnosis, clinical and self-breast examination and nationwide screening program (mammography) in order to enhance early detection, control the upward trends and reduce the mortality rate associated with breast cancer. Routine age appropriate and specific breast screening should become an integral part of healthcare system in Nigeria allowing for early detection and intervention; aggressive awareness campaign on the advantages of early diagnosis and the dangers of late presentation, need to offer universal and affordable treatment, implementation of a strategy to offer annual mammogram to women above the age threshold for breast cancer, increased budgetary allocation for the diagnosis and management of cancer, more investment in the training of healthcare workers involved in the diagnosis and management of breast cancer, provision of Health Education encouraging women to conduct routine Breast Self Examination (BSE). BSE could become a simple, low-priced, secure, effective, appropriate and feasible screening tool in Nigeria. There is need to re-emphasize the importance of prompt reporting of any new breast symptoms to a health professional. Clinical Breast Examination (CBE) should become part of a periodic health examination, preferably at least every three years. Asymptomatic women aged 40 and over should be offered a CBE as part of a periodic health examination, preferably annually. Objective implementation of these steps can help reduce the incidence of breast cancer-related mortality in Nigeria.

While rates of breast cancer continue to rise steadily across the African continent, the disease often remains misunderstood, extremely underfunded and responsible for an inestimable number of needless fatalities. Many factors influence the experiences and the quality of life of breast cancer survivors, yet there is minimal research in Ghana and throughout West Africa on post-treatment and survivorship issues. This research offers a perspective into the experiences of Ghanaian breast cancer survivors, arguing that these Ghanaian women, often with a range of post-cancer treatment issues, are not passive, powerless victims. With the help of a survivor community, women with breast cancer are able to move from stigmatized and ostracized social roles to one that transforms and exalts their cancer experience. This research emphasises the importance of recognizing not only the structural forces that shape cancer experiences, but also the narratives of transformation, resilience and strength and the role these play in enabling survivors to exercise agency over their disease.

Book excerpt: The economic burden of breast cancer for women under 50 in the United States remains largely unexplored, in part because young women make up a small proportion of breast cancer cases overall. To address this knowledge gap, we conducted a web-based survey to compare data from breast cancer survivors 18–39 years of age at first diagnosis and 40–49 years of age at first diagnosis.We administered a survey to a national convenience sample of 416 women who were 18–49 years of age at the time of their breast cancer diagnosis. We analyzed factors associated with financial decline using multivariate regression.Survivors 18–39 years of age at first diagnosis were more likely to report Stage II–IV breast cancer (P < 0.01). They also quit their jobs more often (14.6%) than older survivors (4.4%; P < 0.01) and faced more job performance issues (55.7% and 42.8%, respectively; P = 0.02). For respondents in both groups, financial decline was more likely if the survivor had at least one comorbid condition (odds ratios: 2.36–3.21) or was diagnosed at Stage II–IV breast cancer (odds ratios: 2.04–3.51).

But these interventions remain inaccessible for many people in the world, especially those residing in low-resource communities that are characterized by a lack of fundsâ€"on an individual or a societal basisâ€"to cover health infrastructure and care costs. Few guidelines and strategies for cancer control consider the appropriateness and feasibility of interventions in low-resource settings, and this may undermine the effectiveness of care. Recognizing the challenges of providing cancer care in resource constrained settings, the National Academies of Sciences, Engineering, and Medicine developed a two- workshop series examining cancer care in low-resource communities, building on prior work of the National Academies. The first workshop, held in October 2015, focused on cancer prevention and early detection. The second workshop was held in November 2016, and focused on cancer treatment, palliative care, and survivorship care in low-resource areas. Book excerpt: The new global cancer data suggests that the global burden has risen to 18.1 million new cases per year and 9.6 million cancer deaths per year. A number of factors appear to be driving this increase, in particular, a growing and aging global population and an increase of exposure to cancer risk factors linked to social and economic development. For rapidly-growing economies, the data suggests a shift from poverty- or infection-related cancers to those associated with lifestyles more typical in industrialized countries. There is still large geographical diversity in cancer occurrence and variations in the magnitude and profile of the disease between and within world regions. There are specific types of cancer that dominate globally: lung, female breast and colorectal cancer, and the regional variations in common cancer types signal the extent to which societal, economic and lifestyle changes interplay to deferentially impact on the profile of this most complex group of diseases. Unfortunately, despite advances in cancer care, a significant proportion of patients at home, experience sub-optimal outcomes. Barriers to successful treatment outcomes include, but are not limited to: access to oncologists in the primary health centers, non-adherence, lack of experienced oncology and palliative care nurses in the community, inadequate monitoring and the lack of training of family and pediatric physicians. Telemedicine approaches, including telephone triage/education, telemonitoring, teleconsultation and status tracking through mobile applications, have shown promise in further improving outcomes, in particular for chronic cancer patients following their hospitalization. Lessons can be learned from existing hospices in North America, the United Kingdom, Australia, Centers of Excellence in African (Uganda) and modern community services in India (Kerala). An important goal of this book is to describe and encourage professionals to develop new community programs in palliative care, which include training and empowering physicians and nurses in the community on the principles of palliative care. The Middle East Cancer Consortium (MECC) together with the American Society of Clinical Oncology (ASCO) and the American Oncology Nursing Society (ONS) have conducted multiple courses ranging from basic palliative care to more specialized training in palliative care for multiple nationalities in Europe, Asia and Africa. Our experience clearly indicates that, to promote such activities, one needs strong leadership and confirmed political will to support the endeavor. The new book will emphasize the importance of having a core of multiple stakeholders including community leaders, government, NGOs and media to be actively involved in advocating for the cause and generating public awareness. This text will provide the reader with a comprehensive understanding of the outside-of-the-hospital treatment of cancer patients by medical, paramedical and volunteer personnel. In doing so, this text will encourage the creation of new palliative care services improving upon the existing ones and stimulate further research in this field. Part 1 of the text will begin with an overview of the current state of affairs of services provided to cancer patients while being cared for by primary health centers. It will also review the current literature regarding medical and psychological-based therapy options in the community for cancer patients at different stages of their disease. Part 2 will address the unique role of the community nurse, within the framework of the multidisciplinary team treating the patient, in the attempt to provide optimal evaluation and care in very challenging situations (such as with terminal patients). Part 3 will provide insightful models of this new discipline and serve as a valuable resource for physicians, nurses, social workers and others involved in the care of cancer patients. The book will take a multidisciplinary approach, integrating clinical and environmental data for practical management to enhance the efficacy of treatment while relieving suffering. Part 4 will also discuss the application of modern technological approaches to track symptoms, quality of life, diet, mobility, duration of sleep and medication use (including pain killers) in chronic cancer patients in the community. Part 5 of the book will also be devoted to modes of developing a collaborative program between governmental and non-governmental organization sectors. This includes volunteer workers in close collaboration with medical professionals for providing emotional and spiritual support, nursing care, nutritional support and empowering family caregivers. Such a model makes palliative care in the community a “people’s movement”, thus transferring part of the responsibility and ownership to the community.
