Devant un couple infertile reçu en cabinet de médecine générale, nous précisons les facteurs à prendre en compte pour décider d'une attitude permettant d'optimiser les chances de grossesse. Les options étudiées sont les mesures hygiéno-diététiques, le traitement des facteurs favorisants, l'induction de l'ovulation par citrate de clomifène et l'orientation du couple vers un médecin spécialiste de l'assistance médicale à la procréation. Du fait de leur impact sur les chances de conception, les mesures hygiénodiététiques et la correction d'un surpoids doivent être la priorité, ainsi que la correction des facteurs favorisant l'infertilité (infections génitales, sevrage tabagique, information sur l'effet de l'âge). La place du traitement par induction de l'ovulation en soins primaires doit être repensée en tenant compte des importants travaux qui ont modifié la prise en charge du syndrome des ovaires polykystiques (SOPK). Le citrate de clomifène (CC) est le seul inducteur de l'ovulation qui peut être prescrit par le médecin généraliste. Il est recommandé en 1ère intention dans l'anovulation de type II, mais ne doit plus être proposé dans l'infertilité inexpliquée. Compte tenu des évolutions récentes, la prise en charge thérapeutique de l'infertilité en médecine générale nécessite une formation accrue, une harmonisation des pratiques et une bonne coordination avec les spécialistes de l'infertilité, notamment les centres AMP.

La littérature accorde une place importante aux MG dans leur prise en charge mais peu d'études ont évalué leur rôle. Objectif: Évaluer la pratique des MG dans la prise en charge des couples infertiles. Méthodes: Enquête déclarative par questionnaire envoyé par courriel aux 1750 MG libéraux de Midi-Pyrénées du listing e-mail de l'URPS. Résultats: 174 réponses dont 138 complètes. 93% ont déjà eu une consultation pour infertilité et 69% considèrent qu'il s'agit d'une problématique de médecine générale. Dans leur démarche diagnostique, 91% interrogent les deux partenaires. 83% examinent la femme, 70% l'homme, 2/3 systématiquement les 2. 57% prescrivent un bilan. Echographie pelvienne (88%), courbe de température (85%), FCU (68%), FSH, LH (68%) et sérologie Chlamydia (63%) sont les examens les plus prescrits chez la femme. Chez l'homme, 90% prescrivent un spermogramme. Gynécologues et médecins d'AMP sont les principaux correspondants, bien que la communication ne soit pas satisfaisante (66%). Conseils, informations et accompagnement sont assurés par la majorité des MG. 17% ont déjà prescrit un traitement pour infertilité. Seulement 8% affirment connaitre les effets indésirables des traitements et 77% considèrent leurs connaissances sur l'infertilité comme insuffisantes. Faible fréquence en pratique quotidienne, formation insuffisante et hyperspécialisation sont les principales difficultés citées. Conclusion: La diffusion d'un référentiel pour la prise en charge initiale, une meilleure formation et une meilleure communication avec les spécialistes semblent nécessaires pour optimiser la prise en charge de l'infertilité en soins primaires.

Les pratiques de prévention primaire et secondaire de l’infertilité diffèrent chez les médecins interrogés. La méconnaissance des facteurs de risque d’infertilité et des recommandations, le manque de conviction sur l’efficacité d’une telle prévention et des difficultés à cibler la population à informer, les questionnements éthiques sur le rôle du médecin traitant, et certains freins pratiques semblent influencer leurs démarches de prévention.Les informations collectées ont permis d’identifier les obstacles à la prévention de l’infertilité en médecine générale et ont fait naître des pistes de réflexion pouvant permettre de la faciliter. Une sensibilisation des professionnels de santé sur la problématique de l’infertilité et ses facteurs de risque, une formation aux techniques de communication, une information globale des jeunes patients en médecine générale et médecine scolaire, une valorisation des actes de prévention par la création d’une consultation de prévention dédiée au couple, pourraient permettre d’améliorer la prévention de l’infertilité en soins primaires.

This takeoff can be abetted by a demographic dividend from the changes in population age structure. Declines in child mortality, followed by declines in fertility, produce a 'bulge' generation and a large number of working age people, giving a boost to the economy. In the short run lower fertility leads to lower youth dependency rates and greater female labor force participation outside the home. Smaller family sizes also mean more resources to invest in the health and education per child boosting worker productivity. In the long run increased life spans from health improvements mean that this large, high-earning cohort will also want to save for retirement, creating higher savings and investments, leading to further productivity gains. Two things are required for the demographic dividend to generate an African economic takeoff. The first is to speed up the fertility decline that is currently slow or stalled in many countries. The second is economic policies that take advantage of the opportunity offered by demography. While demographic change can produce more, and high quality, workers, this potential workforce needs to be productively employed if Africa is to reap the dividend. However, once underway, the relationship between demographic change and human development works in both directions, creating a virtuous cycle that can accelerate fertility decline, social development, and economic growth. Empirical evidence points to three key factors for speeding the fertility transition: child health, female education, and women's empowerment, particularly through access to family planning. Harnessing the dividend requires job creation for the large youth cohorts entering working age, and encouraging foreign investment until domestic savings and investment increase. The appropriate mix of policies in each country depends on their stage of the demographic transition.
