Le traitement s’oriente selon deux axes : ouverture ou fermeture orthodontique des espaces d’agénésie (sachant que l’ouverture d’espace doit être associée à une thérapeutique implantaire ou un bridge collé). Les progrès récents en implantologie permettent de répondre à beaucoup de cas d’agénésie d’incisive latérale. Ce traitement nécessite néanmoins un investissement important de la part du patient et du praticien, ce qui en fait une thérapeutique difficile et délicate à mettre en œuvre. De ce fait la fermeture d’espace orthodontique par substitution canine ainsi que la prothèse collée sont toujours considérées comme des techniques de choix pour remplacer des incisives latérales maxillaires absentes.Souvent diagnostiquée en denture mixte, la difficulté d’un tel traitement est de prendre une décision définitive alors même que la bouche est en pleine croissance. La céramique utilisée est une vitrocéramique renforcée au Disilicate de lithium: en plus d'être apte au collage, elle présente des propriétés optiques et mécaniques intéressantes. Pour mettre en œuvre les bridges collés cantilever tout céramique, la céramique et l'émail de la dent pilier sont préparés de manière spécifique. L'orthodontiste collabore avec le chirurgien-dentiste en aménageant un espace correspondant à la largeur de l'incisive latérale maxillaire. Le chirurgien-dentiste prépare le site gingival, la dent pilier avec un design de préparation précis et colle le bridge collé cantilever tout céramique. Le bridge collé cantilever tout céramique n'est pas seulement un moyen transitoire de remplacement de l'incisive latérale maxillaire chez le patient présentant des contre-indications au traitement implantaire. Il représente une thérapeutique à part entière pour le patient chez qui la pose d'implant ne peut pas être réalisée pour raison médicale ou autre et le sera d'autant plus si de nouveaux matériaux céramiques aux propriétés mécaniques améliorées sont développés.

Book excerpt: This important theoretical work by Paul de Man sets forth a mode of reading and interpretation based on exemplary texts by Rousseau, Nietzsche, Rilke, and Proust. The readings start from unresolved difficulties in the critical traditions engendered by these authors, and they return to the places in the text where those difficulties are most apparent or most incisively reflected upon. The close reading leads to the elaboration of a more general model of textual understanding, in which de Man shows that the thematic aspects of the texts--their assertions of truth or falsehood as well as their assertions of values--are linked to specific modes of figuration that can be identified and described. The description of synchronic figures of substitution leads, by an inner logic embedded in the structure of all tropes, to extended, narrative figures or allegories. De Man poses the question whether such self-generating systems of figuration can account fully for the intricacies of meaning and of signification they produce. Throughout the book, issues in contemporary criticism are addressed analytically rather than polemically. Traditional oppositions are put in question by a rhetorical analysis which demonstrates why literary texts are such powerful sources of meaning yet epistemologically so unreliable. Since the structure which underlies this tension belongs to language in general and is not confined to literary texts, the book, starting out as practical and historical criticism or as the demonstration of a theory of literary reading, leads into larger questions pertaining to the philosophy of language.
