Book excerpt: Three-dimensional analysis of mandibular growth provides the potential for pedodontists, orthodontists and surgeons to prescribe treatment that works in harmony with the individual growth of the patient. Despite efforts by 3D pioneers, the visualization of growth and development remains reminiscent of the landmark-based cephalometric analyses used in two-dimensional studies. The objective of this study was to identify 3D topographical changes of the mandible during growth and development of adolescent orthodontic patients. Nine pairs of pre- and post-orthodontic cone-beam computed tomography (CBCT) scans were used to generate mandibular surfaces. Surfaces were superimposed on trabecular bone in the anterior mandible using a mutual information algorithm, and topographical changes were visualized and quantified. The intra- and inter-rater intraclass correlation coefficients for surface generation (0.94 and 0.93, respectively) and superimposition (0.96 and 0.82, respectively) demonstrate the reliability of the techniques. The findings of this study support the theories of bone remodeling reported in histological, implant-based, and landmark studies of mandibular growth.

Traditionally, evaluation of mandibular growth uses 2D cephalometric analyses. While still used by orthodontists today, these 2D analyses are fraught with errors in measurement associated with interpreting 2D projections of 3D structures. Cone Beam Computer Tomography (CBCT) provides a means to study craniofacial structures in 3D. The objective of this study is to identify 3D topographical changes of mandible during the period of growth and development in orthodontic patients. Ten subjects with serial CBCT images were selected from the Bolton-Brush collection. The age of the subjects ranged from 8 to 14 years-old. Mandibular growth is evaluated using serial pairs of CBCT images which are segmented, superimposed using regional voxel-based mutual information, and quantitatively evaluated in AmiraXImage®. Surface topography changes over time in these 19 subjects are reported as follows: 56% of surface remodeling at a threshold of 0.5mm, median remodeling of 0.6mm, 0.8mm 0.6mm SD. Intra- and inter-rater ICC reliability for for surface generation (0.82 and 0.90, respectively) and superimpositions (0.94 and 0.93, respectively) demonstrate the reliability of the technique. The findings of this study support theories of bone remodeling reported in histological and radiographic studies of mandibular growth.

Book excerpt: Objectives: Current methodologies in the field of morphometrics still employ the use of 2-dimensional images followed by a landmark-based Procrustes superimposition method to evaluate differences in shape, which can be tedious, subject to operator error, and fail to capture the true nature of shape variation between samples. The primary objective of this study is to address these limitations through the exploration and application of current methodologies used in neuroimaging and brain mapping to the field of morphometrics. By collaborating with Paul Thompson and his team at the Laboratory of Neuroimaging (LONI), we aim to: 1. Generate a 3D surface image of the mouse mandible and create an average surface for a cross sectional sample of parental strains in the Hybrid Mouse Diversity Panel (HMDP), as well as a developing longitudinal sample within a single strain. 2. Apply and modify the latest technologies used in brain mapping research to identify regional shape differences between the mandibles of these samples in 3 dimensions without the use of landmark identification, with the ultimate goal of identifying heritable quantitative traits. 3. Develop a useful, intuitive and visual method for evaluation of shape differences. Methods: A total of fifty-three mice were studied. Ten mice, equally divided across five parental inbred strains, were obtained from the HMDP, as well as an additional 43 animals at various ages from the C57BL/6 strain. Skulls were subsequently scanned using [mu]CT, then DICOM files transferred into a Beta version of Dolphin Imaging ® 11.7 Software. Hemi-mandibles were segmented from the entire skull and surface meshes were created using the same software. Topological correction of each hemi-mandible was performed before parametric registration and resampling of the surfaces to the same number of mesh points. Once registered to each other, a population average was then created and used as a reference template for shape comparison. Shapes were compared using the independently validated neuroimaging techniques of medial axis and tensor-based morphometrics. Results: Population averages were created for the mandibles and four specific areas of significant shape change were identified in both the interstrain and intrastrain samples. Visual heat maps were also created to display shape differences. Conclusions: Tensor-based morphometric evaluation of mandibular shape has far superior visualization and localization potential when compared to the current method using landmark-based analysis, while offering a more reliable solution as it eliminates the need for consistent landmark placement. Coupled with medial thickness computation, followed by additional refinement, this methodology could potentially be applied to a variety of applications concerned with evaluating shape difference in three dimensions.

Book excerpt: The purpose of this study was to find a suitable method for studying growth and development of the human dentition during the first two years of life, and having established such a method, to study in detail: 1. Changes in dimension of the maxillary and mandibular dental arches. 2. Changes in dimension of the vault of the palate. 3. Occlusal relation of the dental arches. 4. Eruption and movement of the deciduous teeth. Dental casts were obtained from 38 healthy Edmonton children, during a 21-month period. In all but five cases the first cast was obtained prior to the eruption of any deciduous teeth. Subsequent casts were obtained at six-month intervals, giving a total of one hundred and ten pairs of dental casts for the entire group. Growth changes were studied by means of direct measurements of the casts and by symmetrograph and pantograph tracings. Some of the observations are as follows: There is an active growth period in the dental arches during the first six months of life followed by a leveling off in the anterior region. Active growth continues in the posterior region of the dental arches after the six months period and particularly so in the mandibular arch. The observed growth changes in the dental arches appear to be associated with the eruption of the deciduous teeth.
