Book excerpt: In the early stages of planning the Third International Conference in System Science in Health Care, the steering committee members, most of whom had participated in the first conference in Paris (1976) and the second in Montreal (1980), made some basic decisions about organization of subject matter. The earlier meetings had been very successful in bringing together specialists from the health professions and the traditional sciences. In addition to physicians and nurses, these were representatives of the disciplines of the behavioral sciences, system theory, economics, engineering, and the emergency fields of management science and informatics -all concerned with the development of health resources in a broad system context. The reported research and experience of the many disciplines represented had dealt with one or more of three concerns: 1) a major health problem, such as cardiovascular disease, or an important popUlation at risk, such as the elderly or children or workers; 2) some generic aspect of organization and decision making, including trial and evaluation ofinnovative health strategies; and 3) the methodology of research and analysis in system of health service. The challenge to the conference organizers lay in the eliciting and arranging of experiences in such a way that the health services could be seen as purposeful,living, evolving systems.

More and more, issues of health enter the stage and curative medicine is not just loosing ground but some interest. 'How effective is health education?' and 'Whose task is primary pre vention?' are questions which stimulate great concern and public debate. It seems also to be the first time since the last decades of the 19th century that the social dimension of health receives adequate consideration. Thus the reeent publication of the so-called Black Report on Inequalities in Health (Penguin 1982) led to a heated discussion in Great Britain, certainly of relevance to many other countries. The 6 years since 1978 are much too short a time to proceed far towards 'Health for All', as the slogan of the World Health Organization puts it, but the concepts are spelled out, vaguely but convincing to many of us. It is the right time to discuss first experiences and to move from philosophy to empiri eal work. This work - as it relates to the process evolving since Alma Ata - is characterized by participation, cooperation and prevention: Participation, because 'disease' concerns only the individual patient, his or her family and immediate social environment, while 'health' concerns everyone and should involve everyone. It is a task of the whole of society rather than the domaine of a single profession or party.

Divided into three parts, this volume covers social theory, design theory, and the sociotechnical system with respect to CSCW. The next section concentrates more on the lessons that can be learned at the design stage given that one wants to build a CSCW system incorporating these insights--what kind of work does one need to do and how is understanding of design affected? Accord ingly, a conference was held in Hannover in 1980 where the first ap proaches were described and experiences in system analysis, system construction and evaluation were discussed (Rienhoff, O. and Abrams, M.E. (eds.): The Computer in the Doctor's Office, North-Holland, Amsterdam: 1980). Computer hardware and software development has been rapid over the last years. Thus the prerequisites for a successful support of the work of the physician in his office have improved. But system con struction still lags behind and the actual penetration of systems is lower than 2 % of the doctors' offices in most countries. This applies to industrialized countries. However, attempts are made everywhere to improve primary care by means of modern information technology. Information systems depend upon the real environment into which they are placed: administrative procedures govern priorities and procedures in doctor's office computers, possibilities to defray cost upon the various carriers or the patients have a great influence on the propagation of systems. Furthermore, various procedures of accounting or re-imbursement may lead to a facilitation or to a delay of the in troduction of systems. The 'art of medical practice' has reached a comparable standard within at least the industrialized countries.

While policy decision making remains at the country level, there is a strong possibility that indirectly, as a result of ideological imperatives and financial constraints, policies will change. National health policy is a case in point. This important volume explores the current and probable effect of European integration on health care protection. Will it tend to encourage all European member states to provide equitable and universal access to quality care? Or is the European integration process likely to lead to social exclusion of some? The high degree of social welfare as a health expectancy holds great significance for decisions in countries like the United States facing similiar pressures for expanded coverage. In answering these questions, Panos Minogiannis examines policies in Greece, France, Germany, and the Netherlands. Minogiannis frames his argument through an exploration of the history of the institutionalization of health care. Chapter 1 explores the nature of challenges that health care faces in an era of integration and the ways in which these challenges have emerged. Chapter 2 discusses centralization of governance in Brussels, describing the structure and relations of different European Union institutions, and their interactions with member states. The final portions of the book, through case studies of the Dutch, French, German, and Greek health reforms, explore the history of the political development of health care institutions with a particular interest in reform proposals in the last fifteen years. Chapter 7 brings together lessons from previous chapters and discusses the dynamics of health policy making in the European Union. Minogiannis concludes that health insurance will most likely remain at the member state level as far as politics are concerned, at least for the present, although policy makers will most likely have to deal with the issue of cross-border health more comprehensively than in the past. Those interested in comparative policy, and in particular health care policy, will find this volume highly informative reading. Those interested in the impact of European integration will find it provocative. Panos Minogiannis is with the Mailman School of Public Health at Columbia University and is a research associate at the Eisenhower Center.
