Book excerpt: Hypertension, or elevated blood pressure, is a major risk factor for various cardiovascular, renal diseases, and stroke. The form of hypertension with no identifiable cause is referred to as Essential Hypertension. Familial studies indicate that Essential Hypertension is heritable and, thereby, classical genetic approaches have been applied on both human and other mammalian models of hypertension to map the locations of the allelic variants within quantitative trait loci for blood pressure. The post genome era has further elevated this area of research into large-scale genome-wide association studies of hypertension in humans. Collectively, these studies have resulted in the prioritization and cataloging of several genomic regions containing allelic variants as candidates linked or associated with essential hypertension. Further, they are providing evidence to suggest that the inheritance of hypertension is rather complex, encompassing multiple variants both within protein-coding and non-coding annotations, each of which may act independently or interactively with other genes and/or environmental factors to differentially regulate blood pressure. This book provides an overview of the various methods employed to study the genetics of hypertension and discuss the progress and prospects of this area of research that may contribute towards individualized clinical management of hypertension in the future.

Book excerpt: The Clinical Research Institute of Montreal, inaugurated in April 1967, intro duced a new concept of organization of clinical research in teaching medical centers. Adjacent to the Hotel-Dieu Hospital, a major teaching hospital of the University of Montreal, the "professional" or career clinical investigators were grouped under one roof with common auditorium, seminar rooms, instruments rooms, cold rooms, experimental surgical rooms and illustration department, with administration, accounting division and machine shops at the service of the research workers. Besides its large multidisciplinary team working on various aspects of hyper tension, the Institute includes research teams in the following fields: 1) neuro biology and Parkinson's disease, 2) lipids and atherosclerosis, 3) molecular biology, 4) immunochemistry, 5) pituitary polypeptides and proteins, 6) nutrition and 7) clinical pharmacology of new drugs. Research is financed entirely by grants obtained from accredited agencies, mostly from the Medical Research Council of Canada. These grant monies constitute about 50 % of the entire budget of the Institute while the other half for administration, maintenance and physical facilities is financed through the support of the Ministry of Social Affairs of Quebec.

Scientists from 13 countries - from western, southern, and eastern Europe, Australia, and the United States - were present. The meeting was held in Cologne in March 1985. After previous meetings and workshops at Titisee in the Black Forest and several symposia on the renin angiotensin-aldosterone system here in Cologne, this symposium on prima ry hypertension was organized in order to induce and to stimulate critical discussion on the basic pathogenetic mechanisms involved and the recently established therapeutic implications. Numerous studies have been based on the concept that the elevation in blood pressure is mediated by several vasopressor substances. For this reason the renin-angiotensin system has been considered to be the essential endocrine mechanism involved in developing and sustaining arterial hyper tension. Other vasopressor factors, such as catecholamines, vasopressin, and serotonin, have been studied extensively and were assumed to play additional parts in blood pressure regulation. However, several other ap proaches have been initiated in recent years.

Observing their dietary habits, he determined that their primary food was meat, both lean and fatty, and that their diets were very low in sugary or starchy carbohydrates. Stefansson’s classic Not By Bread Alone chronicles a 1928 scientific experiment, conducted by the Russell Sage Institute of Pathology at Bellevue Hospital in New York, in which Stefansson and his colleague Dr. Karsten Andersen ate a meat-only diet for one year. The two men stayed healthy and fared very well, leading him to claim that we should reexamine our notion of what foods constitute a healthy diet. Later chapters promote the benefits of pemmican, a compact, portable, and high-energy food consisting of a concentrated mix of fat and protein made from dried lean bison meat—sometimes mixed with berries—what you might call the original energy bar.

It goes without saying that Adalat has changed the company as a whole in its research, development, and marketing, both in Germany and, most importantly, world wide. In research and development, Adalat has led the way in to new areas such as hypertension, acute neurology, dementia, atherosclerosis, and heart failure. In pharmacology, Adalat was present at the dawning of the second phar macological revolution, which started out from whole animal models and progres sed, via in vitro screening and pharmacological research on the DHP receptor, down to the molecular level with the cloning of calcium-channel proteins. Having followed the Adalat story closely in its early years as a bench scientist, let me take this opportunity to thank the inventors, the scientists, and the investigators who have helped to tap the broad potential of calcium-channel blockade with dihyd ropyridines in general and Adalat in particular. But, success should not obscure our vision. There is no reason for us to rest on our laurels: it will take true innovations in cardiovascular research to top the gold standard set by Adalat. Let us take up the challenge.
