Les parodontites et les maladies cardiovasculaires partagent des facteurs de risques communs (tabac, stress, âge, diabète...) Mais ces facteurs de risques n’expliquent pas en totalité la survenue des pathologies affectant le système cardiovasculaire. Il est communément admis depuis nombreuses années que certaines pathologies systémiques influencent la santé parodontale. En revanche, ce n’est que récemment que les malades. parodontales ont été considérées, au même titre que d’autres affections chroniques, comme des facteurs de risques pour la santé générale. Ainsi certains auteurs ont montré l’existence d’une association entre les maladies cardiovasculaires et les pathologies parodontales. D’autre part, les thérapeutiques parodontales non chirurgicales (traitement conventionnel ou désinfection globale) entraînent une bactériémie. La présence de bactéries parodontopathogènes, mise en évidence dans les plaques d’athérome et au niveau des lésions de l’endocarde, en est le résultat. L’une des conséquences directes de cette bactériémie d’origine bucco-dentaire est l’endocardite infectieuse. Elle est rencontrée aussi bien chez des patients à risque, que sans antécédent cardiovasculaire, et attribuable à un foyer dentaire dans 25% des cas. Les conséquences indirectes correspondent à une modification des médiateurs de l’inflammation expliquant ainsi le lien entre la maladie parodontale, la bactériémie et les maladies cardiovasculaires. Ainsi compte tenu de l’association entre les maladies parodontales et les maladies cardiovasculaires, on peut se demander s’il existe un risque cardiovasculaire, induit par la bactériémie, dans le traitement non chirurgical des maladies parodontales.
