There was also opposition from some sections of the medical establishment who feared the loss of professional autonomy. Setting health policy in both an historical and modern context (post 1997) Carrier and Kendall weigh up the successes and failures of the National Health Service and examine the conflicts which have continued for over sixty years, in spite of efforts to solve financial problems in the NHS through increases in funding as well as structural and organisational change. After looking at recent responses to supposed failures of the NHS, they conclude that the NHS has successfully faced the challenges before it and is likely to continue to meet the changing health needs of the population. Financial stresses, concerns about the quality of care and demographic change, with consequent issues for the elderly and the chronically ill, continue to be urgent and politically contentious issues. This book is appropriate for a wide range of undergraduate and postgraduate students studying health policy and the NHS.

In this cross-national, empirical study of the workings of voluntary agencies, Ralph M. Kramer cuts through the conceptual confusion surrounding voluntarism and the boundaries between the public and private sectors. He draws on a survey of voluntary agencies helping disabled people in four welfare democracies (the United States, England, Israel, and the Netherlands) to explain the virtues and flaws of different patterns of government-voluntary relationships in coping with the growing demand for human services. Kramer concludes that many of the most cherished beliefs about the voluntary sector have little basis in fact. The most innovative agencies, for example, are not the smallest, but rather among the largest, most bureaucratized, and most professionalized. Government funding does not necessarily constrain agency autonomy. And giving voluntary agencies the primary responsibility for social services can reduce, not increase, citizen participation. This comparative analysis of the distinctive competence, vulnerability, and potential of the voluntary agency should replace some of the myths that guide public policy and the day-to-day activities of social service agencies. Book excerpt: In the early 1980s, the welfare state, for too long regarded as a notable contribution to the establishment of a humane social order, had over the previous decade come under increasing attack. Some of its critics, especially in the UK and the USA, maintained that it had failed to deal satisfactorily with the problem of poverty. Others held that it was over-elaborate, created a psychology of dependence and imposed costs that needed to be reduced as part of a policy of general economic recovery. In a number of countries, cuts had already been imposed or were now contemplated. In this situation it was crucially important to direct attention once more to the basic objectives of the various welfare services from a systematic and comparative standpoint. Originally published in 1982, the authors of this book, one an economist and the other a specialist in social administration, subjected these aims to rigorous analysis and discuss the underlying issues of social philosophy. They then attempt to assess the various methods adopted for their attainment in Britain and comment on those adopted in the USA and in some continental European countries. Although the authors reject the more extreme assertion that the welfare state has been a failure, they point to the need to relate some of the policies followed more clearly to the basic objectives. A number of proposals for reform are put forward which would imply some change of emphasis and should permit a simplification of existing over-complex arrangements.
