Book excerpt: One of the most time-consuming tasks in clinical medicine is seeking the opinions of specialist colleagues. There is a pressure not only to make referrals appropriate but also to summarize the case in the language of the specialist. This book explains basic physiologic and pathophysiologic mechanisms of cardiovascular disease in a straightforward manner, gives guidelines as to when referral is appropriate, and, uniquely, explains what the specialist is likely to do. It is ideal for any hospital doctor, generalist, or even senior medical student who may need a cardiology opinion, or for that ma.

Studies of hypertrophy, heart failure, scribed the dynamic nature of blood flow, but ischemia, and infarction have been vigorously the concept of defining ventricular function was pursued in experimental animals and in human first introduced with the measurement of a subjects, and as a result new areas for study have emerged. These include the process of hypertro mare's blood pressure by Steven Hales in 1733. Book excerpt: Tetralogy of Fallot is the most common form of cyanotic congenital heart disease, and one of the first to be successfully repaired by congenital heart surgeons. Although “fixed”, patients born with tetralogy of Fallot cannot be considered “cured”. Improving survival and quality of life for this ever-increasing adult population will continue to challenge the current and future generations of cardiologists. Adult patients with tetralogy of Fallot should be seen by a cardiologist specializing in the care of adults with congenital heart disease, to be monitored for late complications. They need to be checked regularly for any subsequent complications or disturbances of heart rhythm. This monograph is intended as both an introduction to the subject and a timely, comprehensive review, and will be welcomed by adult cardiologists, pediatric cardiologists, internists, surgeons, obstetricians, and intensivists who wish to learn about the most recent discoveries and advances concerning tetralogy of Fallot in adults. It will also be of interest to advanced undergraduates wanting to learn more about the subject.

A clinical pediatric cardiologist is expected to be adept at the non-invasive diagnosis of congenital heart disease and those who plan to specialize in echocardiography will need to have knowledge of advanced techniques. Echocardiography in Pediatric and Congenital Heart Disease addresses the needs of trainees and practitioners in this field, filling a void caused by the lack of material in this fast-growing area. This new title comprehensively covers the echocardiographic assessment of congenital heart disease, from the fetus to the adult, plus acquired heart disease in children. Topics covered include: ultrasound physics laboratory set-up a protocol for a standard pediatric echocardiogram quantitative methods of echocardiographic evaluation, including assessment of diastolic function in depth coverage of congenital cardiovascular malformations acquired pediatric heart disease topics of special interest, such as 3D echocardiography, transesophageal echocardiography, and fetal echocardiography The approach of this book is a major advancement for educational materials in the field of pediatric cardiology, and greatly enhances the experience for the reader. An accompanying DVD with moving images of the subjects covered in the textbook will further enhance the learning experience.

Book excerpt: The heart and lung are intricately linked. When the heart is affected by disease, the lungs will often show some related pathological or clinical conditions and vice versa. Pulmonary heart disease is by definition a condition when the lungs cause the heart to fail. The left ventricle in combination with the other structures in the “left heart” pumps blood throughout the body. The right ventricle (and structures of the “right heart”) pumps blood to the lungs where it is oxygenated and returned to the left heart for distribution. In normal circumstances, the right heart pumps blood into the lungs without any resistance. The lungs usually have minimal pressure and the right heart easily pumps blood through. However when there is lung disease present, like emphysema, chronic obstructive lung disease (COPD) or pulmonary hypertension- the small blood vessels become very stiff and rigid. The right ventricle is no longer able to push blood into the lungs and eventually fails. This is known as pulmonary heart disease. Pulmonary heart disease is also known as right heart failure or cor pulmonale. The chief cause of right heart failure is the increase in blood pressure in the lungs (pulmonary artery).

Book excerpt: The last 10 years has seen explosive expansion of the number of centres performing cardiov- cular magnetic resonance (CMR) imaging. The majority of this expansion has been in the ? eld of adult ischaemic imaging, but congenital heart disease remains one of the main indications for CMR. Importantly, the greatly improved survival of patients with congenital heart disease gives us a burgeoning adult population living with the sequelae of the disease (grown-up c- genital heart disease – GUCH). Without previous experience or formal training, the interpretation of CMR images of patients with congenital heart disease can be dif? cult. The main aim of this book is to create a portable resource that offers ef? cient access to high-quality MR (and where appropriate, CT) images of the common congenital and structural heart abnormalities. We hope that by prov- ing key images for each condition and a clear interpretation of the MR appearances, we will improve the reader’s understanding of the conditions, facilitate their interpretation of images and optimise the planning of the imaging protocols during their own practice of congenital CMR. As with any publication from a single institution, the contents of this book represent our own practice. We have not written a de? nitive or exhaustive description of the conditions.
