The editor is therefore correct that the ureter itself should be considered a major organ. Though it has been regarded in the recent past as a simple muscular tube, reacting to stretching or filling by contraction, this simplistic view of ureteral phys iology is changing fast. With expanded knowledge of ureteral physiology, a pharmacology is developing which is becoming useful to the clinician in many ways. One of the most interesting aspects of the ureter is its role in inducing the permanent kidney, the metanephros. Relatively slight displacements in the origin of the ureteral bud result in ectopic ureteral orifices and a wide range of congenital anomalies. An ureteral bud which arises medial to the normal position at the genu of the mesonephric duct results in a lateral, and usually incompetent, ureterovesical junction after the duct is taken up to form a portion ofthe trigone. This appears certainly to be the developmental mechanism which results in primary reflux.

The predominant indication is still calculous obstruction but reparative and plastic surgery of the ureter for congenital and acquired lesions are among the most interesting advances in modern urology. The essential goal of this surgery is to ensure free flow of the urine from the kidney to the bladder and so to preserve or improve kidney function which is always affected or threatened by any defect in the excretory pathway. The ideal is to re-establish a closed circuit flow from kidney to bladder by repair or substitution of the ureteral conduit so as to avoid as far as possible the dis abilities that result from diversion of the urine to the skin surface or to the intestine. This objective can now be achieved in the treatment of most of the diseases of the ureter when the relevant kidney is sufficiently healthy to merit conserva tion. The techniques used are not necessarily new but the advent of antibiotics has made it possible to avoid or reduce the risks of infection, pyelonephritis, or pyonephrosis that so often complicated such surgery in earlier times. Progress in the investigation of renal function and of the excretory tract has brought a better understanding of the physio-pathology of the urinary apparatus and more accurate judgement of the results of reparative surgery.

Intravenous urography (VOLKMANN, 1924; VON LrCHTENBERG, PFLAUMER and SWICK, 1929) lent roentgen examinations a more functional aspect. The word "urodynamics" refers to the functional approach to the urinary trac- from the renal pelvis and calyces, the ureter and bladder down to the bladder neck and urethra. The embryologically denned topographie regions of the urogenital tract are peripheral relays, while the actual site of action of both normal and patho logical urodynamics is to be found in the various hollow muscular organs. Despite extensive basic research and much knowledge acquired in this neld, the active transport system between site of production and storage, i. e. the renal pelvis and ureter, is still regarded in clinical practice mainly from the anatomic-morphological viewpoint. It is not easy to obtain adequate information for the clinical assessment of functional disturbances of the ureter. For instance, retrograde exploration methods may often be complicated by infection. Clinically applicable methods for functional assessment of the upper urinary tract are (RUTISHAUSER): direct observation of the ureteral orinces, intravenous and retro grade visualization by roentgen contrast medium, sequential pyelography, roentgen kymography, and cinematography. Such methods allow good functional exploration by an experienced investigator but they yield hardly any objectively reproducible records.
