Book excerpt: Excerpt from Hip Disease in Childhood: With Special Reference to Its Treatment by Excision The greater part of the present work has been already published in America, in the Archives of Pediatrics; and most of the cases in Appendix I., with some of those in Appendix II., have appeared in the annual numbers of Abstracts, edited by the staff of the General Hospital for Sick Children, Manchester. The text has, however, been much revised and expanded from the American issue, into which many mistakes crept from my having no opportunity of correcting proof sheets. The list of excisions has been enlarged, and later records of the condition of many of the cases obtained. That this record is not more complete is due to the practical impossibility of tracing many patients after their discharge from hospital. Most of the cases, however, have been followed up for considerable periods. I have thought it worth while to publish the work in its present form since hip disease is one of the subjects on which the most directly conflicting views are still held, especially as to the question of excision, and the results of an experience of several hundred cases cannot be without interest. The present uncertainty in the matter is, in my opinion, to no small extent due to the habit of assuming that all joints of the body are affected by disease in the same way, while, in fact, the effects of disease are as different as is the construction of the several joints, and the hip stands much by itself in its pathology. It will be seen that the head of the femur has been crowded upward and backward. This in typical cases continues until the head of the femur is partially absorbed, the acetabulum enlarged, and finally a subluxation takes place, and the exaggerated pressure of the femur upon the acetabulum is diminished. After a while, in successful cases, cicatrization of bone follows, and ossification, with a resulting deformity - the deformity con sisting of a shortened and adducted limb with subluxation, as indicated by the fact that the trochanter is higher than the Nelaton line.About the PublisherForgotten Books publishes hundreds of thousands of rare and classic books. Book excerpt: Excerpt from Rational and Effective Treatment of Hip-Disease, Founded on Experience of Numerous Cases in Hospital Practice During 28 Years As I have found that the principles laid down by the late H.O. Thomas, in his work on Hip, Knee and Ankle, have not been generally known to medical men throughout the world, and as I have proved by successful treatment for many years that the methods of Thomas are the only satisfactory methods for treatment of diseases of the hip-joint, I have decided to bring before the profession the conclusions I have come to from the treatment of numerous cases at the Childrens Hospital, Melbourne, where I have been Honorary Surgeon for twenty-three years, and at the same time to show the improvements introduced by myself, and taught to many of my resident surgeons at this hospital. Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from The Hip and Its Diseases For nearly thirteen years I have resided in the Hospital for the Ruptured and Crippled, all of my time being devoted to daily service in both the in-door and the out-door departments. This hospital is well known for the large number of orthopedic cases that come under observation and treatment. For instance, during my term of service the annual reports show that up to the present time 2048 cases of "hip-disease" alone have been treated, and a very large proportion of this number have been under my own observation. The hospital is further known as an extremely conservative institution. Dr. Jas. Knight, its founder and surgeon in-chief, has been led by his extensive experience to adopt a plan of treatment which coincides, in many respects, with the definition I have elsewhere given of the term expectant. It will therefore be readily seen that the writer of this book has enjoyed unusual facilities for the study of the clinical history of bone and joint diseases. A large number of our cases in the wards are of this nature, and many remain in hospital for two or three years. The record of signs and symptoms as they occur has been made without any bias, and it is seldom that any interpretation even, of these changes appears on the books. My aim, in other words, has been to picture every case from its beginning to its close. Our case books, which now number several volumes, will show how well we have succeeded, and they will show too that the notes have been made by or at the dictation of myself. My observations have not been confined especially to cases under the non-mechanical treatment. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. Book excerpt: Excerpt from On Hip-Joint Disease: With Reference Especially to Treatment by Mechanical Means, for the Relief of Contraction and Deformity of the Affected Limb The recent advances of science, especially in the department known as orthopaedic surgery, have gra dually shown to the world that many deformities of this class admit of alleviation to a much greater extent than in former ages appeared to be practicable, and it therefore appears somewhat singular, and may afford some ground for the accusation of supineness on the part of surgical practitioners, that more numerous and successful attempts have not been made of late years to adopt improved methods of treatment. Book excerpt: Excerpt from Diseases of the Hip, Knee, and Ankle Joints: With Their Deformities, Treated by a New and Efficient Method But that these conclusions will at length be accepted, and the theory will be one day acknow ledged, just as the practice has been already adopted, I have not the slightest doubt. Some have admired the form of my splints and have complimented me as though I were only an ingenious blacksmith, a matter satisfactory, so far as it goes. Such a compliment was not anticipated by me. So very few have understood the principles of the treatment, as I have endeavoured to explain them in previous editions, that I am induced once more to propound and amplify the details of what I hold to be the correct theory, for, assuredly, the surgeon who is guided by a knowledge of the right theory of treatment - though his mechanical aid be limited to a broomstick and a bandage - 2 will be more successful than he who is deficient in this knowledge, though backed by the arsenal of a Weiss or a Charriere. This volume is devoted solely to improving the treatment, and the elucidation of the principles which are supposed to guide it. It will, probably, be my last contribution to this department of surgery, as I hope to see more able workmen enter afield in which there is scope enough for the highest ability. Book excerpt: Excerpt from Some Observations on Hip-Joint Disease and Its Rational Treatment The key of the problem having been found, it is not at all difficult to comprehend its nature, as in all diseases of the joints, we have to contend with a train of symptoms entirely depending upon that mysterious function Of the Spinal cord which we comprehend under the head Of reflex action, and which, in spite of numerous experiments, vivisections, and scientific efforts, is still a deus ea: machina. That epilepsy, catalepsy, eclampsy, St. Vitus's dance, tetanus, trismus, spasms, and scores of other pathological phenomena, are the visible effects of reflex action, is no longer doubtful; besides, we know a vast number of circumstances under which reflex action occurs, and stimuli that produce them; but still our knowledge, as to the histology and the function of the spinal cord, and its ganglionic appendices, is far from perfect and infallible. We may, however, try to point out some analo gies in physiology and pathology, the comparison of which may lead to some conclusions important both to science and practice. Book excerpt: Excerpt from Late Excision of the Hip One may assume that Vincent, of Lyons, ' is a fair exponent of this policy, to which he agrees in his writ ing; and it is interesting to note that in 233 cases of hip disease treated at the Charit Hospital, where this policy was followed, 52 were resected, being at the rate of per cent. Of all cases. At the Children's Hospital, Boston, in cases, 50 were resected, being at the 'rate of per cent. This comparison seems to indicate that French conservatism is about five times as Operative as is American conservatism. Menard probably represents French Opinion in general, in stating that, Resection ought not to be reserved for desperate cases; if one wishes to obtain good results the Operation must be done in time.
