Book excerpt: Excerpt from The Study of the Pulse, Arterial, Venous, and Hepatic: And of the Movements of the Heart In the following pages I give the results of an enquiry into certain features of the circulation which has engaged my attention for the last twenty years. Originally begun in order to ascertain the changes in the maternal circulation during pregnancy, the enquiry has broadened out, so that it embraces all forms of heart failure. For instance, I found pulse irregularity a very frequent symptom in pregnancy. The question was then forced upon me, "What is the clinical significance of pulse irregularity?" I think it will be readily admitted by all who are familiar with books dealing with the pulse that the present-day knowledge is not merely imperfect - it is chaotic. I was consequently led into a very intricate enquiry as to the different forms of pulse irregularity, and in order to appraise its value as a diagnostic sign I had to watch individual patients for many years during a great variety of conditions that entailed great stress upon the heart. The results of my enquiry I have now embodied, and this study has opened up a field for ingenious speculation far beyond what either physiologists or clinicians have imagined. Experimental observations have failed to give anything like the interesting and easily demonstrated results obtained by the simple methods I have employed. Apart from the academic interest that such a study may excite, it is of real service in giving us the true significance of, and bringing us into intimate relationship with, one of the most common and least understood symptoms that daily confronts the medical practitioner. The most obvious unique features include the dual vascular supply; the mechanism of intrinsic regulation of the hepatic artery (the hepatic arterial buffer response); the fact that portal blood flow, supplying two thirds of liver blood flow, is not controlled directly by the liver; the fact that 20% of the cardiac output rushes through the most vascularized organ in the body, driven by a pressure gradient of only a few millimeters of mercury; the extremely distensible capacitance and venous resistance sites; the unidirectional acinar blood flow that regulates parenchymal cell metabolic specialization; and the high concentration of macrophagic (Kupffer) cells filtering the blood. The liver is the only organ reported to have regional blood flow monitored by the autonomic nervous system. This mechanism, when dysfunctional, accounts for the hepatorenal syndrome and offers a mechanistic therapeutic target to treat this syndrome. The trigger for liver regeneration is dependent on hepatic hemodynamics so that chronic liver blood flow regulates liver cell mass. In severe liver disease, the whole body circulation is reorganized, by forming portacaval shunts, to accommodate the increased intrahepatic venous resistance. These shunts protect the venous drainage of the splanchnic organs but lead to loss of major regulatory roles of the liver. The development of knowledge of the hepatic vasculature is presented from a historical perspective with modern concepts summarized based on the perspective of the author's four decades of devotion to this most marvelous of organs. Book excerpt: The sphygmograph was one of the promising instruments of precision that captured the imagination of mid- and late-nineteenth-century physicians eager to plumb the secrets of the circulatory system. Literally a pulse writer, the sphygmograph allowed physicians to study a permanent record (sphygmogram) of the contours and rhythms of the pulse wave. The early masters of the sphygmograph were hopeful that images of the pulse at the wrist could reveal much about the action of the heart and major blood vessels that would prove useful in research and practice. Although the sphygmograph proved to be a frustrating instrument and its pulse recordings confusing, it prepared early twentieth-century physicians to embrace more reliable technologies, such as the sphygmomanometer (blood pressure cuff) and the electrocardiograph. This book traces the European invention, development, and application of the sphygmograph before turning to a detailed study of the novel instruments and clinical investigations of three heretofore unremarked American sphygmograph men and the role of the sphygmograph in American medical practice, most notably in the hands of Dr. Mary Putnam Jacobi. A final chapter examines the pervasive problems of the sphygmograph in the context of recent literature on apparent failures of technology.
