Book excerpt: Excerpt from The Student's Guide to Surgical Diagnosis The habit of note-taking is one which must be adopted early in life, if it is to become easy and serviceable, and the briefest note made at the time of seeing a patient is infinitely more valuable than an elaborate record penned hours or days afterwards. How to observe, is an art to be attained only by practice; but what to observe in any given case can be learnt to a certain extent from books, provided the clue to the nature of the case is given to the student. This, it is the object of the following pages to furnish so far as may be; for it must be remembered that though in surgical descriptions the symptoms are ar ranged according to their order of importance and successively, in the living patient they are more or less commingled, masking one another, and requiring care for their dis entanglement. Book excerpt: Essential Surgery is a comprehensive and highly illustrated textbook for clinical students as well as a practical manual for junior doctors and those preparing for postgraduate qualifications in surgery. The unique feature of the book is its problem-orientated nature as distinct to the traditional disease-based structure. Explains the pathophysiological basis of surgical diseases and of their management to help bridge the gap between the basic medical sciences and clinical problems. Adopts a problem-solving rather than a disease-orientated approach to diagnosis and treatment, reflecting current teaching trends which emphasise the full understanding of how a diagnosis is made and why a particular treatment is used. Includes information about epidemiology, disease prevention and the provision of health care, and tries to relate the community aspects of surgical problems to aetiology, disease prevention and primary care. Contains outlines of common surgical operations, to enable students and junior doctors to explain operations to patients and to allow them to participate intelligently in the operating department, as well as giving them an understanding of how to prevent complications. Includes a major section on accident surgery related to the general surgeon. Now with full text online on Student Consult. Comprehensively updated in line with the evolution and refinement of surgery over the past few years, by an expanded author team and an advisory group of surgical and radiological experts. All line drawings re-presented for greater impact and clarity. All radiological images reviewed and updated. Includes a new chapter on screening. It includes basic introductory material and progresses through each organ system. Within each chapter, there is a brief review of salient normal histology, a discussion of typical specimen types, a strategic approach to the specimen, and a discussion of how the multitude of different diagnoses relate to each other.

Book excerpt: Excerpt from Clinical Surgical Diagnosis for Students and Practitioners When Dieffenbach summarized his views on trephining, in his classical work, some sixty years ago, before the question was definitely settled, he wrote as follows: - "Up to quite recently it was the first urgent duty of the surgeon, immediately after the injury, to make a large crucial incision, and to search for fissures or fractures. Occasionally I found on my arrival that the hairy portion of the head had been ploughed up by incisions, backwards and forwards, crucial and transverse. This was considered to be indispensable in all head injuries. A surgeon who had omitted to make the crucial incision would have incurred the same responsibility as one who failed to open the window in endeavouring to rescue a victim of coal gas suffocation." At that time there was no "cerebral diagnosis"; but nevertheless the surgeon's need for a diagnosis had to find some expression. To leave an injury to Nature, without knowing whether or not a fissure was present, was considered to be an unworthy and culpable piece of surgical inactivity, because the real danger of a fractured skull had not yet been clearly ascertained. Philosophical speculation took the place of observation, and therefore surgery for a long time was guilty of one of its greatest sins, in breaking the law primum nil nocere. We smile at this kind of diagnosis; but posterity will look upon some of our exploratory procedures, involving flaps of half the skull, much in the same way as we regard those meaningless "crucial and transverse incisions." The maxim "nil nocere" must be observed, not only in treatment, but also in examination. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books.
