Book excerpt: Despite the proliferation of research and the application of behavioral interventions, African American/Black men who have sex with men (MSM) continue to bear the greatest burden of reported new cases of sexually transmitted infections (STIs), including HIV. Previous research has identified numerous socially constructed factors that contribute to the syndemics involved that include HIV infection, substance abuse and risk taking behaviors that increase health disparities observed in African American/Black MSM. There is evidence that behaviors are, acquired through family and community interactions, messages about sexual identity, gender expectations and life experiences. Social networks both positive and negative have a significant impact on the individual's self-perception and acceptable behavior. The syndemics theory holds relevance in these studies in that there are multiple factors at work in the lives of Black men who have sex with men that contribute to their health disparities. In this study, 10 New York City African American/Black MSM between the ages of 18 and 30 years old were engaged in individual in-depth interviews. The participants described in detail their social and sexual experiences beginning with the point at which they self-identified as same gender-loving Black males. Data analyses showed the significance of family acceptance, and social support, and seeking validation in syndemic formation in these subjects. The data further demonstrated that disparities and syndemic formation are dependent upon several reinforcing experiences, not on one single incident.

Book excerpt: This study examined the lived experiences of African American men who publicly identified as heterosexual, but privately engaged in intimate relationships with other men. These men are identified by several terminologies including Down Low (DL) and men who have sex with men (MSM). Seven men participated in the study which consisted of three audiotaped phone interviews over the course of three months. One of the participants withdrew from the study before his last interview. The participants identified themselves as being African American, over the age of 19, and having lived, or are currently living, on the DL. The participants were interviewed about their experiences including family of origin beliefs about people who were gay, influences in the African American community that shaped their sexual identity construction, their lives on the DL, mental health issues that they may have experienced, and disclosure and non-disclosure of their sexual identity. Phenomenological research methodswere used to collect and analyze and data along with the theoretical methodological framework of Critical Race Theory (CRT), which was used as a tool to identify how factors of race, gender, and sexuality play roles in the construction of African American DL and MSM. QSR NVIVO qualitative research software was also used to code categories and identify relationships that resulted from coding the transcripts. Themes that resulted from the data analysis included how the African American family and community (including the church) influenced the construction of sexual identity of African American men. Other themes included masculinity, mental health issues and the issue of disclosure and non disclosure of sexual identity among African American DL and MSM. The CRT concept of counterstories allowed the men to discuss pivotal stories that marked a defining moment in their lived experiences. Research is still needed to further explore sexual behavior of African Americans. Counselors and mental health providers are encouraged to educate themselves about the sexual identity construction of African Americans, and how factors in the African American family and community continue to shape the sexual identity of its members.

While many social factors may be associated with the increasing disparity of HIV infection among Black MSM, high levels of religiosity and spirituality prevalent in Black culture may be correlated with high risk behavior among Black MSM and may help explain this population's increasing incidence of HIV infection. Research supports a relationship between religiosity, spirituality, and high risk behavior among populations. Some research has suggested that religiosity and spirituality among Black MSM may be correlated but are multifaceted constructs. Conclusions suggest that among Black MSM these constructs account for certain levels of cognitive dissonance among Black MSM addressing their sexual and religious identity and can lead to negative coping behaviors and negative health outcomes (e.g., higher sexual risk, higher substance abuse, and higher levels of depression) among Black MSM. Findings also suggest positive spiritual and religious experiences may reduce the level of dissonance experienced by some Black MSM and may lead to positive health outcomes. Despite the unique role of religiosity and spirituality among Black MSM, research has not examined the relationships of these constructs and high sexual risk behavior among Black MSM. Future effective HIV prevention interventions intended for Black MSM may well include dissonance reduction, as well as, spirituality and religiosity components to achieve risk reduction interventions that may well decrease the continued HIV incidence among Black MSM.
