Book excerpt: The relationship between substance abuse and dating violence has been well studied over the past few decades, but has not been as extensively researched in the foster care population. Studies that have been conducted with this population have demonstrated an increase in negative outcomes as a result of having experienced foster care. Few studies have examined substance use or dating violence in relation to foster care experiences and the results come from small or non-representative samples. The current study evaluated the role that being in foster care can have in the relationship between substance abuse and dating violence. We predict that individuals that were in foster care, as children, would be more likely to abuse drugs and will experience higher levels of dating violence than individuals who were not in foster care. In addition, the relationship between substance abuse and dating violence will be greater for those in foster compared to those not in the foster care system. Finally, the number of foster care homes a child was in will exacerbate the relationship between substance abuse and dating violence. The results of this study may have implications for foster care programs and may call attention to negative outcomes associated with foster care homes and their aversive consequences.

Book excerpt: Parental substance use is a major risk factor for a child’s entrance into foster care and, in the context of the opioid epidemic, has contributed to an increasing proportion of foster care entrances. However, parental substance use exists on a spectrum of severity and risk to a child’s safety and well-being. Whether or not a child with substance-using parent(s) is removed to foster care and returns to their parents’ home following foster care depends on the extent to which professionals, caseworkers, judges, and other authorities perceive parental substance use as maltreatment and whether it can be reconciled with a child’s safety and wellbeing. Decision-making in the child welfare system largely depends on the personal judgments of caseworkers, judges, and other child welfare workers, which are in turn influenced by external factors varying over time and place. Regarding time, the primary narrative regarding substance abuse has shifted dramatically from one emphasizing criminality and the need for punishment during the 1980s crack cocaine epidemic to one emphasizing public health and the need for treatment during the current opioid crisis. How this new narrative is reflected in child welfare decision-making, however, has not been adequately explored. Regarding place, the policies, availability of services, and more general sociodemographic and health environment of counties and states are known factors in child welfare decision-making, but have yet to be applied specifically to cases involving parental substance use. This dissertation links data from the Adoption and Foster Care Analysis and Reporting System with various county- and state-level data sources to address these gaps in the literature. The first study shows how sociodemographic variation in substance use-associated foster care entry rates over time reveal changes in where and for whom the child welfare system allocates resources. The second study demonstrates how county-level policy, health, and sociodemographic characteristics explain geographic variation in these rates. In the third study, I show how Medicaid expansion and program generosity interact in nuanced ways to predict reunification among children removed from the home due to parental substance use. All three studies illustrate the ongoing connections between the opioid epidemic and child welfare

GAO found that: (1) the 1991 population of young foster children was much larger than the 1986 population and its rate of increase was twice the rate for the general foster care population and over 5 times the rate for the population of all young children; (2) more foster children in 1991 were placed in foster care because of neglect and caretaker absence or incapacity; (3) 78 percent of the foster children in 1991 had at least one drug- or alcohol-abusing parent compared to 52 percent in 1986; (4) 58 percent of foster children in 1991 had health-related problems and 62 percent had a higher risk for future problems because of prenatal drug exposure compared to 43 percent and 29 percent, respectively, in 1986; (5) the number of drug-exposed children could be underreported because of reliance on mothers' self-reporting of drug use and hospitals' failure to perform toxicology tests on all mothers and infants at birth; (6) 17 percent of the foster children in 1986 were exposed to cocaine compared to 55 percent in 1991; (7) more foster children in 1991 were eligible for federal maintenance payments than in 1986; and (8) the increasing number of young foster children and their high percentage of health problems has adversely impacted federal and state funds for foster care, particularly in providing drug treatment services to pregnant women and mothers, and developmental and health therapies for drug-exposed children.
