Book excerpt: The current state of healthcare with its multifaceted demands related to patient care outcomes and the challenges of managing personnel creates a complex work environment for the nurse leader. Striving to meet the competing needs of the organization, employees, patients and families on a daily basis can result in work related traumatic stress and cause burnout. This places an individual at risk for compassion fatigue (CF). The concept of CF has primarily been studied among direct patient care providers and has not been explored among individuals at the leadership level. The goal of this DNP project was to determine if nurses in leadership positions are at risk for or are experiencing CF. For this study, CF was defined as a state in which a nurse leader experiences an inability to maintain a desired level of compassionate energy, due to frequent and regular exposure to prolonged, emotionally and psychologically challenging managerial circumstances in the workplace (adapted from Andreotta, 2013). A mixed methods approach was utilized to gather demographic data and describe the nurse leader’s level of compassion satisfaction (CS), risk for burnout, and secondary traumatic stress as measured by the Professional Quality of Life (ProQOL) Version 5. The use of open ended questions allowed for the identification of factors in the workplace that are modifiable and can be used to develop a CF prevention program for nurse leaders. Thirty-three nurse leaders from two academic medical centers responded to the research survey which revealed an average to high level of CS, coupled with low to average level of burnout and WRTS among the nurse leaders as measured by the ProQOL V5. CS mean score was 54.55 (SD = 3.94), burnout mean score was 43.11 (SD = 4.04), and WRTS mean score was 41.73 (SD = 3.81). The qualitative data provided insight on the source of CF in the work environment which included employee, patient, and organizational stressors. Potential solutions for mitigation and prevention identified by the nurse leaders included personal and organizational solutions such as enhancing coping skills, developing resiliency skills and increasing organizational support. This study identified the presence of risk factors for the development of CF fatigue among nurse leaders and elements in the work environment that can be modified to prevent this condition. These finding may be used to create a satisfactory Professional Quality of Life for the nurse leader which impacts their role effectiveness and supports the delivery of quality patient care.

Book excerpt: The purpose of this study was to explore existing literature on the effects of Compassion Fatigue (CF) and Burnout (BO) on the nurse and social work profession and identify effective interventions. Via a review of literature, 25 studies revealed that the lack of awareness of compassion fatigue, working with trauma survivors, constant exposure to people suffering, high caseloads, difficult work schedule, lack of social support and poor self-care were contributing factors to the development of both CF and BO in the nursing and social work practices. This literature review also revealed themes associating self-care practice, such as addressing one's psychological, physical, emotional and spiritual needs and compared its effectiveness in reducing symptoms of compassion fatigue and burnout to the Accelerated Recovery Program. Results of this study revealed that the Accelerated Recovery Program and certain self-care practices were found to be effective in reducing one's symptoms of compassion fatigue (CF) and burnout (BO). Possible implications of this research should concentrate on enhancing the quality of data collection through specific samples to identify the effectiveness of distinct self-care practices addressing CF and BO. Those in the healthcare industry may draw upon the knowledge presented to gain empathy of these factors that increase the risk of CF and BO to implement effective interventions to treat nurses and social workers, thus, improving their quality of care to their patients.

Book excerpt: Background: Compassion fatigue (CF) is the cumulative physical, emotional and psychological effect of exposure to traumatic scenarios or events when working in a helping profession. Critical care nurses are at high risk for burnout and stress as evidenced by call-in days, use of employee assistance programs, turnover rates. Compassion fatigue has been shown to lead to negative outcomes such as 1) burnout, 2) turnover, 3) substance use or abuse, 4) compulsivity, and 5) dissatisfaction. Evidence-Bases Practice Framework: Jean Watson's Philosophy and Science of Caring Model and her Caritas Processes served as the basis and model of the unit's transformation towards staff's professional satisfaction. The ACE Star Model of Knowledge Transformation operates as the guidance to the restorative effect to achieve with the project to combat CF. Permissions were granted to use these models. Methods: The purpose of this project was to create a departmental evironment of nurturing and caring practices leading to improved role satisfaction for nurses, improved quality of patient care, and enhancement of the department's overall nursing practice environment. The Professional Quality of Life, version 5 (ProQOL5) questionnaire was used to identify the presence of CF among nurses in a surgical intensive care unit. Participants were asked to attend two informatl educational, debriefing sessions as a focus group and complete the questionnaire after attendance to reassess if the education on CF was effective in combatting CF and increasing compassion satisfaction. Permission was granted to use the PRoQOL5. Findings/Results: To test whether there was statistical significance between pre- and post- educational sessions, a one-tailed paired t test, alpha equal to 0.05 as criterion for sigificance was used for statistical analysis. Statistical significance was demonstrated between the test scores for compassion satisfaction and burnout. There was no statistical significance demonstrated between the test scores for secondary traumatic stress. Conclusion: Health care providers may not recognize CF as well as perceived, implying the need for education directed toward the recognition and management of CF. Educational and meaningful recognition help combat CF by offering a solution to lessen the damaging effect, all the while increasing professional satisfaction. Sacco, Ciurzynski, Harvey, and Ingersoll claimed that professional satisfaction, confidence and empowerment reflect in enhanced patient care and improved healthcare outcomes.
