Book excerpt: Recently, Ware and Sherbourne published a new short-form health survey, the MOS 36-Item Short-Form Health Survey (SF-36), consisting of 36 items included in long-form measures developed for the Medical Outcomes Study. The SF-36 taps eight health concepts: physical functioning, bodily pain, role limitations due to physical health problems, role limitations due to personal or emotional problems, general mental health, social functioning, energy/fatigue, and general health perceptions. It also includes a single item that provides an indication of perceived change in health. The SF-36 items and scoring rules are distributed by MOS Trust, Inc. Strict adherence to item wording and scoring recommendations is required in order to use the SF-36 trademark. The RAND 36-Item Health Survey 1.0 (distributed by RAND) includes the same items as those in the SF-36, but the recommended scoring algorithm is somewhat different from that of the SF-36. Scoring differences are discussed here and new T-scores are presented for the eight multi- item scales and two factor analytically- derived physical and mental health composite scores.

Book excerpt: The objectives of this research were to: (1) translate the RAND 36-Item Health Survey 1.0 (aka SF-36) into Arabic; (2) evaluate the reliability and equivalence of the Arabic and English versions in a sample of Saudi Arabian citizens; and (3) assess the health status of a sample of Saudi Arabian citizens using both the Arabic and English versions. Forward and backward translation of the SF-36 with committee review was performed. Both the Arabic and English versions of the survey were administered to a convenience sample of bilingual (English and Arabic) Saudi citizens (N=415) at Saudi ARAMCO Company, Dhahran, Saudi Arabia. Internal consistency, equivalent-forms, and test-retest reliability were estimated for the Arabic and English versions of the survey. The results of the study provide support for the reliability and equivalence of both versions. Additional studies are needed with a representative sample of the general Saudi population to further assess the psychometric properties of the Arabic version.

Book excerpt: Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.
