Improvements in technology and advancements in medicine have enabled more surgeries to become ?ambulatory? or ?outpatient? surgical procedures. Ambulatory surgical procedures are defined as those in which a patient is discharged from the facility the same day as the procedure. The advancements in healthcare have also led to healthcare workings spending less time with patients. The decrease in the time spent with a patient during the perioperative experience can result in communication problems or feelings of unpreparedness regarding the surgical procedure. Effective communication is essential for a satisfactory surgical experience. The purpose of this prospective descriptive study is to examine the concerns of patients undergoing elective surgical procedures, how the concerns are being addressed, and the extent to which the concerns are relieved. Materials and Methods: One hundred sixty seven participants that were undergoing elective outpatient surgical procedures between March 2014 and June 2014 at Butterworth Hospital, Blodgett Hospital and Lake Drive Surgical Center were enrolled in this study. Participants were called prior to their surgical procedure and within seven days following their surgical procedure. During the phone calls, participants were asked to give demographic data as well as their ?Most Important Concern? (MIC) prior to the surgical procedure, their MIC on the day of surgery, and how well how healthcare workers addressed and alleviated those concerns. The Pre-Procedure Planning (PPP) Research team at Spectrum Health developed a survey instrument titled ?Most Important Concerns Questionnaire?. The survey questions included 5two that used a ?yes?/ ?no?/ ?prefer not to answer? response format, two that had an open-ended question format, and two that were scored using a Likert-Style 4-point scale. Results: Of the one hundred sixty-seven participants enrolled in the study, some reported more than one MIC; there were a total of 209 concerns reported. The five most common pre-procedure MIC’s consisted of the following in order of highest to lowest frequency: successful outcome of surgery (18.18%), anesthesia concerns or concerns about waking up during surgery (16.75%), fear of complications (13.40%), return to work or previous level of activity (12.82%), and free of pain (12.44%). Of the 167 participants, 154 participants reported MICs in the top five categories. Of the day of surgery concerns reported, the most frequent MIC was concern about nausea or vomiting (23.53%). This was followed by fear of complications and anesthesia concerns or concerns about waking up during surgery, in which each were reported by a total of three participants (17.65%). The majority of participants (67.7%) felt that their pre-procedure MIC was adequately addressed and 100% of participants felt that the day of surgery MIC was adequately addressed. Data was analyzed using statistical analysis software (SAS) version 9.4 with de-identified data on an encrypted flash-drive. Gender, race and age did not have a significant impact on the pre-procedure MIC categories. Chi-Square Test, Fisher’s Exact Test, and Monte Carlo Estimations were used to check for statistical significance of patient race, age, and gender. Conclusion: This study found that the majority of the five most common pre-procedure MICs were addressed and relieved by the surgical team. Gender, race, and age were not significant variables in the addressing of MICs.

Book excerpt: Problem: At a Northern California hospital (NCH), there is a current effort to ensure the perioperative patients are satisfied with their care. These patients receive the Outpatient and Ambulatory Surgery Consumer Assessment of Healthcare Providers and Systems (OAS CAHPS) survey after surgery. The scores regarding discharge process and patient knowledge over the last five quarters have fluctuated between the 10th percentile and the 36th percentile. NCH relies on patient satisfaction scores to better serve their members. Context: NCH understands the higher the scores, the more members they will retain and the higher the overall income. Based on the low OAS CAHPS scores, an evidenced-based educational program to increase these scores was devised. This quality improvement program uses an educational program to teach the nurses in the post-anesthesia care unit (PACU) to use a perioperative experience booklet to educate total joint home recovery patients when they arrive at the hospital. Intervention: The first interventions for this project are to use an educational program that teaches the PACU RNs how to place a 24-hour post-operative call. The second intervention is to teach PACU RN's how to use a perioperative experience booklet to educate patients during the preoperative phase of their care. Measures: The goal of this quality improvement project is to increase the OAS CAHPS scores by the third phase of the project, which will start in December 2020. This quality improvement project will cover the first phase of the project. By educating the RNs to properly use the perioperative experiences booklet and place the 24-hour calls, patients should have an increase in their perceived care experience, which will impact the OAS CAHPS scores. By implementing this education with the nurses in the post-anesthesia unit, the leader hopes to see an increase in confidence in the RNs' ability to perform these interventions. A survey was given to the RNs before and after the training. The survey was used to measure the difference in their confidence to perform this intervention. The results between the pre- and post-education surveys showed a marked difference in nurse confidence in delivering the post-operative call. The comments section displayed an increase in knowledge and understanding. RNs who took the post-education survey stated that they felt more comfortable in their abilities to implement the quality improvement. Results: The RNs completed a survey before and after the training. The survey measured the difference in their confidence to perform this intervention. In the pre-education survey, 64% of the RNs felt they had little to no confidence in being able to teach total joint home recovery patients before surgery or place a 24-hour post-operative call. In the post-education survey, 57% of the nurses stated that they felt confident if using the perioperative experience booklet, and 100% of the nurses stated that they felt confident placing a 24-hour post-operative call. The results between the pre- and post-education surveys showed a marked difference in the nurse's confidence in delivering the post-operative call. The comments displayed an understanding of the goal of using the perioperative experience booklet and how to make the 24-hour post-operative call. Conclusion: The education session showed it to be a positive tool to use to increase the nurse's knowledge of the interventions. Adequate nurse preparation to implement this intervention will increase the chance that this intervention achieves the goal of increasing the OAS CAHPS scores.
