Book excerpt: External fixation is now being used widely to maintain fractures, osteo tomies, and arthrodeses in a desired position during consolidation. Whereas external fixation has been readily accepted in European countries, its use has weathered a rather stormy course in North America, especially in the treatment of fractures. Only recently has external fixation found its rightful place on this continent as well. Many different models are on the market today, and the practitioner is faced with a difficult decision in selecting a model. Should he buy a system where the fracture has to be reduced first, or should he work with a device permitting a reduction after insertion of the pins? To enable surgeons to study the different systems, to discuss their advantages and disadvantages, and to permit them to put their hands on these devices and inspect them personally, the Division of Orthopedic Surgery, University of Ottawa organized an applied basic science course in May 1981, External Fixation of Fractures. During this course, all major systems were pre sented to the participants. As happened during the course "Internal Fixation of Fractures" held two years ago, the rigidity of internal fixation was frequently and intensively debated. Whereas the rigidity of internal fixation cannot be altered during the course of healing, the rigidity of external fixation can be changed. In fact, with progression of union, rods of increasing elasticity can be used.

Available in PDF, EPUB and Kindle. Book excerpt: The continuity of a bone may break in response to a trauma, injury or stress. This condition is called a bone fracture. In orthopedic medicine, bone fractures are classified on the basis of the mechanism by which the fracture has been incurred, soft-tissue involvement, fracture pattern, anatomical location, etc. Some of its various types are skull fracture, spinal fracture, complete fracture, spiral fracture, impacted fracture, pathologic fracture, periprosthetic fracture, etc. A bone fracture is assessed on the basis of a physical examination and the medical history. Radiographic imaging of the bone and nearby joints helps to determine dislocations and fracture-dislocations if any. In certain cases, MRI and CT scans may also be required. Bone fractures can be treated surgically or non-surgically. Non-surgical strategies comprise of immobilization, pain management and stabilization. In cases where absence of operative interventions may result in prolonged immobilization and complications such as deep vein thrombosis, pulmonary embolism, pressure sores and chest infections, surgery is a recommended strategy. From theories to research to practical applications, case studies related to all contemporary topics of relevance in the diagnosis and management of bone fractures have been included in this book. It presents researches and studies performed by experts across the globe. Doctors and students actively engaged in orthopedics will find this book full of crucial and unexplored concepts.

The book shows how to treat each fracture--from both an orthopaedic and a rehabilitation standpoint--at each stage of healing. Each chapter on an individual fracture is organized by weekly postfracture time zones. For each time zone, the text discusses bone healing, physical examination, dangers, x-rays, weight bearing, range of motion, strength, functional activities, and gait/ambulation. Specific treatment strategies and rehabilitation protocols are then presented. More than 500 illustrations complement the text.

Book excerpt: Following a presentation of basic principles of fracture fixation and biomechanical principles the book turns to practical problemsolving areas, namely, the hand, wrist, elbow, shoulder, hip, knee, ankle and foot, and spine, and special problems in children. Internal fixation, including intramedullary nailing and the use of external fixation are described. Compound fractures are everyday problems, and the principles of their management and practical solutions deserve separate discussion. The value of microsurgical techniques has been underestimated by orthopaedic experts and the indications, choices, and procedures need to be put into proper perspectives.

Book excerpt: The purpose of this book is to report on my 17 years of experience in the development and implementation of a closed functional method of treatment of certain fractures of long bones of the appendicular skeleton. My personal experiences, clinical results, and the basic concepts underlying the treatment philosophy are reported. Illustrations demonstrating the techniques of application of functional casts and braces are presented in detail as well as a step-by-step description of the management regime and a discussion of the behavior of fractures under this system. Indications and contraindications for the use of this method of treatment are clearly outlined. I consider it important to compile this information for I believe that the system has withstood the test of time and is sufficiently sound to be incorporated in the armamentarium of the orthopaedic surgeon. Also, it may have a favorable socioeconomic impact since it offers a viable alternative in the treatment of frac tures.
