Book excerpt: Excerpt from The Saint Louis Medical and Surgical Journal, Vol. 38: Semi-Monthly; April 5, 1880 The state of the urinary passages, so far as relates to size and irritability, is an important element in the choice between lith otomy and lithotrity. In order that any patient may avail him self ofa capital operation, he must be free from certain organic affections, notably cancer, aneurism, bright's disease and strue tural derangements of the heart and lungs. Granting that he is free from such disorders, the size of the stone and its hardness or softness must be determined. Ifa stone is neither very hard nor very large, or what amounts to the same thing as being very large, the presence of multiple calculi, and the patient's uri nary passages are of such a size as to admit a lithotrite with ease, and not so irritable, but that the crushing instrument may remain Within the vesical cavity for a few moments at a time, the case is one susceptible of relief by lithotrity, it matters not the age of the patient nor the composition of the stone. In the. Majority of cases, patients suffering from stone are free from all other affections; the urinary passages are sufficiently large and there is nothing in the character of the calculus to prevent the use of the lithotrite except the presence of such an extreme degree of irritability, that they cannot bear the slightest in struniental interference. In such patients, lithotomy is often resorted to, when byjust rights the patient is one who should have been relieved by lithotrity. Since morbid irritability of the urinary passages is the only impediment in the path ofthe lithotritist in these cases, and especially since I have recently been compelled to contend with this obstacle in a number of instances, I desire to call the attention of the members ot'this Association to the measures which have rewarded my labors with success. I do this that undue pressure of the catheter against the tube may be avoided, and to pre= vent the mucous membrane from closing the orifice of the catheter. I direct him to move it slightly fiom place to place, in order to find a position which allows the air to enter most freely into his middle ear, of which he is by far the best judge. At the first few treatments, the patient may not hold the instrument in the right position, perhaps, but it is very much better to fail in inflating the ear at the first trial than to bruise the mouth of the Eustachian tube once. Yet in most instances the tympanum may be inflated at the first sitting. There, knowledge counts for much, and rightly, but knowledge is not, nor can it ever be, the agent which alone is capable of combating certain symptoms which are the concomitants Of nearly all the graver symptoms so beloved of all painstaking practitioners. What the im portance Of the so-called lesser symptoms is, need not be contended here: but that they have eluded the supposedly firm grasp of the doctor of today is made manifest to all of us by the success of the multiplicity Of religious healing societies. And they have eluded him because of his unwillingness to reckon them in the same category with all those salient points of disease inculcated into him in his college curriculum, or by the various text-books that have broadened his mind when he was a full fledged doctor. Whether or not a vagary that becomes a fixed idea should be met with sympathy or derision, depends on the mental bias. Of the at tending doctor, and has so many ramifications as an argumentative proposition that only a very brave man would attempt to define his rea sons for assuming a staunch attitude in the matter. But even his strongest contention for his attitude towards peculiar mental manifesta tions, will not make the kindliest critic think better of him, unless the convincing fact Obtains that by sheer force of his individuality there was a complete cessation of all untoward mental symptoms.
