Book excerpt: Excerpt from The Milwaukee Medical Journal, 1896, Vol. 4 When the cystic duct, from any cause, becomes completely obstructed, the outflow of bile, as well as that of the normal secretions of the gall bladder, is prevented, and distension naturally follows, for it must be remembered that stoppage of the duct does not cause a cessation of the physiologic secretions of the bladder itself. The effects of this form of obstruction are purely lo cal, as the obstruction in no way interferes with the free flow of bile from the liver to the intestine, through the hepatic and common ducts. There is no jaundice and the effects are limited to the disturbances excited by the inflammatory changes taking place in and around the gall-bladder and to the changes in the bladder contents. The condition, however, is one that is very far from being free from danger, as these changes may seriously endanger life and are usually very detti mental to comfort and health. Book excerpt: Excerpt from Detroit Medical Journal, Vol. 4: January, 1905 Unless necessity demands, the general practitioner should not attempt special surgery; he cannot get the best results; the best interests of the patient will not be conserved and as a result the reputa tion of the physician will suffer, the inter ests of both being identical. He should, however, endeavor to make an accurate diagnosis and will be expected by the friends to give a prognosis. For this reason he should make effort to know the character of the work done by different specialists. Not all of those who claim special aptitude possess it. Observation will show that some who talk aseptic principles do not observe them. Specialists will also show different grades of workmanship. It is a question of high art, the products of every one will differ in actual value from those of every other one, and in relation to so sacred an object as the conservation of life and the restoration of health it behooves the medical adviser to know He assumes a great responsibility in recommending an operator and should be paid for this knowledge by the patient, who reaps the benefit. In making the selection he should be guided by the answer to the question: To whom would you refer your wife or mother or sister under similar condi tions.
