Book excerpt: At the end of 2020, The Global Health Observatory estimated that globally there are 37.7 million people living with HIV (WHO, 2022). In Ireland, in 2021 over 6,000 people were estimated to be living with HIV, however this figure has already increased in 2022 with 293 notified cases of HIV in the first six months of 2022, compared to 145 for the same period of 2021 (Health Protection Surveillance Centre 2022). The HPSC 2021 statistics reveal that in 2% of HIV cases, intravenous drug use behaviour was the source of transmission. Research studies and health strategies emphasize the importance of adherence to HAART (Highly Active Antiretroviral Therapy) to avoid viral replication and personal/public health consequences. However, it is evident that addiction and homelessness can contribute to non-adherence to medical and pharmacological treatment for a range of diseases. Explanations have been put forward in the research including, side effects, stigma/shame, lack of structure/routine, cognitive impairment caused by drug use, lack of motivation/self-worth, lack of facilities to store and take medication. Adherence to HIV treatment for people who are homeless and in addiction, is further complicated by the required drug regimen which can be complex and difficult to tolerate. This study explores the lived experience of adhering to HIV medication in the context of drug addiction and homelessness, using a qualitative research approach, involving interviews with six people experiencing homelessness and addiction and who are prescribed HIV medication. Individual participant narratives are gathered and analyzed using Riesman's (2008) narrative analysis framework which enables a deep exploration of the stories people tell about their lived experiences. The study findings illuminate how HIV, addiction and homelessness impact on adherence to HAART particularly in relation to diagnosis experiences, stigma and shame as well as the challenges of managing treatment in the context of homelessness and addiction. It is anticipated that the findings, discussion and potential recommendations from the study will contribute to improved understanding of the needs of this population and more sensitive person centred approaches.

Book excerpt: Polypharmacy, defined as the concurrent use of multiple medications simultaneously, is increasingly common in people living with HIV/AIDS (PLWHA) due to the use of antiretroviral and non-antiretroviral drugs for the treatment of multiple chronic diseases. Polypharmacy is a growing concern among PLWHA because of their complex medication regimen, the risk of adverse drug events, drug-drug interactions, medication non-adherence, medication errors, and antiretroviral treatment failure. Medication therapy management (MTM), which are pharmacist-led interventions, have been useful in resolving medication-related problems and optimizing clinical outcomes. However, there are limited studies on the effectiveness of pharmacist-provided MTM services in reducing polypharmacy in HIV/AIDs patients. MTM services should enable the identification and reduction of polypharmacy. Hence, the central goal of this dissertation was to evaluate the impact of MTM services on polypharmacy in PLWHA. A secondary data analysis of a new MTM project by the CDC, UNTHSC, and Walgreens that involved the collaboration of pharmacists and clinicians to provide patient-centered care for HIV patients was done. The study involved 765 participants from 10 states in the United States. Polypharmacy was measured by the number of polyactive substances (pharmacologically active ingredients) in medications used. A paired T-test was used to find the difference between the pill count and the polyactive substances in medicines used by PLWHA. A longitudinal data analysis using a generalized estimating equation was used to assess the impact of MTM intervention on polypharmacy over time by determining the change in polyactive substances in medication pre-post MTM intervention. The relationship between the changes in HIV outcomes CD4 count and HIV RNA count and the change in polyactive substances pre-post MTM intervention was also determined. The results showed a significant average difference between the polyactive substance count and the pill count of about 2.15. Also, the number of polyactive substances in medications used by study participants reduced by an average of 3 from pre- to post- MTM intervention. There was a relationship between the change in HIV outcomes and the change in polyactive substance pre-post MTM intervention. As polyactive substances decrease over time in the study, there is a higher chance that there is viral suppression and improvement in CD4 count at the end of the study. Medication therapy management involving pharmacists and clinicians may be useful in addressing polypharmacy in PLWHA. The MTM program in this study was not designed to address polypharmacy. But the results indicated that the intervention had a positive impact on polypharmacy. Further studies, such as a case-control study or a randomized control trial, are required to assess the effect of MTM on polypharmacy better.

Book excerpt: Since the development of antiretroviral medications in the mid-1990s, treatment of human immunodeficiency virus (HIV) infection has improved steadily and HIV is now considered a manageable chronic condition with improved quality of life for those who are medically adherent. However, medication regimens typically require near perfect adherence to be effective and people living with HIV/AIDS (PLWHA) may experience a unique set of biopsychosocial barriers that limit or inhibit their ability to be medically adherent. The purpose of this study was to examine the influence of biopsychosocial factors and motivational interviewing on antiretroviral medication adherence in people living with HIV/AIDS (PLWHA). Specifically, this study explored how biopsychosocial barriers (i.e., physical health, mental health, substance use, housing, transportation, and employment concerns) and motivational interviewing predict antiretroviral medication adherence in PLWHA as measured by patients' CD4 count and HIV viral load. This study utilized archival data that was collected between July 2013 and July 2016 at the Brody School of Medicine HIV Clinic and captured patients' biopsychosocial barriers, CD4 count and HIV viral load. A stratified sample of 210 patients was drawn from the archival data for this study. Multiple logistic regression analysis showed that patients' CD4 count cut scores at intake made a significant contribution to the prediction of medication adherence as measured by PLWHA CD4 count cut scores at week 21, when controlling for patients' CD4 count cut scores at intake. Comparatively, results showed a statistical significance for the barrier substance use when examining antiretroviral medication adherence in PLWHA as measured by patients' HIV viral load cut scores at week 21. Further, a non-significant association between patients' CD4 cut scores at week 21 and patients' HIV viral load cut scores at week 21 in predicting antiretroviral medication adherence was found. The results of this study suggest that patients' starting CD4 count and substance use are important components that warrant further research for PLWHA. Further, this study has implications for healthcare professionals, counselors, and counselor educators related to substance use concerns for this specific population. Future research exploring substance use treatment and ARV medication adherence in PLWHA is suggested.

Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.
