Although weight-bearing exercise among previously sedentary adult females can increase bone mineral density (BMD), cross-sectional studies suggest that high-intensity exercise among amenorrheic athletes may be detrimental to bone health. The long-term effects of athletic participation on BMD should be examined. Objectives. This study was designed to: (1) Determine the prevalence of varsity athletic participation among female university graduates who participated in a mail survey; and, (2) Compare BMDs of high school varsity female athletes with women who were sedentary during the same time period, while controlling for covariates. Methods. A survey was mailed to 7291 alumnae of 2 Southern California universities in the classes of 1987-1991. The survey was returned by 1900 (26.1 %), and 1373 (72.3%) consented to further contact. Of the latter, 73 athletes (70.6% of those eligible) and 67 controls (53.1 % of those eligible), all 30-35 years of age, in a clinic evaluation, provided their health histories, anthropometric measurements, physical activity and dietary assessments, and dual energy x-ray absorptiometer measurements of the hip and spine. Book excerpt: Sports participation is associated with adaptations in bone strength that reflects positively on bone mineral density (BMD). Although this participation has been positively associated with increased BMD, there is substantial heterogeneity in the osteogenic response from the various types of physical loading (e.g., gravitational loading, muscle loading) that are within sports. Therefore, the purpose of the current investigation is to determine the most influential predictive factors of BMD using anthropometric and performance-based assessments, as well as to compare BMD values between sports containing components of gravitational loading (GL) and muscle loading (ML) in Division-I female collegiate athletes. The results of this study suggest that the influence of various anthropometric and performance-based factors on BMD in female collegiate athletes, depends on the type mechanical loading that is performed during sport participation. While weight was reported to be the most influential factor for BMD in GL sports, extracellular fluid was the most influential factor for ML sports.

Peak bone development is thought to be achieved in the young adult years of an individual's life and can be indicative of risk of osteoporosis later in life. This study aims to explore the relationship between bone mineral density (BMD) and body composition measurements from a college aged non-student athlete population (n = 61) to that of a student-athlete population (n = 39). Furthermore, sex specific differences between and within the subgroups were evaluated. No statistical difference was found for BMD, weight, lean mass, and bone mineral content between subgroups. However, sex-specific differences were present for all variables of interest. Pearson's correlation coefficient results indicate that there was a moderate to strong relationship between BMD and lean mass for all groups.

Book excerpt: Discordance in bone mass between young adult swimmers and soccer players may be a direct result of differences in bone loading patterns that influence bone mineralization during growth. Our aim was to evaluate whether sports participation (soccer and swimming) had an independent effect on bone mass accrual at the hip and lumbar spine in adolescent female athletes. We recruited boys and girls 10 to 14-years of age from Corvallis, Albany, Sweet Home, Salem, Eugene, and the greater Portland area. Bone mineral content (BMC, g) and bone mineral density (BMD, g/cm2) of the proximal left hip, spine, and whole body were assessed by dual energy x-ray absorptiometry (Hologic QDR 4500A; Hologic Inc., Waltham, MA, USA). We used ANCOVA and report that baseline BMC and BMD values of girl soccer players at the greater trochanter were significantly higher compared to controls and the swim group, and femoral neck BMC was significantly greater than the swimmers. At baseline, all boy groups were similar at the hip and spine. After 12-months, ANCOVA was also used to assess absolute change for BMC and BMD at the hip and spine. The girl soccer players had significantly more BMC and BMD at the greater trochanter as well as total hip BMD and lumbar spine BMC compared to the swimmers, but not the controls. The girl control group showed a significantly greater 12-month change for femoral neck and greater trochanter BMC than swimmers. Overall, the girl swimmers demonstrated a lower accumulation of bone mass during the 12-month study period. As for the boys, soccer players had a significantly higher 12-month change for femoral neck BMC than swimmers, but were similar at the spine. There were no differences between the boy control subjects and the swimmers for 12-month change values at the hip and spine. While preliminary and limited by the small sample size, our results indicate that after controlling for growth, soccer players gained significantly more BMC at the femoral neck than swimmers. Furthermore, exposing the young skeleton to impact loading exercise has site-specific benefits at the hip whereas prolonged training in a non-weight bearing environment may compromise skeletal acquisition.

Book excerpt: Problem: Previous research on vitamin D has focused on its role in bone density as well as its autoiznmune and anti-inflammatory benefits. Emerging evidence also supports its role in preventing stress fractures, which are common overuse injuries among populations such as military recruits and female runners. These injuries can lead to major training setbacks and may have long-term implications for bone mineral density. To understand the effect of vitamin D on stress fractures in female runners, a scholarly literature review was conducted. Methods: A scholarly literature review was coordinated utilizing PubMed. The following search keywords were used to access all of the available literature that encompassed the topic: "vitamin D" AND "stress fracture", "vitamin D" AND "stress fracture" AND "female", "stress fracture" AND "female athlete", "vitamin D" AND "stress fracture" AND "runner". Results: Of the available articles, a total of 21 were found to be relevant to the topic. Twelve of the 21 were original research articles, 3 were case reports and 6 were review articles. Additionally, 1 web site was utilized. Overall, the strongest research is among male and female military recruits. There is limited but promising data on vitamin D and its effect on athletes. Conclusions: Much of the original research has focused on military recruits entering basic training, where evidence supports vitamin D's role in bone health and stress fracture prevention. This correlation is also supported in athletic populations, such as female runners, although less research is available to support this conclusion. Overall, future original research on athletic populations is needed to establish a clear cause and effect relationship.

Book excerpt: "Introduction: Young male jockeys compromise bone health by engaging in caloric restriction and high volumes of physical activity during periods of musculoskeletal growth and development. Failure to attain peak bone mass (PBM) during growth can have adverse short and long term musculoskeletal effects, with numerous studies demonstrating inferior bone health in jockey populations. However, no study to date has addressed counteracting the deleterious effects that participation in this sport has on bone health. The purpose of this six-month double-blind randomised placebo controlled trial was to examine the efficacy of 800mg calcium and 400 IU vitamin D daily supplementation on improving bone mineral properties at the tibia (weight-bearing) and radius (non-weight-bearing) using peripheral Quantitative Computed Tomography (pQCT) and blood-borne markers of bone turnover.Three inter-related studies were designed following the findings of a systematic review and meta-analysis examining the effects of calcium and vitamin D supplementation on bone mineral density in healthy males. Study one examined the effect that the protocol had on markers of bone metabolism and bone properties of the radius. Study two was designed to investigate the impact of six months supplementation on weight-bearing bone while study three further explored alterations to radial and polar cortical bone properties at the tibial mid-shaft following the clinical trial.Methods: Twenty-nine male jockeys (age=20.18 ± 3.23yrs) were originally recruited to the study with 17 completing the intervention. Bone properties at the ultra-distal (4%) and proximal (66%) radius and tibia using pQCT and serum vitamin D, Procollagen type 1 N propeptide (P1NP) and C-terminal telopeptide of type I collagen (CTx) were assessed at baseline and six months. Bone properties at the 66% tibial site were further analysed using BoneJ pQCT distribution plug-in. Polar and radial volumetric bone mineral density (vBMD) was measured in 36, ten degree cortical sectors (polar) and three concentric cortical divisions (radial). Polar distribution was further consolidated into four, 90 degree quadrants aligned to anatomical planes. Cortical mineral mass, endocortical and pericortical radii were also analysed.Results: After co-varying for height, body mass and baseline bone measurements, the analysis of co-variance (ANCOVA) results of these combined studies demonstrate that six months calcium and vitamin D supplementation stimulated a reduction in bone resorption together with significant improvements to bone material properties at the proximal tibia in the supplemented group. ANCOVA serum analysis indicated significantly higher vitamin D levels (18.1%, p=0.014) and lower CTx (ng/L) (-24.8%, p=0.011) in the supplemented group with P1NP unchanged.
