Book excerpt: Alcohol is the sixth leading risk factor for disability and premature death all over the world, and one of the leading causes of premature mortality in western societies; it is a leading risk factor for death in young and middle-age males. Heavy drinking accounts for about two thirds of the burden of disease attributable to alcohol. In the early 1980s, screening and brief interventions (SBI) in primary health care settings were proposed as effective strategies to identify risky drinkers and to help them reduce their drinking. Since then, a growing body of evidence, including several meta-analysis and Cochrane reviews, has shown the efficacy and effectiveness of SBI in primary health settings. However, demonstrating the effectiveness of SBI has not been insufficient to facilitate its general implementation in the routines of primary health care physicians, and in fact the dissemination of SBI has proven to be a difficult business. Qualitative and quantitative research has identified most of the facilitators and barriers for its implementation, and publicly funded research has been earmarked to address the dissemination problems worldwide. Some examples are the World Health Organization Phase III and Phase IV studies on the identification and management of alcohol-related problems in primary care, EU funded projects (PHEPA, AMPHORA, ODHIN, BISTAIRS), the UK SIPS trials and the SBIRT developments sponsored by the Substance Abuse & Mental Health Services Administration (SAMHSA) in the USA. The efficacy and effectiveness of SBI in primary health is now well established, but there are still some questions that remain unsolved: which practitioners should deliver them; what length should they be; is there a need for booster sessions; is there added value of a motivational approach? These questions, together with other relevant aspects of SBI, need ongoing research. In recent years, SBIs have been tested in settings other than primary health care, including hospitals, accident and emergency rooms, criminal justice, colleges and universities, social services and pharmacies. In some of those areas, the evidence is scarce (for example, pharmacies) while in others it is very promising (for example, students and hospitals). New technologies have also offered the possibility of online tools, and, in the last few years, different digital-based applications have been tested successfully as new ways to deliver effective SBIs to larger amounts of people. Brief interventions have also spread to drugs other than alcohol. This book aims to be an update of the state-of-the art of brief advice. It is a compilation of articles published by some of the most relevant researchers in the field in Frontiers in Psychiatry between 2014 and 2016.

Book excerpt: Brief Motivational Interventions (BMIs) have been identified as one of the most effective individually focused alcohol intervention strategies for college students in the United States. Despite the central role of motivation for change in BMIs, whether BMIs increase motivation for change has rarely been investigated. The current study conducted a meta-analysis of individual participant data (IPD; 15 studies, N = 5,903) from Project INTEGRATE (Mun et al., 2015) to examine whether BMIs increase motivation for change in drinking. Different measures and responses used in the original trials were harmonized across studies, and effect size estimates were derived from a model that adjusted for baseline motivation and demographic variables for each study (step 1) and subsequently combined in a random-effects meta-analysis model (step 2). After adjustment for baseline levels of motivation level and demographic variables, the intervention effects of BMIs on motivation for change was not statistically significant (standard mean difference [SMD]: 0.026, 95% CI: [-0.001, 0.053], p = .06, k = 19). Subsequent metaregression analyses among BMI subtypes indicated that the intervention effect did not differ between individually delivered motivational interviewing with personalized feedback (MI+PF), stand-alone personalized feedback (PF), and group-based motivational interviewing (GMI). Among all BMI subtypes, only GMI had a statistically significant intervention effect on motivation compared to controls (SMD: 0.055, 95% CI: [0.007, 0.103], p = .025, k = 5). Within the first three months post-intervention, there was a decrease in SMD of 0.05 (95% CI: [0.01, 0.08]) in motivation per month. However, no statistically significant difference in the intervention effects was found between studies with short-term vs. long-term follow-up. Although the results from the current study do not support the hypothesis that BMIs improve motivation for change, the evidence as a whole suggests ways in which motivation may be improved following intervention and can be tested in future studies.

Book excerpt: Alcohol abuse among college students continues to be a significant problem by which the consequences impact the student, their peers, and the university. Although quantitative research with volunteer participants supports the use of enhanced brief motivational interventions and cognitive behavioral skills training in reducing risky drinking behavior (binge drinking), research with mandated students has shown inconsistent findings. The current study is a phenomenological qualitative study exploring the students' perspectives after attending a mandated college-level alcohol intervention program. Mandated students are students who have been referred to an alcohol intervention as a result of violating an alcohol related policy on campus. Protective behavioral strategies were used by the mandated students and perceived as helpful in reducing alcohol consumption and minimizing alcohol related consequences. Mandated students expressed more satisfaction with meeting face-to-face in the SPARK motivational intervention as compared to computer-based intervention programs. Factors that facilitate motivation to change are a relatively untapped field of exploration. There is a relationship between the satisfaction with the facilitator and satisfaction with the alcohol intervention. As a result, the SPARK participants reported reducing their risky drinking behavior. Mandated students should attend a face-to-face motivational intervention consisting of motivational interviewing and cognitive behavioral skills in an effort to influence a motivation to change. Nine themes emerged from the data. The findings build on previous research by providing a more nuanced understanding of mandated students' experience through a college-level alcohol intervention program called SPARK.
