This is a completely revised and updated sixth edition of the highly respected standard for stroke diagnosis and treatment. Dr James Toole has stayed on as a consultant for the text, and Drs E. Steve Roach, Kerstin Bettermann, and Jose Biller have reworked Dr Toole's book to include chapters on genetics, pregnancy-related stroke, and acute treatments. The practical focus of the book has not changed, retaining its emphasis on bedside diagnosis and treatment. Easily accessible both for stroke specialists and residents, the sixth edition has been modernized to keep pace with the rapid expansion of knowledge in stroke care and includes evidence-based recommendations, the latest technology and imaging, and risk factors. The text is supplemented with more than 200 images, many in color.

Book excerpt: The projects in this thesis addressed the knowledge gap regarding optimal timing of aneurysm treatment following subarachnoid hemorrhage (SAH) by evaluating whether earlier aneurysm treatment improves clinical outcomes. This was first examined in a meta-analysis synthesizing evidence from observational studies on timing of endovascular coiling of ruptured aneurysms. The results informed a cohort study using Ontario registry and administrative data evaluating timing of surgical or endovascular aneurysm treatment in SAH, where early treatment within one calendar date of SAH was compared to later treatment. The use of administrative data for evaluation of outcomes in SAH was bolstered by external validation of the Nationwide Inpatient Sample Subarachnoid Hemorrhage Severity Score (NIS-SSS), as well as a modified version of the NIS-SSS (m-NIS-SSS). The meta-analysis of observational studies demonstrated a possible but inconsistent benefit of treatment within 1 day of SAH. Treatment.

Book excerpt: Evidence-based medicine is a concept that has come to the fore in the past few years. Clinicians are increasingly encouraged to practise patient management based on available evidence in the scientific literature. For example, new pharmacological therapies are only used when large randomized trials have 'proven' that a particular drug is better than existing ones. This is also the case in surgical specialties, although surgery has traditionally seen a lack of useof this information, with individual surgeon's preferences being most influential in treatment choices. However, more recently, there has been a large expansion of trials and studies aimed at providingsurgeons with information to guide their choices using firm evidence. This new edition has been revised and expanded to include new data where relevant, and also features a new chapter on pituitary surgery. Landmark Papers in Neurosurgery 2e remains a key collection of the most important trials and studies in neurosurgery, allowing the reader to rapidly extract key results, and making it essential reading for all neurosurgeons and trainees in the field.

Book excerpt: During recent years, major advances in surgical techniques, diagnostic methods, anesthesia and adjunctive treatment in the care of patients with subarachnoid hemorrhage have been achieved. Nevertheless, the overall outcome of patients with SAH cannot be regarded as satisfactory. The first part of the book concentrates on the pathogenetic mechanisms underlying vasospasm and edema, the principal causes of poor outcome. Recent progress in the field of membrane lipid metabolism has allowed the problem to be approached from an entirely new perspective. The possible participation of free radicals, membrane lipids and eicosanoids is thoroughly discussed. The second part features the problems of practical management of SAH patients. Timing and indication of surgery of aneurysms and the surgical techniques are described in detail.

They emphasize the need for clinical reasoning, integrative thinking, problem-solving, communication, teamwork and self-directed learning – all desirable generic skills for health care professionals. This book is a teaching tool that bridges the gap between textbook information and everyday experience of clinicians 'in the trenches'. Leading practitioners bring a practical approach to these complex conditions, highlighting specific areas of diagnostic uncertainty in evaluation and treatment. Each case is taken from real-world clinical practice and reviews the diagnostic and treatment process in a systematic manner, identifying common challenges and potential pitfalls. This concise and useful guide in the Common Pitfalls series provides a step-by-step guide for everyday clinical practice, invaluable to anyone dealing with cerebrovascular disease on a front-line basis. The intended readership is trainees and non-specialist practitioners in neurology, stroke medicine, and neurosurgery.

Available in PDF, EPUB and Kindle. Book excerpt: A practical textbook, based on a problem-oriented workflow, that will improve patients' likelihood of full recovery from stroke and prevent future strokes from occurring Stroke is the leading cause of adult disability and is in the top five causes of death globally. Warlow's Stroke: Practical Management, 4th Edition takes a problem-oriented approach and addresses the questions posed by a stroke patient in the order they are likely to present in clinical practice, for instance, 'Is it a stroke?', 'What sort of stroke?', 'What caused it?', and 'What can be done about it?'. Beginning with chapters phrased as questions, the book walks the reader through a standard clinical workflow, exploring the practical skills and assessment required at each stage of patient management. Early chapters cover: locating the vascular lesion, identifying the involved arterial territory, the role imaging should play, and the application thereof. Subsequent chapters look at what causes a transient or persistent ischemic event, an intracerebral hemorrhage and a subarachnoid hemorrhage. Unusual causes of ischemic stroke and transient ischemic attack are also covered. The book then presents a practical approach to the management of stroke and transient ischemic attack; offers specific treatments for acute ischemic stroke and aneurysmal subarachnoid hemorrhage; provides ways for professionals to prevent first or recurrent stroke; and more. Final chapters of the book discuss rehabilitation after stroke, how patients and carers can be supported in the short term and long term, prevention of recurrent stroke, and the organization of stroke services. Warlow's Stroke: Practical Management, 4th Edition Follows clinical workflow for stroke analysis Features evidence-based approach throughout Offers practical application aimed at improving patient outcomes Written and edited by internationally renowned experts in the field An essential resource for all practitioners involved in the care of patients who suffer from cerebrovascular disease, but particularly suitable for neurologists, residents, geriatricians, stroke physicians, radiologists and primary care physicians.

The first section is titled “Resuscitation” and exposes the reader to a condensed version of generic topics in primary intensive care medicine. The sections that follow have been categorized according to medical and surgical subspecialties and cover the most germane of problems encountered in a tertiary surgical intensive care unit. Sections of certain chapters, while repetitive, have been left intact in an attempt to maintain the authors' messages and provide the reader with some contradictory but referenced views. The technical chapters describe a very introductory approach to various exercises such as airway management and vascular cannulation.
