Hospitals are recognized as organizations in which waste, unnecessary administrative burdens, failures of care coordination, failures in execution of care processes, and even fraud and abuse are frequently identified as causes. Adoption of management control as a response to hospital problems is consistent with the conviction that control is a critical management function that has the greatest impact on organizational performance. Research proves that the lack of adequate control, adapted to modern organizational solutions, causes many harmful consequences, such as faulty services, dissatisfied patients and employees, inability to effectively compete on market, low flexibility and innovativeness, and, consequently, poor performance of the organization. This book comprehensively presents issues related to management control and develops a breakthrough theory about management control in hospitals. It is the result of many years of research and outlines the concept of control and related theories, which are discussed in detail, taking into account the unique characteristics of medical services, the health-care market, and hospitals as public organizations. Research has shown that the main elements of management control in hospitals are information systems, diagnostic control, interactive control, innovativeness, manager’s trust in physicians, and perceived uncertainty. And that proper relationships between these elements positively influence the hospital’s performance. This book describes how the success of the entire control process is based on the hospital’s top management and its interaction with clinical managers, department heads, and directors of other medical departments as well as clinicians. Given the rapid growth of health care expenditures in the developed countries, the implementation of management control systems in public hospitals has been seen as an effective way to restrain medical costs. While previous studies show the administrative control has little effects on physician efficiency in providing medical service, our findings indicate the critical role of management control systems in improving quality and efficiency of medical care. This study applies the concept of enabling (Adler and Borys, 1996) to our case. In our case, an enabling use of management control systems helped medical doctors to understand the financial impacts of their daily clinical decisions and to provide more productive service without sacrificing service quality. Our results offer new insights into management control in the process of management reform in professional bureaucracies.

Book excerpt: This study examines the association between the use of Management Control Systems (MCSs) and the effectiveness of Performance Management Systems (PMSs) in hospitals. Specifically, the study utilizes Simons' levers of control framework to examine the impact of the enabling use of controls (interactive and belief levers) and the constraining use of controls (diagnostic and boundary levers) on the achievement of PMS process outcomes and hospital performance. The findings indicate that the achievement of process outcomes (performance related and staff related) is affected by the enabling use of controls, with the achievement of staff related outcomes mediating the association between the enabling use of controls and hospital performance (medical facilities and effectiveness). In addition, both the enabling and constraining use of controls exhibited a direct impact on hospital performance. The study contributes to the management accounting literature by providing an insight into the role of MCSs and the importance of process outcomes in enhancing the effectiveness of PMSs in hospitals.

Two years into the pandemic, most countries report sustained disruptions across service delivery platforms and health areas with a profound impact on health outcomes. The impact of these disruptions is magnified within marginalized communities and in countries experiencing protracted conflict. There is an urgent need to focus on recovery through investment in the essential public health functions (EPHFs) and the foundations of health systems with a focus on primary health care, and whole-of-government and -society engagement. The aim of this Research Topic is to gather, transfer and promote operationalization of key experiences from COVID-19 to inform global and country level recovery that better promote health; guide policy direction towards building health systems resilience; and thereby ensure economic and social prosperity. Experience with COVID-19 has demonstrated that traditional approaches to health system strengthening have failed to achieve the complementary goals of Universal Health Coverage (UHC) and health security with the divide between the most vulnerable and well-off only widening. Much of what had been learned from previous experiences such as Ebola in West Africa has not been widely applied. This has left health and economic systems vulnerable to 21 st century public health challenges, ranging from conflict and natural disasters to aging demographics and rising rates of non-communicable and communicable diseases and antimicrobial resistance. These challenges require intentional focus and investment as well as whole-of-government and -society engagement with health to build health system resilience. Greater action is needed to prevent the devastating effects of war and conflict on the health of the most vulnerable. This Research Topic will convene the knowledge and practices of leaders in public health, health systems, and humanitarian and development sectors. This is to ensure lessons from COVID-19 inform the recovery agenda and promote sustainable health and socioeconomic recovery for all. Lest we forget and find ourselves again unprepared and vulnerable in the face of an even greater threat.
