The survey was conducted Aug. 10 to Sept. 1, 2017. Over 3,560 adults ages 21-64 who had health insurance through an employer, purchased directly from a carrier, or purchased through a government exchange participated in the survey. This Issue Brief focuses on whether and how attitudes and behaviors change with the amount of time an individual has been enrolled in their health plan. Overall satisfaction does not increase when participants are in health plans for long periods of time (e.g., 10 years), regardless of whether the plan is a traditional health plan, consumer-driven health plan (CDHP), or high deductible health plan (HDHP). Likewise, consumer engagement does not increase with time enrolled in a health plan. However, there is some evidence that health savings account (HSA)-eligible health plan enrollees are more engaged in some aspects of their HSA the longer they have been enrolled in their HSA-eligible health plan.

Book excerpt: This paper explores differences in health-plan choice using data from the 2011 EBRI/MGA Consumer Engagement in Health Care Survey as well as earlier versions of the survey. It examines the likelihood of having a choice of plans and how that is changing over time; the main reasons for choosing a plan; and how demographics, health status and health behaviors vary by plan type among those with a choice of health plans. Satisfaction with various aspects of health care is also examined by plan type among individuals with a choice of health plans. Most Americans get their health insurance coverage from employment-based plans, yet most employers do not offer a choice of health plans. In 2011, 84 percent of employers offering health benefits offered only one health plan; 15 percent offered two choices; and 1 percent offered three or more choices. Large firms were more likely to offer a choice of health plans than small firms; 42 percent of large firms offered two or more choices, compared with 15 percent of smaller firms. As a result, nearly one-half (47 percent) of covered workers had a choice of health plans, and according to the 2011 EBRI/MGA Consumer Engagement in Health Care Survey, 59 percent of adults 21-64 with employment-based health coverage had a choice of health plans. Consumer-driven health plan (CDHP) enrollees are more likely than individuals with traditional coverage to have a choice of health plans, and the availability of health plan choice is trending upward for CDHP enrollees, although not for those enrolled in traditional plans. Among individuals with a choice of health plans, CDHP enrollees tend to have higher incomes and higher education than individuals with traditional coverage; they are also less likely to be obese; and they are less likely to be satisfied with many aspects of their health plan. Among individuals with a choice of health plans, those opting for CDHPs are more likely to cite cost factors, but also more likely to express dissatisfaction with the cost of prescription drugs and out-of-pocket costs for other health care. Additionally, they were less likely to be satisfied with the program overall. It examines the likelihood of having a choice of plan by various demographics and work-related variables. It also examines choice by health status and health behaviors. The relationship between satisfaction with health insurance and health care and health plan choice is then explored, as is the role of type of health plan. Most employers do not offer a choice of health plan. In 2009, 86 percent of employers offering health benefits offered only one health plan; 13 percent offered two choices; and 1 percent offered three or more choices. Large firms are more likely to offer health insurance and to offer a choice of health plan than small firms. Forty-five percent of large firms offered two or more choices, whereas 13 percent of small firms did so. As a result, about one-half of covered workers had a choice of health plan, and according to the 2009 EBRI/MGA Consumer Engagement in Health Care Survey, 59 percent of adults ages 21-64 with employment-based health coverage had a choice of health plan. While the percentage of individuals with traditional employment-based health benefits who have a choice of health plan was in large part unchanged since 2005 (ranging from 54 percent to 62 percent), the percentage of individuals with a consumer-driven health plan (CDHP) and a choice of health plan rose steadily between 2005 and 2009, increasing from 47 percent to 70 percent. Increasing choice of health plan is a key component of health reform advocates. The health insurance exchange is built on Alain Enthoven's model of managed competition, which entails sponsors acting on behalf of groups of individuals to negotiate with insurers and offer participants a menu of choices among different plans. To obtain the benefits of competition requires that insurance policies be easily comparable to facilitate consumer choice, and quality measures be developed that consumers can use to make informed decisions. This study is based on an online survey of 4,498 privately insured adults ages 21-64 and was designed to provide nationally representative data regarding the growth of account-based health plans and high-deductible health plans (HDHPs), and the impact of these plans and consumer engagement more generally on the behavior and attitudes of adults with private health insurance coverage. Findings from this survey are compared with findings from the 2005, 2006, and 2007 EBRI/Commonwealth Fund Consumerism in Health Care Survey, and the 2008-2011 EBRI/MGA Consumer Engagement in Health Care Survey. The 2012 EBRI/MGA Consumer Engagement in Health Care Survey finds continued slow growth in consumer-driven health plans: 10 percent of the population was enrolled in a CDHP, up from 7 percent in 2011. Enrollment in HDHPs remained at 16 percent. Overall, 18.6 million adults ages 21-64 with private insurance, representing 15.4 percent of that market, were either in a CDHP or were in an HDHP that was eligible for an HSA but the enrollee had not opened the account. When their children were counted, about 25 million individuals with private insurance, representing about 14.6 percent of the market, were either in a CDHP or an HSA-eligible plan. The 2012 EBRI/MGA Consumer Engagement in Health Care Survey continues to find that CDHP enrollees are somewhat more cost conscious in their decision making than those in traditional plans. While CDHP enrollees, HDHP enrollees, and traditional-plan enrollees were about equally likely to report that they made use of quality information provided by their health plan, CDHP enrollees were more likely to use cost information and to try to find information about their doctors' costs and quality from sources other than the health plan. CDHP enrollees were more likely than traditional-plan enrollees to take advantage of various wellness programs, such as health-risk assessments, health-promotion programs, and biometric screenings. In addition, financial incentives mattered more to CDHP enrollees than to traditional-plan enrollees. It is not clear from the data whether the differences in consumer engagement can be attributed to plan design differences or whether various plan designs attract certain kinds of individuals. Regardless, it is clear that the underlying characteristics of the populations enrolled in these plans are different: Adults in a CDHP were significantly more likely to report being in excellent or very good health. Adults in a CDHP and those in an HDHP were significantly less likely to smoke than were adults in a traditional plan, and they were significantly more likely to exercise. People in a CDHP were also less likely to be obese compared with adults enrolled in a traditional health plan. CDHP and HDHP enrollees were also more likely than traditional-plan enrollees to be highly educated. As the CDHP and HDHP markets continue to expand and more enrollees are enrolled for longer periods of time, the sustained impact that these plans are having on cost, quality, and access to health care services can be better understood. The eight years of consumer engagement surveys reported here provide unique data from which to measure future changes in this evolving type of health insurance.

Differences in out-of-pocket costs may have explained some of the differences in overall satisfaction rates. In 2014, 48 percent of traditional-plan participants were extremely or very satisfied with out-of-pocket costs (for health care services other than prescription drugs), while 19 percent of HDHP enrollees and 26 percent of CDHP participants were extremely or very satisfied. Satisfaction with out-of-pocket health care costs has been trending upward among CDHP enrollees. Policy forum participants heard two very different presentations on the prospects for consumer-directed plans. One speaker was optimistic, saying consumer-directed plans have worked because individuals in these plans have substituted less expensive care for more expensive care in order to minimize their out-of-pocket costs. Another speaker was skeptical, saying consumerism will have a "marginal impact" but will not solve the problem of rising health care costs. Two speakers discussed value-based insurance design in which employers attempt to tailor their health plans to balance the demonstrated value of a service against its cost. Value-based design encourages consumers to use health services when the clinical benefits exceed the cost and at the same time discourages the use of services when the benefits do not justify the cost. Value-based design seeks to influence consumer behavior by linking co-payments to the use of a clinically demonstrated benefit, while consumer-driven health plans use tax-sheltered accounts for much the same purpose. Value-based design is still relatively limited in use, shows some cost-saving potential, but questions about the concept remain, the speakers said.

Book excerpt: Objective: To investigate the effects of High Deductible Health Plans paired with a Health Savings Account on the healthcare expenditure and health services utilization. Methods: Data from the 2014 and 2015 Medical Expenditure Panel Survey was analyzed. Subjects enrolled in a Consumer Directed Health Plan (CDHP) were compared with subjects enrolled in non-CDHP after matching. A Chi-square analysis was conducted to assess frequency distribution of subjects in the treatment group as compared to the control. The control group was weighted using propensity score weights to match treatment group. The difference in the healthcare expenditures between the two groups was estimated using a Wilcoxon sign rank test and independent sample t-test. The effect of CDHP enrollment on health services utilization was estimated using a regression model. Results: The total sample population was 2132. The CDHP group was found to have a higher proportion of White, Non-Hispanics with high level of education and high family income as compared to the control group. A significant difference was found between the out-of-pocket costs (OOP) (p=0.0004) and the overall healthcare expenditure (p=0.0119) with the CDHP group having higher costs. There was no significant association between low or high use of health services. Conclusion: The findings suggest that CDHPs might not be fulfilling their required goal of reducing healthcare expenditure as well use of unnecessary care.

The 2014 EBRI/Greenwald & Associates Consumer Engagement in Health Care Survey finds that 15 percent of the privately insured population was enrolled in a consumer-driven health plan (CDHP); 11 percent was enrolled in a high-deductible health plan (HDHP); and 74 percent was enrolled in more traditional coverage. Overall, 26 million individuals with private insurance were enrolled in a CDHP--a health plan associated with a health savings account (HSA) or health reimbursement arrangement (HRA), or an HSA-eligible health plan. This study finds evidence that adults in a CDHP and those in an HDHP were more likely than those in a traditional plan to exhibit a number of cost-conscious behaviors. Specifically, those in a CDHP were more likely than those with traditional coverage to say that they had checked whether the plan would cover care; asked for a generic drug instead of a brand name; talked to their doctors about prescription options and costs; checked the price of a service before getting care; asked a doctor to recommend less costly prescriptions; talked to their doctors about other treatment options and costs; developed a budget to manage health care expenses; and used an online cost-tracking tool provided by the health plan. There is also some evidence that adults in a CDHP were more likely than those in a traditional plan to be engaged in their choice of health plan. Specifically, those in a CDHP were more likely than those with traditional coverage to say that they had attended a meeting where health plan choices were explained; consulted with their employer's human resources (HR) staff about health plan choices; and were more likely to have consulted with an insurance broker to understand plan choices. The survey also finds that CDHP enrollees were more likely than traditional-plan enrollees to take advantage of various wellness programs, such as health-risk assessments, health-promotion programs, as well as biometric screenings. In addition, financial incentives mattered more to CDHP enrollees than to traditional-plan enrollees.
