Due to its beneficial effect on people self-development, there is a growing interest to apply coaching method in personal health development through health coaching. Health coaching as a part of diabetes self-management support (DSMS) can be applied as a supplementary support to diabetes self-management education (DSME). Despite the limited studies in Asian countries, studies in health coaching for diabetes has shown that health coaching has an emerging role in empowering patients to undertake self-management, facilitate healthy behaviour, and improve glycemic control. Health coaching can be done by a certified health coach (nurses, pharmacist, paramedics, educator, peers, and family). Coach will support patient to discover and develop their capability to take responsibility for their actions such as making decision, solving problems, and taking care of themselves. Aims: This study aims to evaluate the impact of health coaching as a supplementary diabetes education for T2DM patientu2019s life style. Method: This was an open label randomized control trial study, conducted from January to August 2018 in Dr Cipto Mangunkusumo General Hospital and Fatmawati General Hospital, Jakarta, Indonesia. Sixty patients were recruited through consecutive sampling and randomized into control and intervention groups. Both groups received standard medication and 12 sessions of diabetes education. The intervention group received additional 12 coaching sessions using CARE Model (Clarity of the goal, Awakening of the u2018ahau2019 moment or meaningful awareness, Resolution of the decision and plan, and Empowerment of the coacheeu2019s or patientu2019s accountability and commitment) by two certified coaches. All tests were taken three times at baseline, 3-month, and 6-month after intervention. Quality of life were measured through EQ-5D questionnaire. Physical activities classified as low, moderate, and high using GPAQ questionnaire (General Physical Activities Score). Result and discussion: will be available for the congress.

Health coaching is one of the form of DSMS that assist patient with T2DM to perform and maintain diabetes self-management. AIMSThis study aims to evaluate the impact of health coaching on the glycemic control and medication adherence of T2DM patients. METHODWe conducted a randomized controlled trial involving 60 uncontrolled T2DM patients (HbA1c > 7.5%) from two tertiary care hospital in Jakarta, Indonesia (Dr. Cipto Mangunkusumo National Referral Hospital and Fatmawati General Hospital). This study was carried out from January until August 2018. All recruited subjects (n=60) were randomized into two groups, which were intervention and control group. All subjects received standard T2DM treatment and 12 standardized diabetes education sessions every 2 weeks, while subjects in the intervention group received additional 12 individualized health coaching sessions every 2 weeks. HbA1c and medication adherence were measured at baseline, 3-month and 6-month after intervention. Medication adherence was measured using Morisky Medication Adherence Scale-8. MMAS-8 scores can range from 0 to 8 points, score 0 indicated high adherence, score 1-2 indicated medium adherence and score >2 indicated low adherence. RESULTS We observed a reduced HbA1c levels in both groups at 3 and 6 month compared to baseline. Whereas, no significant differences between both groups at 3-month after intervention were observed [-1.71% vs -1.12%, for intervention and group respectively, p value = 0.25], at 6-month, we observed a more significant reduction of HbA1c levels in the intervention group [-1.8% vs -0.26%, p value = 0.003]. We also tried to find out any differences according to patientsu2019 goal for behavioral change which could be the clue to success of change. MethodWe runned six-month health coaching program from 2013 to 2018 with 144 diabetes patients in 38 public health center branches. The health coaching program was made up of monthly face to face counseling and weekly telephone counseling. At enrollment and after 6 months, we performed a questionnaire survey and examined knowledge, disease management, self-efficacy, subjective health status, lifestyle(smoking, drinking, physical activity) and blood glucose. We analyzed pre- & post-test.ResultsWe analyzed 6-month follow-up data from 144 patients. They showed significant improvements in diabetes knowledge, disease management of foot care, self-efficacy, the amount of exercise, and systolic blood pressure. For diet, only patients with diet goal showed significant increase in u201cDays following healthful eating plan for diabetes.u201d For lifestyle change, both of patients with diet goal and physical activity goal showed improvement in practice of exercise, but only patients with physical activity goal showed significant improvements in frequency, amount, and monthly amount of exercise. Patients with physical activity goal also showed improved amount of smoking, frequency of drinking, monthly amount of drinking. They also showed better fasting blood glucose and post prandial blood glucose. Both of patients with diet goal and physical activity goal showed significant improvements in systolic blood pressure.DiscussionAlthough this study didnu2019t show statistically significant HbA1c improvement in T2DM patients with diet goal nor physical activity goal, it showed some improvements in fasting blood glucose and post prandial blood glucose. All patients showed improvements in knowledge, foot care, self-efficacy, subjective health and exercise. But only patients with diet goal showed improvement in diet as disease management. Patients with diet goal also showed improvement in practice of exercise but only patients with physical activity goal showed significant change in real effects like frequency and amount of exercise. In this study, goal setting seemed like to pull out behavioral change among patients. We thought health coaching program could be helpful in empowering T2DM patients in rural area with branch of community health center.

Diabetes self-management education has been involved in the prevention of diabetes-related complications among type 2 diabetes mellitus (T2DM) patients. Emphasis was on healthy food choices, increase in physical activities, and medication adherence. Purpose: The purpose of the evidence-based practice (EBP) project was to evaluate the effect of education on T2DM inmates 18 years and older on lifestyle changes such as weight loss. Methods: This was an EBP project conducted on 20 participants with T2DM. Project implementation lasted a total of 10 weeks. Interventions were individualized education sessions focusing on various aspects of diabetes control. Pre- and post-test utilized diabetes knowledge test 2 (DKT2) for interventions. Results: The mean percentage score on the pre-test was 63.65% with a SD of 8.35 compared to post-test score of 89.47% and SD of 3.64. There was a 25.82% increase in participants' knowledge post-intervention. The secondary outcome measured weight changes among the participants. Mean of pre-intervention weight was 234.31 pounds, median of 225 pounds, mode of 214 pounds, and a SD of 40.38. Post-intervention weight had a mean of 231.37 pounds, median of 219 pounds, mode of 256 pounds, and SD of 40.24. The difference between pre-and post-intervention weights was 1.25%. Implications for practice: Nurse practitioners play major roles in leading the interprofessional team and in providing health promotion policies and principles to various groups of patients. Successful planning, implementation, and sustaining continuity of change requires multidisciplinary involvement. The sessions were based on culturally sensitive physical activity and gender-specific exercise programs for at least 30 to 60 minutes most days of the week as established at the base of the Arab Food Dome and the World Health Organization-Eastern Mediterranean Region (WHO-EMRO) user-friendly guide. The educational sessions included the Holy Qur'an acceptable alternatives to fasting during Ramadan to prevent acute T2DM complication. Results: The group mean weights was reduced by 3.4%, the diabetes knowledge test (DKT) group mean score increased by 30% and the diabetes self-management questionnaire (DSMQ) group mean scores increased by 27% indicating that the project was effective. Conclusion: Educational interventions are effective at improving the health outcomes of patients with T2DM. Book excerpt: Background: Although the adoption of self-management behaviors is crucial for maintaining good health after a type 2 diabetes diagnosis, many individuals with T2DM fail to meet target blood glucose levels. Adherence to gluco-regulating behaviors like regular exercise and balanced diet can be challenging, especially for individuals of lower socioeconomic status (SES). Providing cost effective interventions that improve adherence to self-management behaviours is important for improving quality of life for patients and the sustainability of health care systems. Objective: To design and test a health coaching protocol administered by trained health coaches in a lower SES community aimed at improving the health profile of patients with poorly controlled T2DM, with and without smartphone connectivity. Methods: Dissertation methodology is described in two studies. The first study describes the pilot trial run at the Black Creek Community Health Centre (BCCHC) between February 2010 and March 2011which recruited a total of n=21 participants intervened with by n=1 health coach. The second study describes the randomized controlled trial conducted primarily at BCCHC from March 2012 to March 2014 and intervened with n=131 participants with n=6 health coaches. The primary outcome is change in glycated hemoglobin (HbA1c) from baseline to 6-month follow-up for each study. Secondary outcomes include changes in weight, waist circumference, and BMI, as well as within group changes of HbA1c. Psychometric measures collected pre/post for the RCT include the Satisfaction with Life Scale, Positive and Negative Affect Schedule, Hospital Anxiety and Depression Scale, and the 12-item Short Form Health Survey (SF-12v2). Hypothesis: Patients who receive health coaching with electronic support will exhibit greater reductions in HbA1c than the health coach only group. There will also be greater improvements in anthropometric and psychometric outcomes favouring the group who receives electronic support. Results: In the pilot study, a total of 21 individuals consented to participate, of whom 19 (90.4%) completed the 6 month trial; 12 had baseline glycated hemoglobin (HbA1c) levels >7.0% and these participants demonstrated a mean reduction of 0.43 (0.63) (p

The review of literatures shows the increased concern about the development of type 2 diabetes related complications. The use of diabetes self-management education as revealed by studies helps in a tremendous way in the self-management of type 2 diabetes, thereby helping to reduce the incidence of the complications that occur from poor management of T2DM. The complications when they occur, leads to increased frequency in hospital admission, long hospital stay, high cost of treatment, increased morbidity and mortality rates. Diabetes education has been an ongoing process in the management of diabetes, but the cases of increased hospitalization and development of complications still exist. The aim here is to create more awareness through education for self-management of the condition by the patients with T2DM. The education should be of self-management approach, and not the traditional instructional approach that leaves the patient dependent on the clinicians only for the management of the ill-health. The various literatures reviewed support the use of DSME to achieve this purpose of engaging, empowering, and encouraging the T2DM patients to take control of their diabetes care. The nurses as well as other clinicians can make this happen by reinforcing what is taught and encouraging their patients to adopt the self-management approach. The implication to nursing practice is in the need for nurses to help patients with T2DM to embrace this self-management education approach, and physician/other clinicians should endeavor to refer their patients with T2DM to diabetes classes.

Book excerpt: Type 2 Diabetes Mellitus (T2DM) currently affects approximately 1 in 9 individuals in the United States. Access to education, transportation to appointments, and other social determinants of health can reflect poor self-management, adverse outcomes, and increased morbidity and mortality. The purpose of this quality improvement (QI) project was to evaluate the effectiveness of implementing a series of expert-led, educational classes with a toolkit for patients diagnosed with prediabetes or T2DM. The Health Belief Model was the framework used to develop and implement educational intervention through a 4-month period. This project was conducted at two sites in Northeast Kansas: one which provides primary care services to an underserved population, while the other is a satellite clinic located within a housing authority. The PDSA cycle was used to develop and evaluate program implementation throughout the project. A toolkit was developed and provided to individuals who attended the sessions. Educational sessions focused on T2DM overview, risk factors, comorbidities, medication, diet, exercise, stress, and self-management. Expert specialists were recruited for two of the educational sessions. A diabetes self-management questionnaire (DSMQ) score and glycated hemoglobin (A1c) value were obtained prior to the first educational session. A final DSMQ score and A1c value were obtained at the conclusion of the last educational session. Qualitative measurement revealed improved perceived self-management of T2DM as well as an overall improvement of A1c control after a 4-month period.

Book excerpt: This report is a systematic review and critical appraisal of the literature on the use of formal outpatient diabetes education as a therapeutic tool for self-management in the adult patients with type 2 diabetes. Formal patient diabetes education to promote self-management (PDE) at different levels of complexity and comprehensiveness has emerged from instructions by physicians, nurses, and dieticians in physicianþs offices and hospitals. Since 1990 there has been a shift from traditional approaches of PDE to focus on patient-centered perspectives, self-efficacy, self-management and empowerment issuses. Quantitatives research on the use of formal PDE as a therapeutic tool promote self-management in adults with type 2 diabetes has produced mixed results in terms of improved metabolic control and reduced risks for cardiovascular disease. The long-term diabetes control outcomes of formal PDE when used to promote self-management in adults with type 2 diabetes have yet to be established. Further investigations are needed to determine which types of programs or what components are most effective in improving the ability of adults with type 2 diabetes to self-manage their disease; or which category of patients might benefit most. Findnings from qualitative research studies helped to better understand the context in which formal PDE should be applied to be successful in promoting self-managemenet behaviors in adults with type 2 diabetes.
