Book excerpt: Gait parameters and synchronization capabilities were examined following a one-day experimental implementation of Rhythmic Auditory Stimulation (RAS) for adults with severe traumatic brain injury (TBI). Eight individuals (N=8) who had experienced severe TBI participated in this study. Participants ambulated four 10-yard trials across a plastic runner along a flat surface with paint applied to their shoes (via the footprint analysis gait assessment). Parameters including cadence, velocity, stride, step lengths, and symmetry were calculated through footprint and videotapes analysis. All participants ambulated under the following conditions: baseline with no music, RAS at resonant frequency, RAS with a 10% frequency modulation increase, and removal of RAS. Quantitative data were reported through raw scores as well as percentage change between trials. Results of quantitative data and gait analysis revealed that at least 7 out of 8 participants showed capacity to synchronize to an external rhythmic auditory cue for portions of a trial, respond to the frequency modulation, and/or reproduce a given frequency cue once rhythm was removed. Gait kinematics appeared optimal for most participants within the resonant frequency pass (trial 2), especially including decreases in asymmetry.

As clinical understanding of human locomotion has evolved, there has been a shift in recent years from more traditional gait rehabilitation methods towards the inclusion of sensory stimuli such as rhythmic auditory stimulation (RAS) and visual feedback (VFB). There is however limited knowledge about the comparative effects of stimulation to the visual and auditory cortex during gait rehabilitation training. Purpose: The aim of this study was to compare the effects of visual feedback and rhythmic auditory stimulation on walking performance in individuals with hemiparetic gait. Design: A total of 5 participants (age 61± 6.928) with hemiparesis completed 5 weeks of treadmill walking incorporating visual feedback and 5 weeks of treadmill walking incorporating rhythmic auditory stimulation. Participants first completed 5 weeks of VFB training using the BIODEX Gait Trainer II® treadmill followed by a 5 week detraining period and then a final 5 weeks of treadmill walking with RAS via digital metronome. Gait analysis was performed using the VICON 3D motion analysis system. Participants had their gait analyzed using the VICON plug-ingait model before and after each 5 week training intervention. Gait outcomes included stride length, step length, walking speed, cadence and lower extremity joint excursion at the hip, knee and ankle. Book excerpt: The following study investigates the impact of a rhythmic cue on the observational gait parameters of a population of adults with blindness or severe visual impairment. Forty-six adults who had sight loss significant enough to require the use of a long cane for mobility purposes participated in the study. Participants were between the ages of 18 - 70 years. The study design was a within-subjects, repeated measures design with two levels for the independent variable of the metronome (uncued versus cued) and two levels for the independent variable of tempo (normal walk versus fast walk). Dependent variables of cadence (steps per minute), velocity (meters per minute), and stride length (cadence [divided by] (velocity / 2)) were recorded. Within-subjects repeated measures statistical analyses identified a main effect for the independent variable of the metronome; subsequent analysis revealed that the metronome had a significant effect on the dependent variable of cadence. The presence of a rhythmic cue seemed to improve observational gait parameters for many of the study participants. A more in-depth investigation reveals the complex interrelationship of gait parameters, as well as the need to differentiate between the clinical importance of the study and the need for additional basic scientific research. While compelling clinical inferences can be drawn from this study, there continues to be a need to establish rhythmic auditory-motor entrainment as a sound theoretical framework upon which further research and clinical protocol development for this sample population can be based.
