Book excerpt: This is the first book on Einthoven in the English language; two earlier extensive reviews of his life and work were published in Dutch by his former co workers Hoogerwerf and de Waart, in 1946 and 1957 respectively; the latter provides a brief summary in English; the former also wrote a succinct article on 73 Einthoven in English. 74 Hoogerwerf's review mentioned above was published as a chapter in "Helden der Wetenschap" i. e. "Heroes of Science," a book on Dutch Nobel Prize winners which has a somewhat romantic character corresponding 108 with the title. On the other hand, de Waart's book gives more scientific and technical details, and provides full reference to Einthoven's papers, his assistants and the doctoral theses written under Einthoven's guidance. On the whole there fore the latter is the best source of information and I have used it often and gratefully. At the same time it requires more effort to read; the other book may have had a wider reading public. The main reason however for which I decided to write a book on Eintho ven and was encouraged to do so by the Einthoven Foundation, is not the want of an English text, but the fact that more information has become available in the meantime.

The wide range of experimental studies conducted by Einthoven and his fellows in the fields of (amongst others), eye function, the nervous system, vagal nerves and respiration led to the award of the Nobel Prize on Physiology and Medicine. The results of his experiments and electrocardiographic recordings of patients from 1886 to 1927 were registered on hundreds of "glass negatives". These have remained stored in the Boerhaave State Museum, Leiden. Now, for the first time these recordings have been carefully analyzed and considered in their historical context. In that year, I visited the University of Minnesota to discuss collaborative opportunities in cardiac anatomy, physiology, and medical device testing. The meeting was with a faculty member of the Department of Anesthesiology, Professor Paul Iaizzo. I didn’t know what to expect but, as always, I remained open minded and optimistic. Little did I know that my life would never be the same. . . . During the mid to late 1990s, Paul Iaizzo and his team were performing anesthesia research on isolated guinea pig hearts. We found the work appealing, but it was unclear how this research might apply to our interest in tools to aid in the design of implantable devices for the cardiovascular system. As discussions progressed, we noted that we would be far more interested in reanimation of large mammalian hearts, in particular, human hearts. Paul was confident this could be accomplished on large hearts, but thought that it would be unlikely that we would ever have access to human hearts for this application. We shook hands and the collaboration was born in 1997. In the same year, Paul and the research team at the University of Minnesota (including Bill Gallagher and Charles Soule) reanimated several swine hearts. Unlike the previous work on guinea pig hearts which were reanimated in Langendorff mode, the intention of this research was to produce a fully functional working heart model for device testing and cardiac research.

Book excerpt: Electrocardiography is a mature discipline, so familiar to both doctors and patients that it's hardly noticed, one of those tests that have always been there, like the white count and hemoglobin, not something one has to think about much, or question. To some extent this view is valid, but it overlooks some important points. Like the white count and hemoglobin, electrocardiograms are produced by technicians using mechanical devices that turn out numbers, but there is a difference. The white count and hemoglobin are reported as single values to be interpreted by the doctor who knows the patient and ordered the test, but the graph produced by an EKG machine represents millions of numbers displayed as XY plots, a message written in a language different from one's own. It requires transla tion, and this means that the translator must not only know the lan guage, but also be able to assess the effects on it of the many factors that may have modified its meaning between origin and delivery. There is potential for harm to the patient, as well as for help, in every facet of the process, and to lose sight of this, to see the tracing as a single whole, would be like seeing words as units without con sidering the letters that compose them. When we read, we do recog nize whole words, patterns, but, having learned the letters first, revert to this base intuitively when we encounter a new word, or one that is misspelled.
