Book excerpt: Mental health clinicians often work with clients who have been through significant trauma, and clinicians face risks of traumatization themselves through exposure to the traumatic stories of clients. This indirect traumatization is known as vicarious trauma (VT), and research has shown this to be related to burnout in mental health workers. Mental health clinicians must find a delicate balance between work and family roles, and the present study aimed to determine how this work interference with family (WIF) can explain the relationship between VT and burnout, and how support from the organization can ameliorate the negative effects of VT on the balance between work and family. Questionnaire responses were anonymously collected and analyzed from 110 mental health professionals working in mental health organizations sampled from several counties around Ohio. Analyses of the responses supported the hypotheses that WIF explains the relationship between VT and burnout, but perceived organizational support (POS) does not moderate the relationship VT and WIF, therefore rejecting those hypotheses. On this basis, it is recommended that mental health organizations educate employees on balancing work and family roles and provide adequate resources such as paid time off to help clinicians avoid burnout. Further research is needed to determine if POS is more effective at preventing VT than addressing it.

Book excerpt: Burnout, vicarious trauma, and job-related stress have been associated with impaired functioning in various populations, including clinical psychologists that work with trauma survivors. For clinicians, these conditions may compromise wellness, increase emotional exhaustion and distress, impair clinical judgment and competency, and negatively impact clients. Trait mindfulness may be one protective factor against vicarious trauma and burnout in mental health providers. Mindfulness has been associated with positive coping, emotion tolerance, reduced negative affect, lessened perceived stress, and improved self-care. This study used a cross-sectional, correlational design and surveyed 116 doctoral level clinical and counseling psychology students and licensed psychologists on burnout (Maslach Burnout Inventory-Human Services Survey; Maslach et al., 1996), vicarious trauma (Impact of Events Scale-Revised; Weiss & Marmar, 1997), and trait mindfulness (Mindful Attention and Awareness Scale; Brown & Ryan, 2003). Controlling for age, trait mindfulness was found to be inversely associated with burnout and with vicarious traumatization. Exploratory analyses suggested that greater time in the field was associated with less burnout, greater sense of personal accomplishment, and less vicarious traumatization. These findings suggest that trait mindfulness may be a protective factor for burnout and vicarious traumatization in psychologists. Engaging in mindfulness practice may increase psychological and emotional health in clinicians.

Book excerpt: Vicarious traumatization (VT) describes the gradual, transformative shifts in internal experience that occur as a result of cumulative exposure to clients' trauma material. VT is thought to develop in the therapist due to empathic engagement with clients, resulting in profound disruptions in frame of reference. Because VT is conceptualized as a condition that develops due to frequent exposure to clients' traumatic material, a rapidly emerging body of theoretical literature suggests that clinicians can safeguard against VT by maintaining a more balanced workload (i.e., a caseload of clients with a variety of presenting problems) and limiting the number of trauma cases. However, the quantitative research base on VT is limited and has been plagued by several methodological shortcomings, most notably the lack of comparison groups of non-trauma clinicians. As such, a primary aim of the present study was to characterize the prevalence and severity of VT among one group of clinicians treating predominantly traumatized populations, and one group providing treatment for a wider variety of presenting issues. Further, a secondary aim of this project was to identify both therapist-level and occupational-level contributors to VT. In our cross-sectional, online survey study of 114 generalist mental health providers (Mage = 33.36, 75.4% female, 88.6% Caucasian) and 107 trauma clinicians (Mage = 42.66, 81.3% female, 86.9% Caucasian) recruited from various professional organizations, levels of VT were low and not significantly different between the two provider groups. Risk factors for VT included fewer years of experience, having a greater personal history of trauma, and a personal distress empathy style. Protective factors included a perspective-taking empathy style, problem-focused and emotion-focused coping styles, and high-quality supervision. When the VT construct was examined alongside similar (but conceptually different) occupational stress constructs of secondary traumatic stress and burnout, there was a high degree of overlap, indicating that VT may not be a distinct phenomenon or unique to working with trauma clients. Results suggest that claims about the deleterious effects of trauma therapy are likely overstated, thereby refuting the original conceptualization of VT. Future research directions and implications for prevention and intervention are discussed.
